BACKGROUND: Hallux valgus (HV) is a three-dimensional deformity commonly assessed using the intermetatarsal angle (IMA), hallux valgus angle (HVA), and sesamoid rotation angle (SRA). We aimed to evaluate a multidimensional metric (MDHV) combining these parameters.
METHODS: This retrospective case-control study included 61 symptomatic HV and 60 control feet assessed by weight-bearing CT. IMA, HVA, and SRA were measured, and MDHV was calculated using principal component analysis.
RESULTS: IMA, HVA, and SRA were significantly higher in HV feet (p < 0.001). MDHV achieved the highest diagnostic a…

One of the most common procedures performed in the foot and ankle is correction of hallux abducto valgus deformity or "bunion surgery." Most foot and ankle surgeons recognize the challenges associated with defining each patient's individual deformity and selecting the optimal procedure for the best long-term results. Using current 2-dimensional algorithms that focus on the severity of the transverse plane deformity, surgical outcomes have varied. In the past 10 years, high recurrence and complication rates for popular procedures have been reported. In the same period, the reported data have el

More than 150 corrective procedures for hallux valgus exist and an incorrect choice of procedure leads to insufficient correction. Distal first metatarsal osteotomy cannot correct large deformities and degenerative changes at the metatarsophalangeal joint impede functional recovery. Incongruence of the joint must be corrected during surgery. Recurrence is most often caused by insufficient correction, especially of the first metatarsal bone. Overcorrection is often due to technical problems with the initial metatarsal osteotomy. This also applies to insufficiency of the first ray due to shorten…

Various causes of hallux valgus exist. However, mass-induced hallux valgus is a rare phenomenon. To the best of our knowledge, there has been 1 previous report on mass-induced hallux valgus in the English literature; however, that report described giant cell tumor of the extensor digitorum longus tendon sheath. We believe ours to be the first published case of osteochondroma of the second metatarsus-induced hallux valgus. A 19-year-old woman presented with pain and tenderness in the left hallux of 3 months' duration. After a diagnosis of bony mass with hallux valgus, an operative excision was …

Selecting the appropriate surgical procedure for the treatment of hallux valgus not always is clear. Every procedure has its merits depending on the individual and circumstances. Correcting pain and deformity, avoiding recurrence, and preserving or re-establishing normal foot function should be the goals of bunion surgery. Although radiographic measurements can be helpful, their weight is not as important as understanding the function of the first ray in each patient. This article attempts to guide procedure selection based on re-establishing normal foot function as much as possible while meet…

The most common complication following minimally invasive surgery (MIS) foot procedures is recurrence of the deformity, likely a direct consequence of incorrect selection of the procedure, incorrect surgical technique, or underestimated healing time of the osteotomy. This article describes our surgical MIS for hallux valgus correction, including the postoperative management, and reviews the literature on MIS for hallux valgus correction. The reports on such techniques are few, and mainly case series are reported. More adequately planned and executed randomized prospective clinical trials are n…

In the surgical treatment of hallux valgus, proximal, diaphyseal, and distal osteotomies of the first metatarsal bone are commonly used. In an overview article different procedures are described as well as our own stage-adapted concept. Deformities with congruent articulation of the metatarsophalangeal joint are successfully treated with a distal chevron osteotomy. In an incongruent joint a distal soft tissue procedure is required for reduction of the joint. The metatarsus varus displacement is corrected with a proximal osteotomy of the first metatarsal bone. The indications and details of the…

Hallux valgus is the abnormal lateral deviation of the great toe. The principal cause is biomechanical, specifically the habitual use of footwear which constricts the toes. In this study, descriptions of the anatomical changes of hallux valgus from published cadaveric and clinical studies were used to generate criteria for identifying the condition in ancient skeletal remains. The value of systematic scoring of hallux valgus in paleopathology is illustrated using two British skeletal series, one dating from the earlier and one from the later Medieval period. It was found that hallux valgus was…

A retrospective study of the Chevron osteotomy procedure for hallux valgus was carried out on 112 patients (168 feet) with an average age of 39.7 years (range 19 to 61 years). The average follow-up was 4.4 years (range 13 months to 7.5 years). The preoperative diagnosis was symptomatic hallux valgus. The average preoperative intermetatarsal 1-2 angle was 15.5 degrees (range 11 degrees to 18 degrees) and the hallux valgus angle was 28.1 degrees (range 21 degrees to 42 degrees). The operation was effective in improving symptoms, cosmesis, and function and in decreasing the deformity.

The solar pressure zones were analyzed in the feet of 66 patients suffering from hallux valgus, together with 60 normal subjects. The EMED Gait Analysis System was used. In the hallux valgus group, the maximum pressure was found to be increased significantly in the small toe region and more proximally situated, close to the metatarsophalangeal joint. In the normal subjects, the maximum pressure was increased significantly in the first, second, third, and fourth metatarsal and heel regions. In general, the hallux valgus group had smaller contact areas compared to the control group. The increase…
