Obesity promotes urinary tract infection by disrupting bladder focal adhesion kinase signaling.

Obesity is a significant public health concern associated with numerous health risks. Infections are a major complication of obesity, but the mechanisms driving increased infection risk remain unclear. Using a diet-induced obesity mouse model in male and female C57BL/6 mice, we show that high-fat diet feeding heightens vulnerability to uropathogenic E. coli (UPEC) urinary tract infection. Transcriptomic analysis of bladder urothelial cells revealed sex-specific gene expression changes with the shared activation of focal adhesion and extracellular matrix pathways. Western blotting and immunosta

Parechovirus-A3 encephalitis presenting with focal seizure mimicking herpes simplex virus infection.

BACKGROUND: Febrile neonates and young infants presenting with seizure require immediate evaluation and treatment. Herein we experienced two young infants with parechovirus-A3 (PeV-A3) encephalitis, initially presented with focal seizure suspecting herpes simplex virus (HSV) encephalitis. CASES: We have experienced 2 infantile cases, initially presented with focal seizure. At presentation, HSV encephalitis was strongly suspected and empiric acyclovir therapy was started; however, serum and/or cerebrospinal fluid (CSF) PCR for HSV were negative. Instead, serum and/or CSF PCR for parechovirus-A

Paediatric focal intracranial suppurative infection: a UK single-centre retrospective cohort study.

BACKGROUND: Paediatric focal intracranial suppurative infections are uncommon but cause significant mortality and morbidity. There are no uniform guidelines regarding antibiotic treatment. This study reviewed management in a tertiary healthcare centre in the United Kingdom and considers suggestions for empirical treatment. METHODS: A retrospective, single-centre cohort review of 95 children (< 18 years of age) with focal intracranial suppurative infection admitted between January 2001 and June 2016 in Newcastle upon Tyne, United Kingdom. Microbiological profiles and empirical antibiotic regim

Possible association of Henoch-Schönlein purpura in adults with odontogenic focal infection.

BACKGROUND: Odontogenic focal infection (OFI) is considered to be an important precipitating factor in some cases of palmoplantar pustulosis and psoriasis. In Henoch-Schönlein purpura (HSP), it is clear that acute bacterial infection is one of the causes. However, it remains unclear how OFI participates in the pathogenesis of HSP. METHODS: To clarify how OFI is related to the disease activity of HSP in terms of skin lesions and organ involvement, we conducted a retrospective study of 28 cases of adult-type HSP. RESULTS: Panoramic x-ray screening detected OFI in 14 of the 28 patients. Patient

Early responses to Salmonella typhimurium infection in mice occur at focal lesions in infected organs.

Salmonella typhimurium causes an invasive disease in mice that has similarities to human typhoid, with key roles for cytokines and possibly also inducible nitric oxide synthase (iNOS), in mediating host responses to infection. In this paper we demonstrate that iNOS mRNA, protein and enzyme activity is induced within spleens and livers of infected mice as early as 5 h post-infection. Immunohistochemistry and in situ hybridization indicated that iNOS expression occurs predominantly in macrophages in localized, discrete foci in the infected organs. iNOS activity in spleen and liver was not detect

Focal intradural brain infections in children: an analysis of management and outcome.

INTRODUCTION: Focal intradural infections of the brain include empyema and abscess in the supratentorial and infratentorial spaces. These are amenable to surgical management. Various other issues may complicate the course of management, e.g. hydrocephalus with infratentorial lesions or cortical venous thrombosis with supratentorial lesions. Here, we review the management and identify factors affecting outcome in these patients. MATERIALS AND METHODS: This is a retrospective analysis of all children (aged <18 years) treated at the National Institute of Mental Health and Neurosciences, Bangalor

Focal segmental glomerulosclerosis in primates infected with a simian immunodeficiency virus.

Focal and segmental glomerulosclerosis (FSG) with endothelial tubuloreticular inclusions (TRIs) is the typical lesion of human HIV-associated glomerulopathy. Autopsy studies showed the presence of FSG in 3 of 15 macaques dying 15-120 weeks after experimental infection with a simian immunodeficiency virus (SIVMne). Ultrastructural studies generally revealed numerous endothelial TRIs (also present in normals), mesangial expansion, and evidence of mesangial cell injury. One additional animal had a small-vessel polyarteritis with a proliferative and focally crescentic glomerulonephritis; seven ani

Focal neurologic deficits in infective endocarditis and other septic diseases.

