Partially reversible quadruple sectoranopia caused by vascular steal due to an arteriovenous malformation.

BACKGROUND: Lateral, homonymous upper and lower field blind sectors sparing a horizontal zone define quadruple sectoranopia. This rare campimetric deficit involves ischemia or infarction of parts of the lateral geniculate body supplied by the distal anterior choroidal artery. METHODS: A 41-year-old patient presented with a complaint of blurred vision. Visual acuity was 20/20 on both sides. Visual field showed a left quadruple sectoranopia. CT scan, MRI, MRA and conventional angiography showed a right cerebral arteriovenous malformation supplied partly by the right anterior choroidal artery. P

Central pontine myelinolysis with extrapontine lesions.

An autopsy case of central pontine myelinolysis (CPM) was reported. The patient, a 48-year-old man, with a long history of alcoholic intake appeared in a hospital with the diagnosis of alcoholic hepatitis and the neurologic symptoms of delirium tremens. During the hospital course he fell into "locked in" with concomitant development of marked hyponatremia (92 mEq/1). An autopsy revealed typical CPM and the extra-pontine myelinolytic lesions distributed symmetrically in the thalami, subthalamic nuclei, and lateral geniculate bodies. Spongiosis was also found in the deep layer of the cerebral co…

Diabetes mellitus and optic atrophy in two siblings: a report on a new association and a review of the literature.

Two siblings with diabetes mellitus and optic atrophy (Wolfram syndrome) are described. As often noted, they also had atonic urinary bladders. Only one of the siblings had some impairment of hearing. Other findings not previously reported that appeared in each subject were esophageal dysphagia and vertigo. An autopsy in one revealed brain stem hypoplasia and thinning and flattening of the optic nerves with atrophy of the lateral geniculate bodies.

Optic tract syndrome secondary to metastatic breast cancer.

PURPOSE: To report a patient who was initially examined with optic tract syndrome caused by a focal metastasis to the lateral geniculate body and optic tract. METHOD: Case report. RESULTS: A 44-year-old woman with metastatic breast cancer was initially examined with normal central visual acuity, afferent pupillary defect, homonymous hemianopia, and optic atrophy. CONCLUSION: This case demonstrates that an isolated optic tract syndrome may occur as a result of metastatic lesions to the lateral geniculate body and optic tract.

Right lateral geniculate nucleus infarct presenting as a left monocular temporal hemianopia.

A lesion to the right lateral geniculate nucleus should cause a complete left monocular hemianopia. We present a case of a patient with left eye monocular temporal hemianopia with two MRIs demonstrating an ischemic defect at the right lateral geniculate body as the likely culprit. Patient improved after 4 days.

Magnetic resonance findings in the pregeniculate visual pathways in Leber hereditary optic neuropathy.

Two relatives, a 61-year-old man and the 21-year-old grandson of his sister, suffered from bilateral visual loss and were diagnosed with Leber hereditary optic neuropathy. In both cases, the diagnosis was molecularly confirmed with the 11778 mitochondrial mutation. MRI showed increased T2 signal not only in the optic nerves and chiasm but also in the optic tracts, extending to the lateral geniculate bodies. To our knowledge, the latter finding has not been described previously.

Risk to the lateral geniculate artery during arthroscopic lateral meniscal suture passage.

PURPOSE: The purpose of this study was to evaluate the risk of lateral geniculate artery injury during suture passage through the lateral meniscus. METHODS: Sutures were passed via inside-out and outside-in techniques in 11 embalmed cadaveric knees. The number of punctures of the lateral geniculate artery and quantitative distances of needle penetrations were measured. RESULTS: The lateral geniculate artery was punctured 7 times during inside-out passes through the junction of the middle and anterior third of the meniscus and 8 times during passes through the body of the middle third of the

[A case of posterior choroidal artery infarction].

The posterior choroidal artery supplies the lateral geniculate body, the posterior thalamus and the caudate body. Currently, a few cases of infarction in this arterial territory have been reported. This is a case of 59-year-old male, exhibiting left homonymous hemianopsia and left hemiparesis. Clinically it was impossible to make a diagnosis of infarction in this area. We were able to determine that it was a posterolateral choroidal artery infarction rather than an anterior choroidal artery infarction using 1.5 T-magnetic resonance imaging (MRI). The MRI scan clearly disclosed the areas of inf

Using perceptual tasks to selectively measure magnocellular and parvocellular performance: Rationale and a user's guide.

The visual system uses parallel pathways to process information. However, an ongoing debate centers on the extent to which the pathways from the retina, via the Lateral Geniculate nucleus to the visual cortex, process distinct aspects of the visual scene and, if they do, can stimuli in the laboratory be used to selectively drive them. These questions are important for a number of reasons, including that some pathologies are thought to be associated with impaired functioning of one of these pathways and certain cognitive functions have been preferentially linked to specific pathways. Here we ex

The development of cell size in the dorsal lateral geniculate nucleus of monocularly paralyzed cats.

