Rheumatoid arthritis is characterized by hypertrophic synovitis destroying the involved joints. If medical treatment fails to control the synovitis, synovectomy is a worthwhile prophylactic procedure that might help to delay further distention of the joint capsule and ligament. Degenerative changes of the finger joints often lead to various surgical treatments such as arthrodesis or artificial joint replacement. There are a lot of surgical approaches for surgeons to choose from. The different approaches with their characteristic advantages and disadvantages during surgery and for the postoperative treatment should be selected and applied based on patient-, implant- and aftercare-specific indications. This article presents a short review of possible approaches and associated surgical techniques for finger joint replacement.

BACKGROUND: Simultaneous dislocation of the proximal and distal interphalangeal joints of the same finger is a rare event. In literature simultaneous double interphalangeal dislocation in the same finger has been reported only one case in a skeletally immature patient which was a fourteen year old male. Locking of the second metacarpal joint is common, but locking of other metacarpal joints is unusual. We present a case, in which the joint of the third finger locked and the mechanism was an osteophyte catching of the ulnar accessory collateral ligament. In such a case, the joint should be explored through a palmar approach. Conservative treatment is not recommended.

A consecutive series of 44 patients with septic arthritis of finger joints is presented. The patients were treated with antibiotics, incision, debridement and usually drainage. In 13 patients with destruction of the joint cartilage, an arthrodesis was made at the time of incision. The failure rate with conventional therapy was 42 per cent. OBJECTIVE: To report the MR imaging findings of painful injured metacarpophalangeal (MCP) joints of the fingers.
DESIGN AND PATIENTS: MR imaging of 39 injured MCP joints in 38 patients was performed after a mean delay of 8.8 months. The MR images were obtained with the fingers in extended and flexed positions using T2-weighted and T1-weighted sequences before and after intravenous injection of a gadolinium compound.
