Mansonelliasis, a neglected parasitic disease in Haiti.

Reported in Haiti as early as 1923, Mansonella ozzardi is still a neglected disease ignored by the health authorities of the country. This review is an update on the geographic distribution of the coastal foci of mansonelliasis in Haiti, the epidemiological profile and prevalence rates of microfilariae in people living in endemic areas, the clinical impact of the parasite on health and the efficiency of the transmission of the parasite among three Culicoides biting-midge species identified as vectors in Haiti. Additionally, interest in establishing a treatment programme to combat this parasite…

CE: Responding to the cholera epidemic in Haiti.

While Haiti was still recovering from the January 12, 2010, magnitude-7 earthquake, an outbreak of cholera spread throughout the nation, soon reaching epidemic proportions. Working through the faith-based nongovernmental organization Samaritan's Purse, an NP, an epidemiologist, and a physician joined the effort to prevent the spread of disease and treat those affected. Here they describe the prevention and intervention campaigns their organization initiated, how they prepared for each, and the essential elements of their operations.

Nepalese origin of cholera epidemic in Haiti.

Cholera appeared in Haiti in October 2010 for the first time in recorded history. The causative agent was quickly identified by the Haitian National Public Health Laboratory and the United States Centers for Disease Control and Prevention as Vibrio cholerae serogroup O1, serotype Ogawa, biotype El Tor. Since then, >500 000 government-acknowledged cholera cases and >7000 deaths have occurred, the largest cholera epidemic in the world, with the real death toll probably much higher. Questions of origin have been widely debated with some attributing the onset of the epidemic to climatic factors an…

Cutaneous manifestations of anthrax in rural Haiti.

In industrialized countries, the zoonotic disease anthrax has been virtually eradicated because of effective public health measures including animal vaccination and quality control of animal products. In developing parts of the world, however, anthrax remains an occupational hazard of herdsmen and workers who have direct contact with infected animals or who process animal hides, hair, bone and bone products, and wool. For clinicians unfamiliar with this interesting infectious disease, the major dermatologic characteristics and clinical evolution of five cases of cutaneous anthrax are reviewed

Clinical evaluation of Mansonella ozzardi in Haiti.

The clinical evaluation of 150 individuals in Bayeux, Haiti, revealed no significant morbidity that could be attributed to infection with Mansonella ozzardi. However, the distribution of patent infections and their microfilaria density were significantly affected by the age and sex of the individuals. Also, the eosinophilic response was associated with the intensity of the microfilaremia when age was considered as a co-variable. The microfilaria rate was significantly higher in certain occupations, but this was discounted as a risk factor for reasons that are explained.

Epidemiology of Wuchereria bancrofti in Leogane, Haiti.

A survey for Wuchereria bancrofti in Leogane, Haiti, revealed that 140 of 421 individuals (33%) had a patent infection, of which 40% lived in the suburban outskirts of the city. The median microfilaria density was 19.1 per 20 mm3 of blood for suburban dwellers compared with only 8.8 for those living in the city. The vector, Culex quinquefasciatus (Say), breeds mostly in and around numerous rum distilleries, located exclusively around the periphery of the city, and this undoubtedly accounts for the higher prevalence and intensity of infection among suburban dwellers.

Enhanced Risk for Epidemic Cholera Transmission, Haiti.

Sporadic cholera outbreaks continue to occur in Haiti. We used a novel space-time analysis to gain insight from limited government surveillance data. We identified concerning patterns of disease spread in areas known to be at high risk for epidemic cholera in and around the capital city of Port-au-Prince.

Mansonella ozzardi in Haiti. I. Epidemiological survey.

A survey of 1,165 inhabitants of Bayeux, Haiti revealed that 16% were infected with Mansonella ozzardi. This was determined from a single 20 mm3 sample of finger prick blood from each individual. Amont children and young adults (< 20 years of age), fewer than 2% had detectable microfilaremias. Beyond this age the prevalence of infection for males and females was 49% and 24%, respectively. The microfilariae circulate in the peripheral blood with no significant periodicity. In general, the microfilaremias were low, with 71% of the positive cases having less than 10 microfilariae in 20 mm3 of blo…

Child malnutrition in Haiti: progress despite disasters.

Undernutrition, a chief child killer in developing countries, has been a major public health problem in Haiti. Following the 2010 disasters (earthquake and cholera) and the intensive relief efforts to address them, we sought to determine the trends of child undernutrition in Haiti using data from the 2005-06 Haiti Demographic and Health Survey (HDHS) and from a Standardized Monitoring and Assessment of Relief and Transitions (SMART) survey in 2012. Growth data analyses included 2,463 (HDHS) and 4,727 (SMART) children ages 0-59 months. We calculated the prevalence of stunting, wasting, and unde…

Histopathologic improvement with lymphedema management, Léogâne, Haiti.

In countries where bancroftian filariasis is endemic, lymphedema of the leg is a public health problem, particularly for women, who are disproportionately affected. We investigated the effect of basic lymphedema management (hygiene, skin care, and lower limb movement and elevation) on the histologic features of lymphedema. A total of 118 skin-punch biopsy specimens were collected from the legs of 91 patients enrolled in a lymphedema treatment clinic in Léogâne, Haiti. Follow-up biopsy specimens were collected from 27 patients succeeds, equals 12 months later. Keratinocyte hyperproliferation, c…

Congenital rubella syndrome in Haiti (Short communication).

OBJECTIVE: To determine if there is an unrecognized problem of congenital rubella syndrome (CRS) in Haiti, a country without a national rubella immunization program. METHODS: During March 2001 and June 2001, screening physicals were conducted on approximately 80 orphans at three orphanages in Haiti that accept disabled children. Children were classified as probable CRS cases based on established clinical criteria. Photo documentation of findings was obtained whenever possible. RESULTS: Six children met the criteria for probable CRS. Using data from surrounding Caribbean countries and from th…

The experience of violence against children in domestic servitude in Haiti: Results from the Violence Against Children Survey, Haiti 2012.

BACKGROUND: There have been estimates that over 150,000 Haitian children are living in servitude. Child domestic servants who perform unpaid labor are referred to as "restavèks." Restavèks are often stigmatized, prohibited from attending school, and isolated from family placing them at higher risk for experiencing violence. In the absence of national data on the experiences of restavèks in Haiti, the study objective was to describe the sociodemographic characteristics of restavèks in Haiti and to assess their experiences of violence in childhood. METHODS: The Violence Against Children Survey …
