Liver function and histology, together with the haematological changes, were investigated in 12 patients with Felty's syndrome. Six of them were anaemic. The red cell mass was either normal or increased, and the anaemia was due to a considerable increase in the plasma volume. We describe a case of left upper eyelid necrosis due to Pseudomonas aeruginosa in a patient with Felty syndrome. This is a rare but potentially serious condition. In our patient, medical management and reconstructive surgery achieved a good outcome.

A 68-year-old woman with Felty's syndrome had massive hepatomegaly. Liver biopsy showed diffuse infiltration of the sinusoids with mature lymphocytes (sinusoidal lymphocytosis of the liver). Nodular regenerative hyperplasia of the liver and portal fibrosis were not found. Although liver involvement is frequent in association with Felty's syndrome, sinusoidal lymphocytosis of the liver is unusual.

RATIONALE: Felty syndrome (FS) is a rare type of rheumatoid arthritis, and its combined occurrence with liver cirrhosis was rarely reported. FS was easily misdiagnosed as autoimmune cirrhosis or myelodysplastic syndrome, which led to improper medication and serious consequences.

HISTORY AND FINDINGS: A 59-year-old asymptomatic man, first diagnosed to have rheumatoid arthritis 27 years ago, was admitted to hospital because of splenomegaly and neutropenia, first noted 2 years ago. OBJECTIVE: To estimate the incidence of cancer (especially lymphoproliferative malignancies) in patients with the Felty syndrome.
DESIGN: A retrospective cohort study.
SETTING: A computerized database of all discharge records for 1969 to 1990 from a Veterans Affairs hospital.
PATIENTS: 906 men with a discharge diagnosis of the Felty syndrome.

Immunofluorescence on polymorphonuclear cells (PMN) of patients with Felty syndrome (FS) revealed increased amounts of IgG, IgA, and IgM bound to the PMN surface compared with PMN of patients with rheumatoid arthritis alone. A patient with Felty syndrome underwent splenectomy because of persistent high fever, leg ulcer, and granulocytopenia. Subsequently, his rheumatoid synovitis went into prolonged remission. In addition, the leg ulcer healed; his WBC count, hemoglobin level, temperature, ESR, and weight returned to normal; the rheumatoid factor titer dropped substantially; and the antinuclear antibodies disappeared. These changes have persisted to the present time, more than 5 1/2 years after surgery.
