Anticoagulant Therapy for Mechanical Heart Valves - Unmet Need Persists.

Direct oral anticoagulants such as apixaban or dabigatran have revolutionized anticoagulant treatment. These drugs, which specifically inhibit either factor Xa or thrombin, respectively, are at least as effective as vitamin K antagonists (e.g., warfarin) in patients with atrial fibrillation or venous thromboembolism and have important safety advantages that include reduced risks of intracranial hemorrhage, fatal bleeding, and all-cause mortality.1-3 They are also much easier to use because they are given in a fixed dose without routine laboratory monitoring of the anticoagulant effect and dose

Sucrose Diffusion in Decellularized Heart Valves for Freeze-Drying.

Decellularized heart valves can be used as starter matrix implants for heart valve replacement therapies in terms of guided tissue regeneration. Decellularized matrices ideally need to be long-term storable to assure off-the-shelf availability. Freeze-drying is an attractive preservation method, allowing storage at room temperature in a dried state. However, the two inherent processing steps, freezing and drying, can cause severe damage to extracellular matrix (ECM) proteins and the overall tissue histoarchitecture and thus impair biomechanical characteristics of resulting matrices. Freeze-dry…

Dabigatran versus warfarin in patients with mechanical heart valves.

BACKGROUND: Dabigatran is an oral direct thrombin inhibitor that has been shown to be an effective alternative to warfarin in patients with atrial fibrillation. We evaluated the use of dabigatran in patients with mechanical heart valves. METHODS: In this phase 2 dose-validation study, we studied two populations of patients: those who had undergone aortic- or mitral-valve replacement within the past 7 days and those who had undergone such replacement at least 3 months earlier. Patients were randomly assigned in a 2:1 ratio to receive either dabigatran or warfarin. The selection of the initial …

Extracellular matrix in deoxycholic acid decellularized aortic heart valves.

BACKGROUND: Only limited information is available regarding the influence of decellularization on the extracellular matrix in heart valves. Within the extracellular matrix proteoglycans (PG) play a central role in the structural organization and physical functioning of valves and in their capability of settling with endothelial and interstitial cells partially myofibroblasts. We have therefore estimated the effects of decellularization using deoxycholic acid on the structure of the extracellular matrix and PG´s in porcine aortic valves. MATERIAL/METHODS: Cupromeronic blue was used, alone or i…

Recellularization of decellularized mitral heart valves in juvenile pigs.

BACKGROUND AND AIM OF THE STUDY: Glutaraldehyde-preserved bioprosthetic heart valves are non-viable and have a limited durability because of calcification, tissue wear, and inflammation, especially in children. Decellularized porcine heart valves, when treated with deoxycholic acid (DOA), have exhibited complete recellularization and an absence of calcification when implanted into the pulmonary position in juvenile sheep. The study aim was to determine the degree of recellularization and calcification in DOA-treated heart valve prostheses in the mitral position in juvenile pigs. METHODS: A mi…

Lipid-mediated inflammation and degeneration of bioprosthetic heart valves.

BACKGROUND: The durability of bioprosthetic valves is limited by structural valve degeneration (SVD) leading to bioprostheses (BPs) stenosis or regurgitation. We hypothesized that a lipid-mediated inflammatory mechanism is involved in the SVD of BPs. MATERIAL AND METHODS: Eighteen Freestyle stentless BP valves were explanted for SVD at a mean time of 5.9 +/- 3 years after implantation and were analysed by immunohistochemistry and transmission electron microscopy (TEM). RESULTS: The mean age of the patients was 65 +/- 8 years and there were 11 male and seven female patients. Two of the 18 BPs

[Cerebral embolia in patients with prosthetic mechanic heart valves].

The review focuses on embolia of cerebral vessels in patients with prosthetic mechanic heart valves. The highest risk for embolia is observed in the first 3 months after the surgery, a frequency of embolia being dependent on the type and position of the artificial valve. Methods for prevention of post-surgery thromboembolic complications are considered.

Pregnancy and mechanical heart valves replacement; dilemma of anticoagulation.

OBJECTIVES: To establish a uniform anticoagulation regimen for pregnant patients with mechanical heart valves taking into account the socio-economic background and to evaluate the incidence of anticoagulant related complications. METHODS: A retrospective study on 63 pregnancies in 21 women with mechanical heart valves was evaluated. These pregnancies were divided into two groups: Group I (n=42) received oral anticoagulants throughout pregnancy and Group II (n=21) received subcutaneous heparin in the 1st trimester and oral anticoagulants for the rest of pregnancy period. Both groups received h…

[Morphologic changes in the valves in acquired heart defects].

