Community-acquired fungemia in immunocompetent hosts remains exceptionally rare. Kodamaea ohmeri is an emerging opportunistic yeast pathogen increasingly recognised as a cause of invasive fungal infections, particularly among immunocompromised individuals and in healthcare settings. A 33-year-old previously healthy female farmer presented with severe leptospirosis with multiorgan dysfunction. Blood cultures obtained before central venous catheter insertion grew K. ohmeri, identified by the VITEK 2 YST card and matrix-assisted laser desorption ionisation-time of flight (MALDI-TOF) mass spectrom…

BACKGROUND: An intracardiac foreign body causing recurrent fungemia is a rare clinical situation. Clinicians should be that aware of rare sources of sepsis despite a thorough history and examination.
RESULTS: The authors describe a 63 year-old man, with unremarkable past medical history, who presented with a fever for 2 weeks. Blood cultures persistently grew Candida albicans and Streptococcus constellatus. Echocardiogram assessment showed a suspected vegetation over the tricuspid valve. Surgical exploration with median sternotomy and cardiopulmonary bypass revealed a tooth-pick impacted with…

In this article, we describe the first outbreak of Candida parapsilosis fungemia in our hospital. We examined a cluster of four nosocomial cases of C. parapsilosis fungemia that occurred in the neonatal intensive care unit (NICU) of the Affiliated Xingtai People's Hospital of Hebei Medical University over a two-week period. We ascertained patient parameters including clinical characteristics, blood and sputum cultures, and drug sensitivity test results. Cultures from eight blood samples obtained from the four infected preterm infants showed identical characteristics and were identified as C. p…

Pancreatic necrosis infection is the most common complication affecting mortality of severe acute pancreatitis (death rate 80%). Bacterial infections particularly with coliforms or anaerobes account for the majority of cases of infected necrosis. Fungal pancreatic infections with Candida species are rare and often nosocomial. We report herein the first case of pancreatic necrosis infection with Candida parapsilosis associated with fungemia confirmed by molecular typing.

Purpose: To identify the prevalence of fungal ophthalmic involvement and evaluate risk factors for positive screening for inpatients at an academic tertiary care hospital by assessing a priori screening criteria that may determine which patients need ophthalmic evaluation. Methods: This retrospective cohort study comprised patients with a documented positive blood culture for fungemia and an ophthalmic screening examination from January 1, 2015, to September 30, 2019. Ophthalmology notes and laboratory results taken during admission were evaluated. The primary outcomes were ocular involvement,…

Among etiologies of hyperammonemic emergencies, infection must be considered in certain clinical contexts, particularly among immunocompromised individuals. Although Cryptococcus neoformans is known to be urease-producing, to our knowledge it has not previously been described as a cause of hyperammonemia in patients. We report an immunocompromised man with acute on chronic kidney disease with hyperammonemic crisis due to Cryptococcal meningitis and fungemia. It is important to be aware of C. neoformans as a possible cause of hyperammonemia.

Microbiologic study of bacteremia and fungemia in chronic hemodialysis patients. Bloodstream infections are the second cause of death in patients in chronic hemodialysis (CHD), and the knowledge of the epidemiology is useful to establish proper empiric therapies. The aim of this study was to evaluate the frequency and distribution of microorganisms, in bacteremia and fungemia in 530 patients in CHD. Two hundred and forty eight blood culture series from 114 patients with suspected bacteremia were processed; 44% of them were positive from which 71% (n=78) were clinically significative and belong…

Sixty-two episodes of fungemia which occurred in patients with hematological disorders between 1976 and 1996 in our hospital were analyzed with respect to background and prognostic factors. Forty-four of the patients were male and 18 were female. The underlying diseases were acute leukemia in 36 cases, chronic myelogenous leukemia in 9, malignant lymphoma in 9 and others in 8 cases. Trichosporon beigelii and Candida tropicalis were the most frequently isolated fungal pathogens. The prevalence of C. crusei increased while that of C. albicans decreased after 1988. Fuungemia frequently occurred i…

Patients with acquired immune deficiency syndrome (AIDS) are known to have identifiable host defense deficiencies, especially deficiencies in cell-mediated immunity. They are at increased risk for developing infections of the bloodstream caused by Cryptococcus neoformans and Salmonella species. However, bacteremias caused by other enteric gram-negative rods and Pseudomonas aeruginosa are found less frequently in patients with AIDS than in patients without AIDS (P less than 0.001 and P less than 0.01, respectively). The findings of specific organisms in blood is consistent with the known types

Candida parapsilosis fungemia secondary to nasal colonization following application of nasal prongs for oxygen therapy developed in a 61-year-old man with known chronic lymphocytic leukemia and pulmonary infiltrates. Amphotericin B controlled the candidal infection, but the patient died of complications related to Aspergillus pneumonitis, intra-abdominal mucormycosis, and leukemia. The source of candidal infection was probably a combination of nasal ulceration resulting from oxygen administration by nasal prongs and alteration of the normal mucosal flora by multiple broad-spectrum antibiotics.…

Talaromyces funiculosus is an unusual and opportunistic pathogen. This case describes a bloodstream infection caused by the rare fungus Talaromyces funiculosus, originating from a hepatic abscess. A 53-year-old female presented acutely with high fever, chills, abdominal pain, and markedly elevated inflammatory markers. Imaging suggested suspected hepatic abscess. The patient showed poor response to antibiotics. On hospitalization day (HD) 5, blood culture yielded positive results, with microscopic examination revealing fungal hyphae. Based on these findings, caspofungin was empirically added t
