Patients with prion diseases can live for long periods of time in a state of akinetic mutism given appropriate management of their symptoms. To provide long-term gastric fistulas for collection of third-compartment gastric contents, Janeway mucosal tube gastrostomy was performed, using a gastrointestinal stapling instrument, in 6 castrated adult male llamas. Mean operative time (+/- SEM) was 65 +/- 4.16 minutes. All llamas survived the 6-week study period. At the surgical clinic of the Faculty Hospital in Ostrava-Poruba since November 1993 till June 1998 20 laparoscopic gastrostomies were performed. The main indication was to ensure enteral nutrition in patients where, due to obstruction of the oesophagus, it was impossible to implement percutaneous puncture gastrostomy. The surgical procedure lasted on average 37 minutes and was completed successfully in all patients. Three patients died within thirty days. This procedure made it possible that eighty percent of the patients could be discharged from hospital and go home.

In order to prevent the complications of long-term nasogastric suctioning and increase patient comfort, we have been using gastrostomy tubes (G-tubes) in gynecologic oncology patients who are deemed to be at risk for protracted postoperative bowel dysfunction. This study describes our patient selection criteria and evaluates the results with 35 MIC (Medical Innovation Corporation, Milpitas, CA) gastrostomy tubes inserted between September 1, 1992 and April 30, 1993. Percutaneous endoscopic gastrostomy (PEG) was performed on 28 elderly patients (mean age 82 years) who were dysphagic and intolerant of naso-gastric feeding. Twenty-six patients were recovering from a stroke; the interval between the onset of stroke and PEG averaged 63 days. The procedure was successful and well tolerated by all patients. Nineteen (68%) still had a functioning PEG a median of 14 weeks after placement. One patient whose swallowing recovered had the tube removed 6 months after its insertion.
