Dimensions and morphological characteristics of human newborn glottis.

OBJECTIVES: The anterior glottis has the most important role in phonation. However, the posterior glottis appears to have an equally important role in respiration. Dimensions and morphological characteristics of the newborn glottis were investigated. STUDY DESIGN: Histologic analysis of the human newborn glottis. METHODS: A total of 16 larynges were used. The whole organ section technique was employed. RESULTS: There was no statistically significant difference between the genders. The average length of the glottis was 5.4 ± 0.6 mm in males and 5.7 ± 0.4 mm in females. The average length of …

Effectiveness of classical chordectomy in the treatment of cancer of the glottis.

From 1980 to 1992, 85 patients diagnosed with tumour of the glottis, localized exclusively in the vocal cord area, had undergone surgery. A 90% 5-year survival rate without recurrences had been obtained. Taking into account the cases of life-saving surgery, the percentage of the 5-year survivals amounted to 94%. Considerably better results were obtained by the excision of the entire vocal cord and not just one of its sections (1/2, 2/3 etc.). The degree of deformation of the voice following chordectomy was assessed by means of acoustic and laryngographic methods (Laryngograph Processor PCLX). …

Neurochemical markers in the nervous plexus of the canine glottis.

The structure of the nervous network and the distribution of tyrosine hydroxylase (TH)- and various neuropeptide-containing nerves were immunohistochemically studied in the glottis of the dog. The nervous network in the glottis revealed apparent regional differences in morphology. The nervous network in the cartilaginous vocal fold of the posterior glottis consisted of nerve bundles running parallel to the edge of the vocal fold. Only a small number of nerve bundles were observed in the anterior glottis, specifically in membranous vocal fold. In the subepithelial layer of the posterior glottis…

Barrier between the supraglottis and the glottis: myth or reality?

Controversial opinions on the existence of a "barrier" between the supraglottic and glottic regions of the larynx are reported. Even if the two areas have different embryological derivations, there is no anatomic evidence of a "barrier" that could prevent supraglottic cancer from extending downward to the glottis. Numerous adequate pathologic studies, including whole organ sections, confirm that for advanced cancers, anatomic compartments delimiting the spread of the neoplastic process from the supraglottis to the glottis do not exist. Therefore, supraglottic laryngectomy should be performed n…

Reconsidering a paradigm: the spread of supraglottic carcinoma to the glottis.

This study was performed to evaluate the spread of supraglottic carcinoma to the glottic level. Whole organ sections of total laryngectomy s … sis of selected supraglottic carcinomas revealed a continuum of spread from the supraglottic to the glottis.

Distribution of intraepithelial nerve fibers in the feline glottis.

It is well known that the protective laryngeal closure is elicited by mechanical or chemical stimulation of the epithelium in the glottis. In this study we used light microscopic observation and relative examination of the intraepithelial nerve fibers in the glottis to clarify the perceptive mechanism using immunohistochemical methods. In the anterior glottis, a moderate number of protein gene product 9.5-immunoreactive intraepithelial nerve fibers were observed, most of which were found to be located just anterior to the vocal process. The number of fibers in the upper surface of the vocal fo…

Carcinoma in situ of the glottis: radiotherapy or excision biopsy?

Excision biopsy, radiotherapy, and laryngofissure cordectomy have all been used, but the best treatment regimen for glottic carcinoma in situ remains controversial. A 20-year experience of 37 cases is reported. For the first 10 years biopsy with radiotherapy was the first-line treatment (n = 20), but for the last 10 years surgical or laser excision biopsy alone has been performed (n = 17). The T stage of the carcinoma in situ does not determine its premalignant potential. Malignancy appears to be a recurrence, rather than a new expression of carcinogenesis. Excision biopsy alone provides local

T1-T2 carcinoma of the glottis: relative hypofractionation.

Radiation therapy, the preferred primary treatment for early squamous cell carcinoma of the glottis, offers high local control rates with voice preservation; however, the optimal treatment schedule is subject to debate. Local control, with and without surgical salvage, and associated long-term effects and complications were retrospectively analyzed in 90 patients treated with definitive radiation therapy for T1-T2 squamous cell carcinoma of the glottis. Patients received three weekly fractions of 333 cGy to a total dose of 60 Gy in 6 weeks. Median follow-up was 51.5 months. With radiation ther…

Cancer of the glottis: prognostic factors in radiation therapy.

The authors conducted a multivariate analysis of the prognostic factors in 96 patients with early glottic cancer treated by radiation therapy. Of these, 73 had T1 and 23 had T2 tumor. The primary tumor was controlled in 82% of T1 and 74% of T2 lesions. Actuarial five-year survival rates were 87% for T1 and 74% for T2. Carcinoma of the anterior commissure associated with bilateral vocal cord involvement, subglottic tumor extension, persistent or recurrent laryngeal edema, and impaired cord mobility was found to adversely influence the prognosis. The data suggest that irradiation is the treatmen

The soft tissue/cartilage relationship in the laryngeal glottis.

A study of 93 larynges obtained at autopsy was made with regard to the proportion of cartilage and soft tissue in the glottis. The relevant measurements showed that the cartilage formed 48 per cent of the glottic opening and soft tissue (vibrating portion of the vocal cord) formed 52 per cent. The importance of this relationship to the classification and origin of laryngeal cancer is discussed, as is its relation to vocal cord nodules.

Mechanism of initiation of oscillatory motion in human glottis.

Phonatory onsets of 17 normal subjects under usual speech conditions were investigated by measuring the cross-sectional area of the glottic aperture, using a photoelectric device. During a normal soft or breathy onset of phonation, the vocal fold vibration begins with an open glottis; the pattern of the optical signal is a sine wave of increasing amplitude with one to eight cycles before steady state oscillation is achieved. The first deflection of the base-line is either towards adduction or towards abduction. The classically assumed--since van den Berg et al. (1957)--retro-aspiration phenome…

Carcinoma of the laryngeal glottis therapy and end results.

The end results of 264 patients with vocal cord carcinoma treated by either surgery or radiation therapy at the University of Illinois from 1955 to 1972 are analyzed. Stage I glottic carcinoma can be cured by either surgery or radiation alone (86 percent and 80 percent). Stage II glottic cancer treated by surgery has achieved survival rates of 70 percent. The five-year end results of Stage III glottic cancer are 49 percent for surgery and 30 percent for radiation. The management of glottic carcinoma, according to anatomic site and indications for various modalities are discussed.