INTRODUCTION: Focal neurologic deficits in sepsis frequently result from parenchymal lesions due to cerebral embolism. The aim of this study was to characterize clinical, laboratory and radiologic patterns of those patients. PATIENTS AND METHODS: Medical records of 30 patients with focal neurologic symptoms during sepsis were analyzed retrospectively. RESULTS: 24 patients (22 with infective endocarditis) had ischemic stroke. Cerebrospinal fluid (CSF) analyses revealed inflammation in 11 of 12 patients. Patients who died (11/24) suffered more frequently from secondary intracerebral hemorrhage

Persistence of positive Brucella Melitensis blood cultures is not associated with focal infection.

OBJECTIVES: Brucellosis is a global zoonotic disease complicated by focal infections requiring treatment modifications. In other bacterial infections, persistent positive blood cultures prompt evaluation for metastatic foci. We aimed to assess whether persistent bacteremia predicts focal infections in patients with Brucella melitensis infection. METHODS: We conducted a retrospective cohort study from 2010 to 2022. We included adults hospitalized due to Brucella melitensis bacteremia. Patients were categorized into persistently positive (PFUBC), negative (NFUBC), or no follow-up cultures. The

Acetylcysteine-decorated Prussian blue nanoparticles for strong photothermal sterilization and focal infection treatment.

The major challenge in bacterial infection in clinical settings is the development of antimicrobial materials in the treatment of drug-resistant bacteria. Herein, we report a new strategy for efficient near-infrared radiation (NIR) photothermal sterilization and focal infection treatment by acetylcysteine-modified Prussian blue nanoparticles (AC-PB). Specifically, AC-PB is fabricated as a multifunctional therapeutic agent via a co-precipitation approach, where PB acts as an effective photothermal agent and AC could prevent the formation of bacteria cluster in biofilms and the bacterial adhesio

Adenovirus infection associated with focal segmental glomerulosclerosis following autologous hematopoietic stem cell transplantation.

Disseminated adenovirus infection is a rare but often fatal complication of hematopoietic stem cell transplantation (HSCT), particularly uncommon in the autologous setting. We report the case of a 59-year-old man with stage IVB diffuse large B-cell lymphoma who underwent autologous HSCT after multiple lines of therapy. Following conditioning regimen, he developed persistent fever, mucositis, dysuria, and gross hematuria, with imaging showing bilateral pyelonephritis, ureteritis, and cystitis. Despite broad-spectrum antibiotics, symptoms persisted. Quantitative PCR revealed rapidly rising adeno

Collapsing focal segmental glomerulosclerosis probably triggered by dengue virus infection - two case reports.

The reported cases describe the association between collapsing focal segmental glomerulosclerosis (FSGS) and acute dengue virus infection. In both cases, patients were diagnosed with dengue virus infection and had a severe kidney disease, with nephrotic syndrome and acute kidney injury. Kidney biopsy was performed and showed collapsing FSGS. The first patient, a 27-year-old man, was diagnosed with dengue virus infection and developed nephrotic syndrome after two weeks of illness. He was treated with methylprednisolone for three days and intravenous furosemide. This patient evolved well, althou

Focal Epithelial Hyperplasia in Adult Patients With HIV Infection: Clearance With Topical Imiquimod.

Case 1 A 41-year-old man with human immunodeficiency virus (HIV) 1 diagnosed 16 years prior to his consultation was referred for an 8-month history of multiple painless lumps in his mouth. He had A2 status (CD4 cell count of 273 cells/mm3 and viral load of 43,000 copies/L) and was taking treatment with lamivudine/zidovudine (combivir) and efavirenz. Physical examination showed multiple small mucosal-colored and lobulated papulonodules located in the palate and lower gingiva and a whitish verrucous plaque on the lower labial mucosa (Figure 1a). The lesions were diagnosed clinically as focal epi