Nissl profiles of cell somata in the lateral geniculate nuclei (LGN) of adult cats which had been reared from 3 to 4 weeks of age with monocular paralysis (MP) were compared to those from normally reared adult cats. In all subjects, measurements were made in 3 different regions of the binocular segments of the A and A1 laminae and in the monocular segment in both LGNs. In the cats reared with MP, reductions in average cell size of about 20%, relative to normal, were detected throughout the binocular segments of the A and A1 laminae in both LGNs whether their innervation was provided by the par

Familial dysautonomia (Riley-Day syndrome). Temporal bone findings and otolaryngological manifestations.

Familial dysautonomia, or Riley-Day syndrome, is inherited in an autosomal recessive fashion and occurs almost exclusively in Jewish families. This disorder is characterized by a smooth tongue devoid of fungiform papillae and of taste buds, and is clinically associated with poor taste discrimination. An unsteady gait and dizziness on change in position are also common presenting symptoms. This study reports the histopathological findings of eight temporal bones from four patients with documented familial dysautonomia. For the control series, 13 normal temporal bones were also studied. Diffusion Restriction in the Optic Radiation of Term Neonates With Hypoxic-Ischemic Encephalopathy Demonstrated by Magnetic Resonance Imaging (MRI).

OBJECTIVE: There has been no previous report of diffusion restriction in the optic radiation of term neonates with hypoxic-ischemic encephalopathy. Here, using diffusion-weighted magnetic resonance imaging (MRI), we assessed diffusion restriction in the optic radiation within the first 2 weeks of life and estimated signal changes and the apparent diffusion coefficient in the optic radiation and lateral geniculate body using T1-weighted MRI. MATERIALS AND METHODS: Forty-five term neonates with hypoxic-ischemic encephalopathy underwent MRI twice during the first 2 weeks of life. Diffusion-weigh

The thalamogeniculate perforators of the posterior cerebral artery: the microsurgical anatomy.

The thalamogeniculate (TG) arteries of 30 forebrain hemispheres were examined. These vessels varied from 2 to 12 in number (mean, 5.7), and from 70 to 580 microns in caliber (mean, 345.8 microns). The average caliber of all the TG vessels per posterior cerebral artery ranged from 700 to 3400 microns (mean, 1972 microns). The TG arteries most often originated as individual vessels; however, in 26.67% of the hemispheres examined they shared a common site of origin, and 33.33% of the hemispheres they arose from common stems. The common stems ranged from 320 to 800 microns in diameter (mean, 583 m

Thalamo-olivary degeneration in a patient with laryngopharyngeal dystonia.

A 67 year old woman with a two year history of laryngopharyngeal dystonia, spasmodic dysphonia, and parkinsonism succumbed to Wernicke's encephalopathy and died six months later. Necropsy showed, besides Wernicke's encephalopathy, degenerative changes in selected thalamic nuclei (dorsomedial, pulvinar, and the medial geniculate bodies) and the inferior olives and numerous cerebellar torpedoes. The substantia nigra and basal ganglia were spared. Immunostaining for prion protein was negative. This patient indicated a new type of presentation of so-called pure thalamic degeneration, or more preci…

The diencephalon of the vervet monkey (Cercopithecus aethiops). Part I: thalamus and metathalamus.

The diencephalon of the vervet monkey (Cercopithecus aethiops) is described and compared with that of other primates, particularly the macaque monkey (Macaca mulatta). In the vervet monkey, the thalamus is divided into six nuclear groups: anterior, midline, medial, dorsolateral, ventrolateral and posterior. The anterior nuclear group shows a regressive nucleus anterodorsalis and a poorly defined demarcation between the nuclei anteromedialis and anteroventralis. There is a large interthalamic adhesion in which the ventral region containing nuclei centralis medialis, interventralis and reuniens,

[Primary optic projections in the rabbit. Study using the horseradish peroxidase anterograde labeling technic].

The primary visual pathways, in particular those to the lateral geniculate body, of 11 albino and 7 pigmented rabbits, were studied using the method of anterograde labelling with horseradish peroxidase following injection of the tracer into the vitreous body of one eye. A heavy projection to the contralateral dorsal lateral geniculate nucleus was seen in all animals. In both albino and pigmented animals a region devoid of label was present in the medial part of the alpha sector of the nucleus. This region corresponded to a compact, oval or wedge-shaped field of terminal label in the ipsilatera

Heat shock protein 72 confers protection in retinal ganglion cells and lateral geniculate nucleus neurons via blockade of the SAPK/JNK pathway in a chronic ocular-hypertensive rat model.

Optic nerve transection increased the expression of heat shock protein 72 (HSP72) in the lateral geniculate body, indicating that this protein is involved in the prevention of neuronal injury. Zinc sulfate and quercetin induced and inhibited the expression of HSP72, respectively. Intraperitoneal injections of zinc sulfate, SP600125 (c-Jun N-terminal kinase inhibitor), or quercetin were performed on retinal ganglion cells in a Wistar rat model of chronic ocular hypertension. Our results showed that compared with the control group, the expression of HSP72 in retinal ganglion cells and the latera
