The impact of a sexual assault/domestic violence program on ED care.

INTRODUCTION: Examination and management of the sexually assaulted patient comprise a complex task. On-call nurses with advanced training are used in some hospitals, but their impact on patient care and appropriate forensic examination is largely unknown. We evaluated the impact of the introduction of a sexual assault/domestic violence program (SADVP) on ED flow, comprehensive patient care, and collection of forensic evidence. METHODS: Patients presenting to the 2 emergency departments in the Kingston area (Ontario, Canada) (population, 250,000) after sexual assault were compared during 2 tim

All forensic examinations of the sexual assault survivor in the emergency department should include treatment and documentation of injuries, treatment and evaluation of sexually transmitted diseases, pregnancy risk evaluation and

When nightclub security agents assault clients.

In 2006, a medico-legal consultation service devoted to adult victims of interpersonal violence was set up at the Lausanne University Hospital Centre, Switzerland: the Violence Medical Unit. Most patients are referred to the consultation by the Emergency Department. They are received by forensic nurses for support, forensic examination (in order to establish medical report) and community orientation. Between 2007 and 2009, among community violence, aggressions by security agents of nightclubs on clients have increased from 6% to 10%. Most of the victims are young men who had drunk alcohol befo

Intrapsychic dynamics, behavioral manifestations, and related interventions with youthful fire setters.

Fire setting in youth has often been overlooked and misunderstood as a coping skill for expressing rage. The act of deliberate fire setting, if uninterrupted, may continue throughout an individual's lifetime. Forensic examiners, mental health care providers, and criminal justice professionals can help guide referral and treatment through better understanding of behaviors and intrapsychic dynamics.

Suicide by exsanguination from a puncture wound to a haemodialysis arteriovenous fistula: a forensic case report with forensic implications.

Fatal haemorrhage from a haemodialysis vascular access site is an uncommon but well-recognized cause of death in forensic practice, most frequently associated with incised or sharp-force injuries. We report a case of suicide by exsanguination resulting from deliberate needle puncture of an arteriovenous fistula in a haemodialysis patient who was a healthcare professional. A comprehensive forensic investigation, including death scene investigation, medicolegal autopsy, toxicological analyses, and evaluation of the available circumstantial evidence, was performed. [Accumulated occurrence of illnesses with Salmonella enteritidis in hospitals and nursing homes in the district Oberallgaeu, Bavaria, in July 2004].

We report on the investigation of a Salmonella enteritidis outbreak in hospitals and nursing homes in the district Oberallgaeu, Bavaria, in July 2004. Affected by this outbreak were hospital patients, inhabitants of nursing homes, kitchen coworkers and maintenance personnel. Within this outbreak six deaths were observed. A forensic medical investigation of four deaths revealed two cases of Salmonella enteritis as the primary cause of death, another cause of death was due to other causes and one cause of death remained unclear. The microbiological investigation of stool samples linked all posit

Illustrations of Unexpected Infant Sleep Deaths.

Case illustrations from central Indiana provide the narrative for infant suffocations because of unsafe sleep environments. Accidental strangulation or suffocation in bed is caused by co-bedding, blankets and pillows in cribs, or wedging and entrapment. Knowledge of the evidence-based risks associated with case data may assist further in the prevention of unexpected infant sleep deaths and may better inform best practice for death scene investigation including forensic nurses.

Vicarious traumatization: concept analysis.

There is growing knowledge of the effects of stress on professionals, including various negative symptoms that may mirror the biopsychosocial effects exhibited by the victims of trauma. Multiple concepts including burn out, compassion fatigue, post-traumatic stress disorder (PTSD), and secondary traumatic stress, are terms that have been incorrectly interchanged with the term vicarious traumatization (VT). Clarity of vicarious victimization and understanding contributing factors is imperative in order to facilitate future research and implement timely and effective interventions, as well as sc

A brief overview of managing prepubertal children who have experienced sexual abuse.

In South Africa, undergraduate medical teaching does not prepare medical and nursing practitioners adequately for the clinical forensic management of patients who have experienced child sexual abuse. Clinicians might be apprehensive when confronted with children who may have experienced sexual abuse. Their insecurities arise from a perceived lack of knowledge and the time-consuming nature of the legal procedures. The high prevalence of the sexual abuse of children demands the involvement of the medical profession. This article aims to allay uncertainty and address the unnecessary barriers to t

Characterizing Possible Acute Brain Injury in Women Experiencing Intimate Partner Violence: A Retrospective Chart Review.

Survivors of intimate partner violence (IPV) often experience violent blows to the head, face, and neck and/or strangulation that result in brain injury (BI). Researchers reviewed the de-identified forensic nursing examination records of 205 women. More than 88% of women were subjected to multiple mechanisms of injury with in excess of 60% experiencing strangulation. About 31% disclosed various symptoms consistent with BI. Women experiencing strangulation were 2.24 times more likely to report BI-related symptoms compared to those who reported no strangulation. In conclusion, women experiencing…

Chlorine gas toxicity from mixture of bleach with other cleaning products--California.

From October 1987 through November 1989, five episodes of chlorine gas exposure with toxicity to at least 14 persons occurred at two state hospitals in California. Each hospital provides inpatient treatment to approximately 1000 forensic psychiatric patients. As part of their rehabilitation programs, selected patients perform cleaning duties under the supervision of janitors or nursing staff. Each incident occurred during the performance of these duties and involved the mixture of bleach (sodium hypochlorite) and a phosphoric acid cleaner by inpatients. This mixture produced chlorine gas and o
