The role of carbon dioxide in the generation of human fetal breathing movements.

To determine the role of carbon dioxide in the generation of fetal respiratory movements, the effect of induced maternal hypocapnia and hypercapnia on fetal breathing movements, gross body movements, and fetal heart rate was studied in 12 healthy pregnant women near term. Patients were studied for a 1-hour control period breathing room air followed by four randomized 15-minute study periods with patients breathing either room air, a prepared gas mixture with 2% or 4% carbon dioxide, or undergoing controlled hyperventilation as determined by monitoring end-tidal PCO2. The percentage of time fet

Effects of naloxone on breathing movements during hypercapnia in the fetal lamb.

To determine whether endogenous opioids influence the fetal breathing response to CO2 we have investigated the effect of the opiate antagonist, naloxone on the incidence, rate, and amplitude of breathing movements during hypercapnia in fetal lambs in utero. In 20 experiments in six pregnant sheep (130-145 days gestation) hypercapnia was induced by giving the ewe 4-6% CO2-18% O2 in N2 to breathe for 60 min. After 30 min of hypercapnia either naloxone (13 experiments) or saline (7 experiments) was infused intravenously for the remaining 30 min. During hypercapnia breath amplitude increased from

Denervation of peripheral chemoreceptors decreases breathing movements in fetal sheep.

The role of the peripheral chemoreceptors in the control of fetal breathing movements has not been fully defined. To determine whether denervation of the peripheral chemoreceptors affects fetal breathing movements, we studied 14 chronically catheterized fetal sheep from 120 to 138 days of gestation. In seven fetuses the chemoreceptors were denervated by bilateral section of the vagus and carotid sinus nerves; in seven others, sham operations were performed. We compared several variables during two study periods: 0-5 and 6-13 days after operation. In the denervated fetuses there were significan

Fetal breathing movements and lung hypoplasia: preliminary human observations.

Hypoplasia of the fetal lung is found in association with long-standing oligohydramnios. In animal models, interference with fetal breathing activity has led to lung hypoplasia. It has been suggested that lung hypoplasia associated with oligohydramnios is due to inhibition of fetal breathing. Observations of seven patients with prolonged oligohydramnios, three of whom had lung hypoplasia, indicate that fetal breathing does occur in these cases and that is unlikely that lung hypoplasia is merely the result of absent fetal breathing activity. In fact, when compared to the four patients with olig

Fetal breathing movements and maternal hyperoxia in the growth retarded fetus.

The effect of fetal breathing movements (FBM) of maternal breathing of 50% oxygen was examined. The procedure caused a two-fold rise in maternal arterial Po2 and did not alter FBM in normal pregnancy. In pregnancy complicated by intrauterine growth retardation (IUGR) a substantial increase in the incidence of FBM was recorded during hyperoxia (control 29.8 +/- 3.9%, hyperoxia 68.2 +/- 3.6). No such change was observed in pregnancies where IUGR was suspected but not proven at delivery.

Fetal movements are related to maternal characteristics: a prospective ultrasound study.

BACKGROUND: Fetal movements are important indicators of well-being that are associated with stillbirth. Yet neither a universal definition nor gold standard exists for what constitutes normal or abnormal. Current monitoring methods such as maternal perception lack diagnostic accuracy, while ultrasound provides detailed assessment in real time that is not practical outside the clinic setting. Maternal perception remains the primary method of fetal movements detection in clinical practice. OBJECTIVES: To assess normal fetal movements patterns using real-time ultrasound and maternal perception a…

From abnormal fetal movements to neonatal seizures: A literature review.

Fetal seizures (FS) are underestimated and underdiagnosed events with a poor prognosis that could precede neonatal seizures. FS diagnosis could be clinical or by means of ultrasound. FS causes are heterogenic. After a PubMed, Google Scholar and SCOPUS research, aimed to find publications concerning FS, we selected 23 studies, mainly case reports with literature reviews, for a total of 29 patients. We evaluated the method of diagnosis, gestational age at diagnosis of FS, FS etiology, time of etiological diagnosis, fetal/neonatal outcome and possible therapeutic options. Fetal seizures were main

Predictive factors in pregnancies with reduced fetal movements: a pilot study.

