Leveraging Emergency Relief Funding for Behavioral Health Workforce Development: Policy Lessons From Nebraska's Experience.

The authors describe Nebraska's implementation of $25.5 million in State and Local Fiscal Recovery Funds of the American Rescue Plan Act through the Behavioral Health Education Center of Nebraska, a state-funded workforce center housed in an academic medical center. Funds were allocated across four workforce priorities: training and education, tele-behavioral health, COVID-19 workforce projects, and financial incentives for clinical supervision. Embedded program evaluation, technical assistance, and an equity-focused application review process enabled rapid administration, accountability, and

WASH-nutrition integration as a global policy priority for antimicrobial stewardship in low- and middle-income countries.

Antimicrobial resistance (AMR) is a global health threat predicted to hit 10 million deaths per year by 2050. Most conventional antimicrobial stewardship (AMS) strategies focus on clinical settings, failing to address community-level overuse, a primary driver of AMR in low- and middle-income countries. This commentary leverages novel evidence to argue for the prioritization of water, sanitation, and hygiene (WASH)-nutrition integration as a global health policy and first-line defense strategy against AMR. This argument is grounded on a target article that utilized causal mediation analysis to

Comorbidity status in hospitalized elderly in Japan: Analysis from National Database of Health Insurance Claims and Specific Health Checkups.

The detailed comorbidity status of hospitalized elderly patients throughout Japan has remained largely unknown; therefore, our goal was to rigorously explore this situation and its implications as of the 2015 fiscal year (from April 2015 to March 2016). This study was based on a health insurance claims database, covering all insured policy holders in Japan aged ≥60 years (male: n = 2,135,049, female: 1,969,019) as of the 2015 fiscal year. Comorbidity status was identified by applying principal factor analysis to the database. The factors identified in male patients were [1] myocardial infarcti

Financial crisis and austerity measures in Greece: their impact on health promotion policies and public health care.

This review study explores the available data relating to the impact of financial crisis and subsequently applied austerity measures on the health care, social services and health promotion policies in Greece. It is evident that Greece is affected more than any other European country by the financial crisis. Unemployment, job insecurity, income reduction, poverty and increase of mental disorders are among the most serious consequences of crisis in the socioeconomic life. The health system is particularly affected by the severe austerity measures. The drastic curtailing of government spending h

Obesity prevention and sustainable food systems: rethinking the nexus through a health in all policies approach.

Obesity and the unsustainability of food systems are intertwined global challenges that demand coordinated solutions. Diets high in energy-dense, ultra-processed foods both elevate obesity risk and carry substantial environmental footprints; meanwhile, policies that prioritize volume and convenience entrench resource-intensive production and obesogenic food environments. Using a systems lens, we map feedbacks across supply chains, food environments, and social inequities that couple obesity and environmental degradation. We propose the Health in All Policies (HiAP) framework to align health an

The longevity dividend: A systems-thinking framework for harnessing the silver economy in low- and middle-income countries.

Population ageing is accelerating in low- and middle-income countries, where accumulated life-course inequalities, labour informality, and limited social protection shape conditions in later life. Conventional policy narratives have framed older adults as fiscal burdens, overlooking the productive and social value embedded in extended healthy life years and the structural determinants that constrain participation. This narrative review introduces the 'longevity dividend', a systems-thinking conceptual framework that positions functional ability across the life course as the central objective o

Trends in Upper Extremity Injuries in Female Marines following the Addition of the Pull-up as a Physical Fitness Test Event.

INTRODUCTION: In November 2012, the U.S. Marine Corps announced a policy change to the female Physical Fitness Test (PFT), replacing the flexed arm hang with pull-ups. This change, which became effective on January 1, 2017, aimed to standardize fitness requirements and facilitate gender integration into combat roles. Musculoskeletal injuries (MSKIs) are a leading cause of medical encounters and non-deployability among service members. Upper extremity (UE) injuries are particularly relevant given the physical demands of pull-ups. MATERIALS AND METHODS: A retrospective open cohort study was con

The role of regulatory policies on unhealthy commodities to prevent and control noncommunicable diseases in the Americas.

The growing burden of noncommunicable diseases (NCDs) cannot be addressed through clinical interventions alone. Population-level regulatory interventions-such as fiscal measures, marketing restrictions, and labeling regulations-are central to effective NCD prevention and control. This article addresses the role of regulatory policies in preventing NCDs, with a focus on progress and implementation gaps in the Americas. In the context of NCDs, unhealthy commodity industries, including companies that produce and promote tobacco, alcohol, and ultra-processed food products, play a significant role

Changes in Reproductive and Sexual Health Diagnoses of Women Veterans Using Department of Veterans Affairs Health Care Between FY2010 and FY2018.

BACKGROUND: Women veterans are the fastest growing group of new Veterans Health Administration (VA) users, with more than half a million women veterans using VA care in 2018. VA has implemented policies and programs to meet the reproductive and sexual health (RSH) needs of women veterans who increasingly rely on VA to provide comprehensive care. OBJECTIVE: We aimed to describe changes in the prevalence of RSH diagnosis categories among women veterans using VA care in fiscal year (FY) 2010 and FY2018. METHODS: Repeated cross-sectional VA administrative data were used to identify women veteran

Integrated policy of medical expense subsidies and clinical registry for patients with liver cancer and decompensated cirrhosis in Japan.

