Epidemiologic and clinical aspects of shigellosis in American forces deployed to Saudi Arabia.

I describe nine cases of shigellosis seen at a large Navy field hospital in troops deployed to Saudi Arabia during the Gulf War. Eight cases were associated with consumption of food at civilian restaurants or civilian-contracted food services. Two adults had "pseudomeningitis" syndromes, illustrating that shigellosis in adults may initially be misdiagnosed. All patients responded rapidly to rehydration and oral fluoroquinolone (ciprofloxacin) therapy. Five patients received an antimotility agent without apparent adverse effect.

Association of trainee involvement in an acute pain service with postoperative opioid use in patients undergoing cytoreductive surgery with hyperthermic intraperitoneal chemotherapy.

BACKGROUND: Several hospitals have implemented a multidisciplinary Acute Pain Service (APS) to execute surgery-specific opioid sparing analgesic pathways. Implementation of an anesthesia attending-only APS has been associated with decreased postoperative opioid consumption, time to ambulation, and time to solid food intake for patients undergoing cytoreductive surgery with hyperthermic intraperitoneal chemotherapy. In this study, we evaluated the impact of introducing an APS trainee on postoperative opioid consumption in patients undergoing hyperthermic intraperitoneal chemotherapy during POD

Carbapenemase-producing rare Enterobacterales in hospital food: a Latin American one health blind spot.

INTRODUCTION: Carbapenemase-producing Enterobacterales (CPE) are a global public health concern. These bacteria are very common in human infections, but reports of their presence in the food chain are increasing and could represent alternative routes for colonization and infection. Hence, we conducted a surveillance study to investigate CPE in food distributed to patients at a tertiary hospital in Brazil. METHODS: The samples were collected right before distribution and screened for β-lactam-resistant Enterobacterales. Isolates that exhibited carbapenem resistance were screened for NDM, IMP,

[Grievances in South African hospitals: from a nursing service management perspective].

The nursing service manager is accountable for adequate and efficient personnel management in the nursing service and the management of grievances is an important aspect of the personnel management function. The question arises, however, how and when grievances in nursing services arose and developed? The purpose of this article is to give a historical description of the development and management of grievances in nursing services for the time frame 1652-1990. An historical analysis was undertaken by means of news paper analysis, as well as other written resources. The results show that the de

Hospital outbreak of Salmonella virchow possibly associated with a food handler.

A foodborne outbreak of salmonella infection at a private hospital in London in 1994 was found to be associated with eating turkey sandwiches prepared by a food handler. One patient, nine staff, and a foodhandler's baby were confirmed to have Salmonella enterica serotype virchow, phage type 26 infection. The attack rate was estimated to be 5% among the approximately 200 patients and staff at risk. A food handler reportedly became ill days after, but her baby days before, the first hospital case. Although it appeared to be a single outbreak, antibiogram analysis, supplemented by plasmid profile

A review of food provision to a renal ward and the proposed appointment of feeding assistants.

Malnutrition among renal patients has been widely documented and is associated with increased morbidity and mortality. Advances in dialysis technology and transplantation have helped to increase patient long-term survival, and as more elderly patients commence dialysis programs, the problem of malnutrition is escalating. Hospitalized renal patients are at a greater risk, as dietary intakes may be reduced for a number of reasons. A multidisciplinary team decided to review the existing cook-chill plated meal system and all food provision to the renal ward. The aim of this review was to assess th

Hospital-acquired listeriosis linked to a persistently contaminated milkshake machine.

One case of hospital-acquired listeriosis was linked to milkshakes produced in a commercial-grade shake freezer machine. This machine was found to be contaminated with a strain of Listeria monocytogenes epidemiologically and molecularly linked to a contaminated pasteurized, dairy-based ice cream product at the same hospital a year earlier, despite repeated cleaning and sanitizing. Healthcare facilities should be aware of the potential for prolonged Listeria contamination of food service equipment. In addition, healthcare providers should consider counselling persons who have an increased risk