BACKGROUND: Pregnancies with reduced fetal movements (RFM) are at risk for poor neonatal outcomes and stillbirth. AIM: To investigate whether Doppler measurements or angiogenic factors are good predictors of adverse neonatal outcomes in pregnancies with RFM. METHODS: This is a prospective pilot cohort study of 3243 women seeking care for RFM. Standard care was carried out in all cases. An extra Doppler examination was performed in 128 women to assess the flow in the middle cerebral artery, the umbilical artery, and the uterine artery. In 62/128 pregnancies, a maternal blood sample was obtain…

The outcomes of pregnancies with reduced fetal movements: A retrospective cohort study.

INTRODUCTION: The objective of this study was to examine the outcomes and interventions in pregnant women presenting with a perception of reduced fetal movements (RFM), and to determine if repeated episodes of RFM increase the risk of adverse outcomes. MATERIAL AND METHODS: This was a retrospective cohort study conducted in six NHS hospitals within the Thames Valley network region, UK and one neighboring hospital, an area with approximately 31 000 births annually. All women with a primary presentation of perceived RFM after 24 completed weeks of gestation during the month of October 2016 were…

Fetal and infant spontaneous general movements as predictors of developmental disabilities.

The challenge of identifying infants who are at risk for developmental delay and possible adverse neurodevelopmental outcome demands methods of evaluation that will lead to early intervention to minimize developmental disability and to maximize the infant's potential. A qualitative assessment of spontaneous general movements (GMs) in the preterm, term, and young infant at risk is a valid and reliable tool for evaluation (Prechtl [1990] Early Hum. Dev. 23:151-158). The aim of this review is to describe the theoretical and clinical bases for the assessment of GMs and its relationship to developm

Effects of morphine and naloxone on fetal heart rate and movement in the pig.

To test the hypothesis that an increasing opioid tonus is involved in decreases in fetal heart rate (FHR) and movement (FM) during late gestation, we studied the effects of intravenous bolus injections of morphine (1 mg) and naloxone (1 mg) on FHR and FM in the fetal pig. Twenty-one fetuses (1 per sow) were catheterized at 90-104 days of gestation (median 100 days). Recordings of FHR (electrocardiograph or Doppler-derived signals) and FM (ultrasonography) were made from 15 min before to 45 min after treatment. Morphine administration significantly decreased FHR, but it increased FHR variation

Fetal breathing movements before oxytocin induction in prolonged pregnancies.

Fetal breathing movements were recorded before elective labor induction in 65 patients with uncomplicated prolonged pregnancies. Breathing activity was observed in 41 patients and was absent in 24. Comparison of these two groups indicated that the induction period was significantly shorter and the oxytocin requirement significantly lower in the group with no fetal breathing movements (mean, 319.3 vs 648.5 minutes and 1133.7 vs 2708.4 mIU, respectively). These findings remained valid regardless of Bishop scores. The data suggest that fetal breathing movements are predictive of the effectiveness

A transducer for recording fetal movements and sounds based on an inductive principle.

Recording fetal breathing movements requires a transducer with a large dynamic range. The compliance of the transducer should be matched to the compliance of the maternal abdominal wall in order to pass the very weak acoustic signal caused by heart sounds and fetal breathing movements on to the transducer. The output signal is stored on the hard disc of a personal computer (PC) simultaneously with a control signal indicating the presence of breathing movements. Ultrasound imaging is exclusively used for verification of breathing movements. The images are stored on videotape simultaneously with

Quality of fetal heart rate monitoring with transabdominal fetal ECG during maternal movement in labor: A prospective study.

INTRODUCTION: Transabdominal electrocardiographic (TAfECG) acquisition of fetal heart rate (FHR) signals has recently been introduced into leading commercial cardiotocographic (CTG) monitors. Continuous wireless transmission of signals has raised the possibility of the technology being used during maternal mobilization in labor. This study aims to evaluate signal quality and accuracy of TAfECG acquisition of FHR signals during static and active maternal positions in labor when compared with Doppler signals and with the gold-standard method of fetal scalp electrode (FSE). MATERIAL AND METHODS:
