Consensus on Indications for Medial Cuneiform Opening Wedge (Cotton) Osteotomy in the Treatment of Progressive Collapsing Foot Deformity.

RECOMMENDATION: Forefoot varus is a physical and radiographic examination finding associated with the Progressive Collapsing Foot Deformity (PCFD). Varus position of the forefoot relative to the hindfoot is caused by medial midfoot collapse with apex plantar angulation of the medial column. Some surgeons use the term forefoot supination to describe this same deformity (see Introduction section with nomenclature). Correction of this deformity is important to restore the weightbearing tripod of the foot and help resist a recurrence of foot collapse. When the forefoot varus deformity is isolated

Resection Arthroplasty: Current Indications and Tips.

Resection arthroplasty for metatarsalgia is a selective procedure primarily indicated for patients with rheumatoid arthritis. These patients present with significant forefoot deformities, poor bone quality, and loss of soft tissue integrity. Resection of the metatarsal heads and correction of lesser toe deformities improve pain and decrease transfer metatarsalgia. Patients with concurrent hallux valgus may benefit from a lapidus procedure or hallux metatarsophalangeal fusion in an effort to improve outcomes and decrease incidence of recurrent hallux valgus. In rare cases, diabetics with neurop

Scarf osteotomy for correction of hallux abducto valgus deformity.

The scarf osteotomy has been used and researched extensively for many years for the correction of hallux valgus deformity in both the adolescent and adult populations. It is an inherently stable construct, which allows for early weight bearing and early return to activities of daily living. The scarf procedure has a wide array of surgical indications with great reproducibility and a low complication rate, and it can be performed bilaterally simultaneously, with long-term predictability. Once the scarf procedure is mastered, it is a rewarding and predictable operation for both the surgeon and p

Effects of the lapidus arthrodesis and chevron bunionectomy on plantar forefoot pressures.

Hallux valgus with or without first ray insufficiency has been strongly implicated as a contributing factor in lesser metatarsal overload. The principle goals of a bunionectomy are to relieve the pain, correct the deformity, and restore first metatarsophalangeal joint congruity. Until now, little evidence has been available to assess the effects of bunionectomy procedures on forefoot pressure. The primary aim of the present prospective study was to evaluate the preoperative and postoperative plantar pressures after 2 specific bunionectomies: the chevron bunionectomy and Lapidus arthrodesis. Inter-segment foot kinematics during cross-slope running.

Cross-slopes are a common terrain characteristic, however there is no biomechanical knowledge of the intra-foot adaptations required for running on these surface inclinations. The purpose of this study was to evaluate the kinematic changes induced within the foot while running on a transversely inclined surface. A three-segment foot model distinguishing between the hindfoot, forefoot, and hallux was used for this purpose. Nine healthy experienced male runners volunteered to perform level (0°) and cross-slope (10°) running trials barefoot at a moderate speed. Multivariate analysis of variance (

An atypical composition of adductor hallucis co-existent with an accessory plantar muscle and duplication of flexor digiti minimi pedis.

Adductor hallucis (AH) is frequently employed in transposition onto the first metatarsal head to alleviate symptoms of hallux valgus. Although tis muscle receives sufficient anatomical description, it is often disregarded in clinical practice. The rare anomalies associated with AH, merit a detailed study owing to its contribution to the treatment of hallux valgus. Additionally, the AH may be employed in plastic and reconstructive surgeries as a graft to cover defects of the foot. Therefore, we sought to report this infrequent anomaly relating to the AH muscle. Through the course of a regular e

Relationship between plantar pressure and soft tissue strain under metatarsal heads with different heel heights.

BACKGROUND: Metatarsalgia due to increased plantar pressure under the metatarsal head (MTH) is often seen with wearing high-heeled shoes. However, the concomitant soft tissue strain has not been well discussed. The objective of our study was to explore the standing plantar pressure and corresponding soft tissue strain under the metatarsal heads (MTHs) with different heel heights. MATERIALS AND METHODS: Twenty-one healthy subjects (10 male and 11 female) participated. The plantar pressure and soft-tissue thicknesses were measured simultaneously with a load cell and a 10-MHz linear-array ultras

Rheumatoid necroses in the forefoot.

BACKGROUND: The incidence and morphological features of rheumatoid necroses (RN) in the forefoot are unknown. METHODS: Biopsy material from 223 patients with seropositive rheumatoid arthritis (RA) was examined morphologically. The incidence and morphological features of RN in the forefoot were evaluated. RESULTS: RN was found in 65% of cases, which had not been suspected clinically. It was correlated with pain in 69% of cases. CONCLUSIONS: Rheumatoid necrosis contributes to the development of pain and foot deformation. It can be a significant factor worsening the clinical behavior in seropo

Complications of forefoot and midfoot fractures.