The histologic, histochemical and ultrastructural changes of the leaflet tissue of all four cardiac valves obtained by biopsy and autopsy have been studied in 50 patients with acquired valvular defects. The anamnesis confirmed the presence of rheumatic fever in 16 patients and negated it in 34 patients. In all analysed leaflets of all four cardiac valves organic changes were found which were the cause of their dysfunction. The analysis has shown that rheumatic fever does not play a dominant role in development of acquired valvular disease as it has been thought previously. However, degenerativ

Effects of triflusal in patients with prosthetic heart valves.

The biological effects of a new antiplatelet agent, triflusal, were evaluated in patients with cardiac valvular prostheses. Each patient received 900 mg/day of oral triflusal for 30 days. Triflusal significantly inhibited platelet aggregation induced by adenosine diphosphate (1 to 5 mumol) or epinephrine (12.5 mumol), showing a slight effect on bleeding time (basal: 6 +/- 1.7 min; 30 days of treatment: 8.0 +/- 2.7 min). Serum levels of thromboxane B2 were significantly reduced during treatment, but changes in serum levels of 6-keto-prostaglandin F1 alpha were not observed, suggesting that trif

Replacement of heart valves with frame-mounted tissue grafts.

Ionescu, M. I., Pakrashi, B. C., Mary, D. A. S., Bartek, I. T., and Wooler, G. H. (1974). Thorax, 29, 56-67. Replacement of heart valves with frame-mounted tissue grafts. Between April 1969 and March 1973, 213 patients had heart valve replacement with frame-mounted autologous or homologous fascia lata or with heterologous pericardial grafts. There were 111 single aortic, 95 single mitral, and seven tricuspid valve replacements. The incidence of hospital and late mortality was each 10% for the entire series and the main causes were myocardial failure and infective endocarditis. The majority of …

Storage, preservation, and rehabilitation of living heart valves to treat congenital heart disease.

Heart valve disease patients undergo multiple surgeries to replace structurally degraded valve prostheses, highlighting the need for valve replacements with growth and self-repair capacity. Given allogeneic valve transplantation's promise in meeting these goals by delivering a living valve replacement, we propose a framework for preserving and rehabilitating living valves ex vivo.

Polymeric heart valves for surgical implantation, catheter-based technologies and heart assist devices.

Efficient function and long-term durability without the need for anticoagulation, coupled with the ability to be accommodated in many different types of patient, are the principal requirements of replacement heart valves. Although the clinical use of valves appeared to have remained steady for several decades, the evolving demands for the elderly and frail patients typically encountered in the developed world, and the needs of much younger and poorer rheumatic heart disease patients in the developing world have now necessitated new paradigms for heart valve technologies and associated material…

[Heart valves in rheumatic heart disease in the light of scanning electron microscopy].

In case of rheumatic sclerosis the micro-relief of the surface of the mitral and aortic valves changes considerably: the first order plicas disappear, a new chaotic micro-relief is formed that depends on the deep specific changes in the collagen-elastic structures of the valves. In rheumatic heart diseases the valves are characterized by focal disendothelization of the surface, which permits to examine the valve in layers, and to study not only its surface, but also its internal subendothelial structures. Scanning electron microscopy permitted to visualize for the first time in a full three-di

Scientific Evolution of Artificial Heart Valves: A Narrative Review.

Cardiovascular disorders have always been the top contributors to the number of mortality occurring worldwide. But the last few decades have … s a fascinating subject, as it continues to progress. Valve replacements comprise either mechanical heart valves or bioprosthetic heart valves. Both types of valves have their merits and demerits; their usage depends mostly on individual patient requirements. This article aims to review the evolution of the implantation of heart valves, and it is the objective …

Magnetic retrieval of prosthetic heart valves for redo-TAVI.

Bioprosthetic aortic heart valves are known to degenerate within 7-15 years of implantation. Currently, the options for treating a failing valve are (a) redo surgical aortic valve replacement or, increasingly, (b) valve-in-valve transcatheter aortic valve implantation (ViV-TAVI). The ViV-TAVI procedure is referred to as redo-TAVI when the failing valve is a TAVI device. Repeated procedures, such as two or three valve-in-valves, significantly reduce the effective valve flow area, putting a limit on recurrent treatments. With increasing life expectancy and the use of TAVI in younger, lower-risk …
