Pericuboid fracture-dislocation with cuboid subluxation.

The case of a 44-year-old male with a traumatic pericuboid fracture-dislocation consisting of disruption of the midtarsal, tarsometatarsal and pericuboid articulations is presented. The cuboid was partially dislocated without cuboid fracture. Treatment consisted of open reduction with stable internal fixation followed by early range of motion and protected weight-bearing for 12 weeks. Functional outcome results were excellent at final follow-up.

Trapezoid dislocation with a Galeazzi fracture.

A case of volar trapezoid dislocation and Galeazzi fracture in a 28-year-old man is presented. This case illustrates some of the problems associated with diagnosis and treatment of this rare injury.

Missed Monteggia fracture dislocation in children.

Open reduction of the radial head and reconstruction of the annular ligament has been advocated for the Monteggia fracture dislocation in children who present more than a month after injury. Three patients with an anterior Monteggia lesion were treated by open reduction of the radial head which was held in place by a Kirschner wire passed from the humerus to the radius. No attempt was made either to repair or reconstruct the annular ligament. The patients were aged between 2 and 6 years, the delay between injury and reduction was between 6 and 8 weeks, and the length of follow up was 5 years f

Case report. Pediatric carpal fracture dislocation.

Transcarpal fractures in children are rare in the orthopaedic literature. This is a case report of a 10-year-old boy who sustained fractures across the distal radius, scaphoid, lunate, and triquetrum with gross displacement. Treatment consisted of open reduction with internal fixation of the fractures and ligamentous repair through a combined dorsal and palmar approach. The injury healed with good wrist function but abnormal carpal development. This unusual pattern of injury is described so that it may be more readily appreciated in the future.

[Reposition instrument for Bennett's fracture-dislocation].

A special hook and plate can be used to adapt small central fragments in the open reduction of Bennett's fracture-dislocation. The hook is positioned behind the small central fragment and compression exerted between the hook and a plate attached to the opposite side of the base of the first metacarpal bone using an adjustable nut, thus holding the fragments together. The reduction device simplifies X-ray controls and internal fixation with Kirschner wires.

[Hip dislocation with femur head fracture].

It is talked about the classification, the therapy and the late results of the traumatic dislocation of the hip joint with a fracture of the femoral head. Twelve patients with an average age of 36 years (from 18 to 77 years) were treated between the years 1972 and 1982. The rarer anterior dislocation of the hip including a fracture of the femoral head should be specifically classified, in order not to cause any errors for the therapeutic and prognostic assessment following the Pipkin classification. Type I and II with posterior dislocation justify a closed trial of reposition. The miscarried t

Fracture-dislocation of the radiocarpal joint.

In five patients with fracture-dislocation of a radiocarpal joint, the wrist injury was severe and associated with injuries to other systems. Typically there was neurovascular impairment and closed reduction was performed as soon as possible to relieve pressure to tension on vessels and nerves. Fracture of the radial and ulnar styloid processes, fracture of the dorsal rim of the radial articular surface, and a multitude of carpal and intercarpal injuries were present in all cases. Early open reduction, internal fixation of the fractures, and repair of all torn ligaments appeared to be the trea

Fracture-dislocation of the dorsal spine.

A patient with fracture-dislocation of D9 on D8 had considerable posterolateral displacement. The neurologic injolvement of his left leg proved to be progressive. Treatment consisted of posterior decompression, exploration, open reduction and stabilization by means of Harrington rods, and fusion. All neurologic findings resolved completely. Final evaluation 18 months later showed a good and serviceable spine with no residual neurologic deficits. In my opinion, the treatment used in this case under given circumstances represented a reasonable modality of therapy.

Proximal interphalangeal joint dislocations and fracture-dislocations.

Although proximal interphalangeal joint dislocations are generally straightforward to treat, fracture-dislocations are among the most difficult hand injuries to manage. Fracture patterns range from simple to treat palmar plate avulsion fractures to complex, unstable pilon fractures of the base of the middle phalanx, where achieving adequate reduction and fixation can be extremely difficult. Moreover, these fractures may present sub-acutely or chronically, which greatly adds to the complexity of the case. It is therefore no surprise that clinical results vary and are often difficult to predict.

Perilunate dislocations and fracture-dislocations: a multicenter study.

A series of 166 perilunate dislocations and fracture-dislocations from 7 centers was retrospectively studied. The diagnosis was missed initially in 41 cases (25%). A classification system was presented. The perilunate fracture-dislocations were more frequent than the perilunate dislocations at a ratio of two to one. The displacement was dorsal in 161 cases (97%) and palmar in only 5 (3%). The dorsal transscaphoid perilunate fracture-dislocations represented 96% of the dorsal perilunate fracture-dislocations and 61% of the whole series. The clinical and radiologic outcome of 115 perilunate disl

[Radiographic diagnosis of dislocations and fracture-dislocations of the carpus].

A variety of unique and very rare carpal dislocations are reported. According to personal findings, an extension of the descriptive classification of this pathology is proposed, utilizing also the X-ray files of the Rizzoli Orthopaedic Institute. Moreover, the radiographical features are evaluated which help choosing the treatment to be followed.

Adolescent Hip Fracture-Dislocation: Transphyseal Fracture with Posterior Dislocation of the Proximal Femoral Epiphysis: A Case Report.

CASE: A 13-year-old male patient sustained a posterior transphyseal hip fracture-dislocation while playing football. A surgical hip dislocation with a trochanteric flip osteotomy, as described by Ganz and colleagues, was performed. Drilling of the dislocated epiphysis demonstrated no blood flow. Transepiphyseal and transphyseal fracture fixation with screws was performed. Two years after the injury, the patient had no evidence of osteonecrosis or functional limitations. CONCLUSION: Traumatic transepiphyseal fracture-dislocations are rare injuries that commonly result in osteonecrosis. Applica

Rare combination of ipsilateral acetabular fracture-dislocation and pertrochanteric fracture.

Acetabular fracture-dislocations are severe injuries that require urgent closed reduction of the hip and often require surgery to restore hip stability. Other authors have described acetabular fracture-dislocations associated with femoral neck fractures, but to our knowledge, this case report is the first to describe an acetabular fracture-dislocation in association with an ipsilateral pertrochanteric fracture and subtrochanteric extension. The polytraumatized patient initially was not stable enough for prolonged surgery. Through a 3-cm anterolateral hip incision, a 5-mm Schanz screw was intro

Ulnar fracture with late radial head dislocation: delayed Monteggia fracture.

Monteggia fractures are rare but commonly discussed lesions, with increasing complications due to late diagnosis. This article describes a case of a Monteggia fracture with delayed dislocation of the radial head. Previous radiographs of a 2-year 8-month-old boy show complete fracture of the distal ulna, with no radial head dislocation. The radial head remained well positioned after 4 weeks. Seven years later, he sustained another arm injury. He was diagnosed with a hematoma but was later believed to have nursemaid's elbow. He presented to our institution 5 weeks after the injury, and the radia
