[The roof of the anterior ethmoid: a locus minoris resistentiae in the skull base].

Because of its special anatomic features, the roof of the anterior ethmoid is a preferred area for frontobasal fractures as well as an area of hazard during ethmoid surgery. As clinical experience proves, the most critical area for lesions to occur is the vicinity of the anterior ethmoidal artery, especially where this leaves the dome of the ethmoid medially to reach the ethmoidal sulcus in the olfactory fossa. Complete ethmoidal specimen were investigated by the means of serial histological sections in the frontal plane. 40 anterior ethmoidal arteries were anatomically prepared and the specia

The facial features of holoprosencephaly in anencephalic human specimens. II. Craniofacial anatomy.

The craniofacial anatomy of two anencephalic human specimens with the facial features of holoprosencephaly is reported. Many of the malformations have distinctly holoprosencephalic or anencephalic features. Absence of the entire ethmoid complex, observed in both specimens of the present study, has rarely been described and, to our knowledge, has never been confirmed histologically. The significance of agenesis of the ethmoid bone in the development of normal and holoprosencephalic facial features has not been considered previously. On the basis of several morphological variations in the two sp…

Differing roles for TGF-β/Smad signaling in osteitis in chronic rhinosinusitis with and without nasal polyps.

BACKGROUND: Chronic rhinosinusitis (CRS) without nasal polyps (CRSsNP) and with nasal polyps (CRSwNP) is reported to involve different inflammatory processes in sinonasal mucosa and bone tissue, and these processes remain uncharacterized. OBJECTIVE: We aimed to investigate the molecular mechanisms of osteitis in Chinese patients with CRS to better understand the pathogenesis of CRS. METHODS: The study included 10 controls, 16 patients with CRSsNP, and 23 patients with CRSwNP. Ethmoid bone tissue samples were evaluated by histologic examination. Quantitative real-time reverse transcription po…

Is pyogenic ethmoidal osteitis the cause of complicated rhinosinusitis with subperiosteal orbital abscess?

Multiple theories were described concerning the pathogenesis of orbital infection in rhinosinusitis, but no theory was proved. Understanding the cause of complication can allow its proper management. We speculate that subperiosteal orbital abscess (SPOA) secondary to rhinosinusitis is similar to subperiosteal abscess associated with osteomyelitis of bone all over the body. The objective was to evaluate bony changes of the ethmoidal sinuses in complicated rhinosinusitis patients with SPOA. This prospective controlled study was performed on eight patients undergoing endoscopic sinus surgery drai

Orbital wall thickness and the spread of infection from the paranasal sinuses.

Ninety-three human skulls (80 adults and 13 children) have been examined and the extent of thin bone in the party walls between the orbit and the frontal, ethmoidal and maxillary sinuses has been assessed. Translucent bone is most often present in the lateral wall of the ethmoidal labyrinth and least often in the floor of the frontal sinus. In children such bone is present significantly less often in the roof of the maxillary sinus (P less than 0.001) than in adults. Computerized tomography scans and clinical data from 6 patients with orbital cellulitis were reviewed. In one of these an infero

[Computed tomographic and endoscopic analysis of the supraorbital ethmoid cell anatomy].

OBJECTIVE: To understand the anatomy of the supraorbital ethmoid cell and its relationship with the frontal sinus drainage pathway. METHODS: Five patients (5 sides) who had supraorbital ethmoid cell underwent endoscopic frontal sinus surgery. Computed tomographic (CT) scans of the sinuses were obtained in coronal and axial views. The frontal sinus ostium and the supraorbital ethmoid cell were endoscopically identified respectively. RESULTS: On coronal CT scans, the supraorbital ethmoid cell was a separate cell lateral to the frontal sinus. And on axial CT scans, it was lateral and posterior

The intraorbital arrangement of the anterior and posterior ethmoidal foramina.

This study, instigated by surgical observations, and mainly carried out on the dry bone, attempts to analyse both the depth and variability of the anterior and posterior ethmoidal foramina through which pass the ethmoidal aa. The surgical implications of this study relate not only to ligation of the ethmoidal aa. in certain cases of epistaxis, their embolisation and the endonasal microsurgery of the ethmoid, but also to the surgery of the anterior cranial fossa. The study is based on 200 anatomic observations.

Computed tomography in ethmoid cell trauma.

Clinical and radiological evaluation of 19 patients with injury to the ethmoid bones following cranio-facial trauma is presented. The correlation of ethmoid trauma and damage to neighbouring structures is discussed. Two cases with damage to the orbits are demonstrated. These findings indicate that orbital involvement is relatively infrequent in association with ethmoid cell trauma. However, when it does occur, severe complications, such as infection and loss of vision may result. Early evaluation by computed tomography is the key to accurate diagnosis and prevention of these complications. Com…

Ethmoid mucocele in an infant with a benign fibroosseous lesion.