Complications of the forefoot and midfoot are not as common as injuries to the hindfoot. The problem areas that the author will focus on with these injuries will be open injuries, unstable fractures, slow or poor healing fractures, and neurovascular injuries. The author also will discuss the toes, metatarsals, tarsometatarsal, cuneiforms, and talonavicular areas. Because the forefoot and midfoot must support as much as four times the body weight, alterations caused by injury to the weightbearing surface can have significant consequences. Treatment rationale to maintain a sound weightbearing pl

Forefoot pressures during walking in feet afficted with hallux valgus.

Forefoot pressures during walking in feet with hallux valgus were recorded using pressure-sensitive film, and the relationships between deformities and foot pressures were analyzed. There were 32 female subjects comprising 50 feet with hallux valgus, of which 20 feet underwent surgery. As in normal feet, the pressure patterns of feet with hallux valgus were varied: peak pressures were under the first metatarsal head, under the second and/or third metatarsal heads, and first, second, and/or third metatarsal heads. Peak pressures of hallux valgus feet were larger in each type than those of the n

Forefoot pain associated with muscle strain in the lower extremity.

A case of forefoot pain at the first metatarsophalangeal joint is discussed. Local treatment with ultrasound failed to bring significant relief. However, dramatic and lasting relief occurred when treatment was applied to the muscle belly of the extensor hallucis longus. The patient also suffered from hallux valgus with mild foot pronation. Two years after therapy, the patient stated she had not experienced any foot pain since the last treatment, and she continued to wear the prescribed flexible foot orthotics on a daily basis.

Deep Transverse Metatarsal Ligament Transection in Morton's Neuroma Excision: A Cadaveric Study Examining Effects on Metatarsal Alignment.

BACKGROUND: In the dorsal incisional approach for Morton's neuroma, it is required to transect the deep transverse metatarsal ligament (DTML) that lies in the interspace between the third and fourth metatarsal heads. The purpose of this study was to evaluate the relationship between transection of the DTML in the third intermetatarsal space and the metatarsal alignment. METHODS: Nine human cadaveric lower extremity limbs were used for this study. Each limb was mounted to the MTS 858 Mini Bionix biomechanical test system and loaded to 120% of the donor's documented body weight at a rate of 15

Comparison of shank, rearfoot and forefoot coordination and its variability between runners with different strike patterns.

This study aims to compare shank, rearfoot and forefoot coordination and its variability between runners with habitual rearfoot strike (RFS) and non-RFS (NRFS). 58 healthy males participated in this study (32 RFS, 26 NRFS). Coordination patterns and variability were assessed for the shank, rearfoot, and forefoot segments using a modified vector coding technique during running. RFS runners exhibited significantly greater proportion of anti-phase with distal dominancy (rearfoot) during early and mid-stance, as well as a lower proportion of anti-phase with proximal dominancy (shank) during early

Management of Diabetic Foot Ulcers with Two Forefoot Offloading Techniques: Case Series.

Offloading is a key principle to healing diabetic foot ulcers. Nonremovable knee-high offloading devices are considered the criterion standard for offloading plantar forefoot ulcers. However, patients exhibit a limited tolerance for these devices, which contributes to a lack of use. In this case series describing two patients, the authors share two alternative offloading modalities for the treatment of diabetic plantar forefoot ulcers. One patient was managed using a football offloading dressing, and the other was managed with a modified felted football dressing. The football and modified felt

Conservative Surgical Options for the Treatment of Forefoot Diabetic Ulcers and Osteomyelitis: An Evidence-Based Review and a Decision-Making Tool.

Diabetic foot ulcers cause higher rates of amputation and mortality; amputations yield even higher mortality rates than diabetic foot ulcers. Conservative excisional surgical procedures could be a suitable alternative to amputations, with comparable results with respect to wound-healing and infection control. Also, toe-sparing conservative surgical procedures offer good to excellent outcomes when compared with the nonoperative standard of care. Fourteen types of conservative surgical procedures were identified including bone excision with ulcerectomy, interphalangeal or metatarsophalangeal joi

The impact of the Spartathlon ultramarathon race on athletes' plantar pressure patterns.

More than 90% of injuries in runners are recorded in the lower extremity, equally affecting the regions of the knee, shank, and foot. Stress fractures are responsible for numerous running-related injuries. In the current study, the plantar pressure patterns of prerace, immediately postrace, and 24 hours after long-distance running in the Spartathlon were analyzed to compare foot loading in the respective conditions. Forty-six male participants of the Spartathlon ultramarathon were examined before, immediately after completion of the race, and 24 hours later with plantar pressure measurements d

[Scarf osteotomy for the treatment of forefoot deformity].

PURPOSE OF THE STUDY: The outcome of surgical treatment in hallux valgus is sometimes unsatisfactory for both the patient and the surgeon. The valgus position of the big toe in the metatarsophalangeal joint is associated with a deviation to varus of the first metatarsal, resulting in the space between the first and second metatarsals called the intermetatarsal (IMT) angle. In most patients the angle is between 10 and 20 degrees. These patients were indicated for scarf osteotomy as this method has been reported to achieve good outcomes. The results are compared with the relevant literature data
