Osteoblastoma of the Hamate: A Case Report.

INTRODUCTION: Osteoblastomas are rare, benign, osteoid-producing primary bone tumors that typically occur in adolescents and young adults. They constitute only 1% of all bone tumors and are exceptionally rare in the hand and wrist. Carpal bone involvement is exceedingly uncommon, with only 9 cases reported in the literature involving the hamate. This case is important to report because the occurrence of this neoplasm in the fifth decade of life is extremely rare, making it one of the very few reports of hamate osteoblastoma in late adulthood. CASE REPORT: A 51-year-old male presented with a 2

Characteristic radiographic features of the central ray in Apert syndrome.

We reviewed retrospectively seven patients with Apert acrosyndactyly and measured the size of the capitate ossification centre relative to that of the hamate and determined the relative position of the middle finger metacarpal relative to the ring finger metacarpal. We then compared those parameters in 197 normal children. In all patients, the middle finger metacarpal bone had migrated proximally relative to the ring finger metacarpal and the size of the capitate ossification centre was smaller than that of the hamate. After surgical release of the middle finger, relative proximal migration of

Symptomatic congenital pisiform hamate coalition.

Congenital fusions of the carpal bones are rare anomalies, which occur in less than 1% of the population. This article describes a congenital pisiform hamate coalition which became symptomatic when caused by a trauma. The condition was successfully treated with the resection of the synchondrosis, autogenous bone graft, and screw stabilization.

Intracarpal shortening osteotomy for Kienböck's disease: A retrospective study of 28 cases.

INTRODUCTION: Kienböck's disease is rare in patients with a neutral or positive ulnar variance. In these situations, treatment is challenging and controversial. Various intracarpal shortening osteotomy (ICSO) procedures have been proposed. OBJECTIVE: Study the effect of the type of ICSO (isolated capitate osteotomy or combined with hamate osteotomy) on the clinical and radiological outcomes in a retrospective series. METHODS: Patients with Kienböck's disease were treated with ICSO. A dorsal approach centered over the capitate was used. The transverse osteotomy was located 5mm below the capit

MR imaging of the carpal tunnel.

OBJECTIVE: Investigations were conducted regarding changes of carpal tunnel shape during wrist motion and the variations of space for the median nerve as well as the preoperative signs of carpal tunnel syndrome (CTS) and the postoperative restitution. METHODS: Axial MR images (1.0 T) were performed at the level of the distal radioulnar joint, pisiforme bone and hook of hamate level of 20 wrists of patients with clinical symptoms of CTS and further 20 wrists of volunteers. This was conducted with the wrist in neutral position, 45 degrees extension and 45 degrees flexion. T2-weighted signal int

Test of Sex Estimation Equations on Carpal Bones in a Northeastern Thai Population.

BACKGROUND: Sex assessment is an essential step in person identification, both in forensic and anthropological contexts. Many parts of skeletal remains such as skull, pelvis and long bones have been proven to be useful in determining sex. However, literature has shown that short bones such as carpal bones are also sexually dimorphic. In the last few years, there was an unpublished study using lunate, scaphoid and hamate from bone collection in Northern Thailand to create 6 discriminant equations to assess sex. The objective of this study was to investigate the application of those equations in

Destructive Giant Cell Tumor of the Capitate: A Unique Case of Carpal Reconstruction.

We report a unique case of carpal reconstruction for capitate deficiency in a child. The patient is a 15-year-old girl who had a destructive giant cell tumor involving the capitate and hamate. To the author's knowledge, this is the first case in which Achilles tendon interposition allograft was utilized to fill the bony void left behind by capitate and partial hamate excision. Arthrodesis was used as an adjunct for stabilizing the second and third proximal metacarpals. This resulted in a great functional and radiographic outcome with 7-year follow-up. In conclusion, when treating a carpal defe

Trans-scaphoid, Transcapitate, Transhamate Injury: A Case Report.

Traumatic axial dislocation of the carpus in a 20-year-old man is described. This injury was accompanied by a crushing injury to the hand. The disruption pattern was different from those of previously reported cases. Despite the restoration of painless wrist motion postoperatively, grip strength remained below normal. Early accurate reduction, fixation, and range of motion exercise are the treatments of choice in such complex injuries.

Fractures of carpal bones other than scaphoid: epidemiology, methods and results of treatment.

UNLABELLED: Fractures of the carpal bones are rare compared to fractures of the metacarpals and phalanges. The triquetrum and scaphoid are t … stitution. Fractures of the triquetrum was the most commonly seen - 25 cases (71%), followed by the hamate bone - 6 cases (17%). Most fractures were treated conservatively with wrist immobilization for 5 weeks. Six patients (17%) required surgical treatment, most of them with the hamate fractures. RESULTS: In a telephone interview at a mean of 16 months post …

Hamate autograft for proximal pole scaphoid fracture: A systematic review.

BACKGROUND: Proximal pole scaphoid fracture is not uncommon and present therapeutic challenges due to impaired perfusion, with risk of secondary non-union, avascular necrosis and wrist degeneration. There is no consensus on surgical techniques for a non-salvageable proximal pole, but proximal hamate autograft to resurface the scaphoid is gaining interest. This systematic review assesses the evidence for proximal hamate autograft in resurfacing non-salvageable scaphoid proximal poles. METHODS: A systematic review searched the Medline, Embase, PubMed and Scopus databases for the period 2000-202

Hook of Hamate Fractures.

Hook of hamate fractures are uncommon injuries with unclear prevalence. Classic presenting symptoms include volar-ulnar hand/wrist pain and weakness in grip, with possible ulnar nerve paresthesias. Patient factors, such as activity level and desired return to work/sport, shape the most appropriate treatment regimen although excision of the hook has been adopted by most surgeons. Overall, once patients receive the correct diagnosis and are treated, good outcomes with high satisfaction rates are expected.

Treatment of hook of the hamate fractures in adults using low-intensity pulsed ultrasound.

Two delayed unions and one nonunion of hook of the hamate fractures in adults aged 31-, 40-, and 41-years-old were treated with low-intensity pulsed ultrasound (LIPUS). Ultrasound treatment was started at three, four, and six months after injury and ultrasound exposure at the hook of the hamate in the hypothenar eminence was carried out for four to five months. During the management period, there was no immobilisation with a cast or brace and limited strain with only routine daily activities allowed. In all cases, bony union was confirmed on carpal tunnel radiographs or computed tomography at

Carpal tunnel: MR imaging. Part II. Carpal tunnel syndrome.

The magnetic resonance (MR) images of 14 wrists of patients with carpal tunnel syndrome (CTS) were studied. Four general findings visible regardless of the cause of CTS included swelling of the median nerve, best evaluated at the level of the pisiform bone; flattening of the median nerve, most reliably judged at the hamate level; palmar bowing of the flexor retinaculum, best visualized at the level of the hamate bone; and increased signal intensity of the median nerve on T2-weighted images. Findings related to cause were tendon sheath edema in traumatic tenosynovitis, synovial hypertrophy in r…