We present a case of ethmoid mucocele occurring in a two-month-old child, and associated (presumably causally) with a benign fibroosseous lesion of the ethmoid bone. These two conditions may be more frequently associated than previously recognized; MR greatly facilitates their evaluation.

Aneurysmal bone cyst of the paranasal sinuses associated with fibrous dysplasia: CT and MR findings.

An aneurysmal bone cyst (ABC) in the paranasal sinuses is rare with only four previous cases having been described in the sphenoid and ethmoid sinuses. This report describes a case of an ABC involving the sphenoid and ethmoid sinuses as well as the upper nasal fossae. It is currently believed that a preexisting primary bone lesion, such as fibrous dysplasia, initiates an osseous arteriovenous fistula, which creates a secondary reactive lesion of bone, the ABC. These lesions, which are comprised of multiple cysts containing liquid blood and serous fluid, are optimally studied by a combination o

Variations of the ethmoid labyrinth and sphenoid sinus and CT imaging.

Three hundred macerated and partly isolated postmortem mid-facial bones were studied for the development, variations and dimensions of the structures of the nasal cavity. On 184 axial CT scans of bones (102 male, 82 female) from patients ranging in age from 1 to 90 years old, the dimensions of the ethmoid labyrinth and sphenoid sinus were studied in detail in order to determine which anatomic situation might be unsafe during clinical endoscopic interventions. Six anatomic variations were identified. Most unsafe for surgery seemed to be the following types: type III, in which the anterior diame

[Anatomic variations in the ethmoid sinus in patients undergoing dacryocystorhinostomy at the Hassan II de Fès Medical Center (study of 215 patients)].

INTRODUCTION: Although several studies have tried to identify the causes of failure of dacryocystorhinostomy (DCR) surgery, the ethmoid sinus remains an underestimated and little described source of failure. OBJECTIVE: To study anatomical relationship between the ethmoidal sinus, particularly the "Agger nasi" cell, with the lacrimal fossa in the North African population, little described in the literature. This study is based on the results of preoperative analysis of dacryo-computed tomography. PATIENTS AND METHODS: Analysis of preoperative computed tomography images of patients undergoing

An Aneurysmal Bone Cyst in the Skull Base.

An aneurysmal bone cyst (ABC) is believed to be attributable to intraosseous circulatory disturbance. An ABC is a vascular tumor of the bone caused by bony expansion after partial vascular occlusion and congestion. Most ABCs are found in adolescents (approximately 75% of ABCs are observed in patients under 20 years of age). The most common ABC sites are the long bones of the limbs, the vertebrae, and the cranial bone. Aneurysmal bone cysts in the skull base or ethmoid sinus have been but rarely reported worldwide. The authors report on a patient with a very large ABC in the skull base and the

[Resection of tumors involving the cranio-naso-orbital area via fronto-orbito-ethmoidal approach].

OBJECTIVE: To introduce a better surgical approach for the resection of tumors involving the anterior and middle skull base and the fronto-orbito-ethmoidal area. METHODS: A "T" form incision was made in the fronto-orbito-ethmoidal region and along the nasal pyramid down. Parts of ethmoid sinus, lamina papyracea, fronto-orbito bone and behind wall of frontal sinus were resected in order to expose the tumors in the anterior skull base and the fronto-orbito-ethmoidal region. Then, the tumor was resected partly under the operation microscope, protecting the neighbouring important structures, for

Paranasal sinus radiology, Part 2A: Ethmoidal sinuses.

The clinician must have a high level of suspicion in order to detect ethmoidal sinus disease. Survey plain film radiographic examination may provide information about gross changes in ethmoidal sinus transparency and the sinus walls. Conventional tomographic study will often aid evaluation of the ethmoidal sinus walls which may be obscured by overlying structures on plain films. Computerized tomography adds a new dimension to ethmoidal sinus study. By revealing not only bone changes but soft-tissue abnormality as well, more complete understanding of the disease process is gained. Computerized

The sinus septi nasi and other minor pneumatizations of the nasal septum.

The osseous nasal septum (NS) consists of the perpendicular plate of the ethmoid bone (PPE) and the vomer bone. Few studies evaluated the possibilities of septal pneumatization of the PPE, or adjacent to it. We aimed to observe the anatomical possibilities of NS pneumatizations. A retrospective lot of cone-beam computed tomography (CBCT) files was used. One hundred seventy-one CBCT files from 51 males and 120 females were documented. There were found 46 files that were null for septal pneumatization. The other cases (73.1%) had different septal pneumatizations extended from neighboring air spa…