Chronic hepatitis B and C are among the most significant infectious diseases worldwide, and are major risk factors for liver cirrhosis and liver cancer. In Japan, comprehensive hepatitis measures are implemented for the testing and treatment of viral hepatitis, thus enabling the early diagnosis of liver cancer. Nevertheless, patients with decompensated cirrhosis and liver cancer often have unfavorable prognoses and require repetitive long-term treatment. In fiscal year 2018, an integrated policy of medical expense subsidies and research was established in Japan that aimed to alleviate patients

Effect of Austerity Measures on Infant Mortality: Evidence From Greece.

Governments frequently adopt austerity policies when facing economic crises, yet their long-term consequences for population health remain incompletely understood. This paper examines the impact of large-scale fiscal austerity on infant mortality by exploiting the Troika-led economic adjustment program implemented in Greece beginning in 2010 as a quasi-experimental shock. Using the synthetic control method, we construct a counterfactual for Greece based on OECD and Union for the Mediterranean countries that did not experience austerity of comparable depth or duration. Relative to this counterf

Digital inclusion in China's national smart health and older adult care policy design: an access-oriented and uneven embedding pattern, 2013-2025.

BACKGROUND: Population ageing and rapid digitalisation of health and care services have raised concerns about whether older adults are adequately considered in digital policy design. Although digital inclusion is an increasingly visible policy goal, little is known about how national policy instruments embed digital inclusion or respond to older adults' multidimensional needs. METHODS: We developed a Policy Alignment Framework linking policy instruments, digital inclusion orientations, and older adults' needs. A quantitative content analysis was conducted on 35 national-level smart health and

Demographic and economic drivers of healthcare system capacity in Italy: A local projection approach.

RESEARCH IN CONTEXT: What is already known about the topic? Physician density is influenced by demographic and macroeconomic factors. However, most existing studies analyze these determinants in isolation or focus on short-term correlations, providing limited insight on their dynamic interaction over time. What does this study add to the literature? This study provides the first long-run analysis of physician density in Italy across distinct institutional regimes: the pre-National Health Service (NHS) period (1953-1978), the centralized NHS era (1979-2001) and the decentralized regional phase

Government exposure and corporate ESG greenwashing: Evidence from Chinese listed firms.

In government-led economies, ESG metrics increasingly serve as key criteria for allocating fiscal subsidies and green finance, yet whether deeper government-firm administrative interactions curb or foster corporate greenwashing remains unresolved. Using a panel of 12,854 firm-year observations from Chinese A-share listed firms over 2010-2023, this study constructs a continuous, dynamic government exposure index by identifying the sentence-level co-occurrence of government agency names and administrative action verbs in annual reports, thereby capturing the intensity of firms' interactions with

Avoidable pitfalls on the path to health financing self-reliance in low-income and middle-income countries.

Low-income and middle-income countries (LMICs) are facing an urgent and complex challenge: how to transition to greater self-sustainability in health financing amid declining donor support. While this shift is inevitable, the policy responses it elicits carry significant implications for health system equity and access. This commentary highlights four policy choices increasingly observed in LMICs that we argue are unacceptable in the pursuit of sustainability. These include: (1) shifting the financial burden to out-of-pocket payments; (2) over-reliance on contributory health insurance schemes;

Hospital-Acquired Infections Under Pay-for-Performance Systems: an Administrative Perspective on Management and Change.

PURPOSE OF REVIEW: The purpose of this review is to explore the impact of hospital-acquired infection on payment under pay-for-performance systems, and provide perspective on the role of administrators in infection prevention. RECENT FINDINGS: Hospital-acquired infections continue to pose a serious threat to patient safety and to the fiscal viability of healthcare facilities under pay-for-performance systems. There is mixed evidence that use of pay-for-performance systems leads to prevention of hospital-acquired conditions. Use of evidence-based guidelines has been shown to reduce hospital-ac

Aid, debt, International Monetary Fund conditionalities and domestic health financing in low- and middle-income countries.

Across low- and middle-income countries (LMICs), public external debt burdens as well as the number of International Monetary Fund (IMF) loan conditionalities have grown over time. These externally derived macro-fiscal factors, along with official development assistance (ODA), may influence the fiscal space for health and policy decisions that co-determine to what extent countries finance their health systems with domestic government funds (GHE-S), and to what extent they rely on household out-of-pocket payments (OOP). The levels and balance of these sources have great implications for health

Realpolitik, real evidence: what comparative case study methodology teaches us about the policy mixes and political traction needed to reduce health inequalities.

Public health scholarship is shaped by an evidence hierarchy that places systematic reviews of experimental and quasi-experimental studies at the top. This approach is invaluable for isolating the effects of discrete, often downstream interventions. However, it is less suited to evaluating upstream and programmatic efforts to reduce health inequalities at scale, since these methods rarely illuminate how combinations of policies shape inequalities or how such combinations achieve and sustain political and public support. These limitations are not merely technical. Many consequential upstream re

The case for investing in the prevention and treatment of overweight and obesity among children and adolescents in three middle-income countries.

BACKGROUND: Child and adolescent overweight and obesity rates are increasing rapidly, notably in middle-income countries (MICs). Effective policy adoption has been limited in low-income and middle-income countries. To respond, investment cases were developed in Mexico, Peru, and China to understand the health and economic returns on investment in childhood and adolescent overweight and obesity interventions. METHODS: The investment case model applied a societal perspective to predict the health and economic impact of childhood and adolescent overweight and obesity in a cohort aged 0-19 years,
