The normal heart sounds, murmurs, opening and closing sounds of aortic and mitral valve prostheses were recorded on a tape and analyzed in terms of contour sonagrams in order to obtain the highest frequencies which were recordable on the chest wall. The peak frequencies showed a broad range, the maximum beeing reached at about 8000 c.p.s. in the case of the sounds of prosthetic valves. Furthermore, sound level examinations of the normal first and second heart sound, as well as of the opening and closing sounds of the aortic valve prostheses were performed. The highest sound level of all of the…

Early detection of coronary artery disease (CAD) using the acoustic approach, a noninvasive and cost-effective method, would greatly improve the outcome of CAD patients. To detect CAD, we analyze diastolic sounds for possible CAD murmurs. We observed diastolic sounds to exhibit 1/f structure and developed a new method, path length entropy (PLE) and a scaled version (SPLE), to characterize this structure to improve CAD detection. We compare SPLE results to Hurst exponent, Sample entropy and Multiscale entropy for distinguishing between normal and CAD patients. SPLE achieved a sensitivity-specif

In a random sample of patients examined in the outpatient department of pediatric cardiology in Giessen, an innocent heart murmur was found in 1009 children. Results and methods of cardiological examination relevant to the practitioner were established by evaluating the individual diagnostic criteria. An anamnesis without any reference to heart disease is of special significance for the diagnosis of an innocent heart murmur. Another important factor is the experience of the examining doctor. Innocent murmurs are systolic and frequently of "musical quality", their intensity ranges from grade 1/

Auscultation is the core of pediatric cardiovascular evaluation and should be performed according to a routine. The four heart sounds should be sought in order at their usual locations, with particular attention to the second sound because of its important diagnostic role in congenital cyanotic heart disease. The next sounds to listen for are clicks, which can be systolic, pulmonary, aortic, or midsystolic. Finally, murmurs should be sought in systole and then diastole. Besides being identified as systolic, diastolic, or continuous, murmurs should be characterized according to their relationsh…

A 79-year-old man with dilated cardiomyopathy and severe functional mitral regurgitation presented with general fatigue and dyspnea. Auscultation revealed a systolic regurgitant murmur with a minimized second heart sound due to a low output. On the other hand, the third heart sound was ultimately enhanced, being visible and palpable as a pulsatile knock of the precordium. Phonocardiography and echocardiography successfully confirmed early-diastolic rapid distension of the left ventricle along with rapid ventricular filling and abrupt deceleration of the atrioventricular blood flow to be the pr

A simple bedside technique of heart sound simulation using hand and stethoscope is described. The technique utilizes tissue vibration and sound conduction with the usual air column of a stethoscope to reproduce auscultatory events accurately without the distortion that often accompanies electrical production of heart sounds.
