High-frequency jet ventilation using a helium-oxygen mixture.

The physiological basis for the use of helium relates to the relationship described by Poiseuille. During turbulent gas flow, the factors determining the resistance to flow include the density of gas as well as the length and the radius of a tube. While it may not be possible to readily change the latter two, altering the density of the gas is possible by using helium instead of nitrogen. A helium-oxygen combination has been used most commonly to improve air exchange in patients with upper airway obstruction. Anecdotal reports also suggest the beneficial effects of helium during mechanical ven

Hemodynamic effects of high-frequency oscillatory ventilation in children.

The purpose of this study was to evaluate the acute hemodynamic effects of transitioning a patient from conventional mechanical ventilation (CMV) to high-frequency oscillatory ventilation (HFOV). Our hypothesis was that hemodynamic status would not be adversely affected by such a change. Ten pediatric patients with acute hypoxemic respiratory failure and a thermodilution pulmonary arterial catheter in place were prospectively evaluated on the transition from CMV to HFOV. Hemodynamic and respiratory data were obtained before and within 1 hour of transition to HFOV with a "high-volume" ventilati

High frequency oscillatory ventilation in neonates with respiratory distress.

High frequency oscillatory ventilation (HFOV) was attempted in ten infants with severe respiratory failure not responding to conventional ventilation (CV); it was, therefore, used as a rescue measure. HFOV was successful in improving the respiratory status of seven infants, all with hyaline membrane disease (HMD). Five of these infants survived, of the remaining two, one died of massive peri/intra-ventricular haemorrhage and the other of cholestasis associated with total parenteral nutrition. It was unsuccessful in three infants, one with meconium aspiration, the second died within two hours c

Necrotizing tracheobronchitis following high frequency ventilation: effect of hydrocortisone.

A piglet model of acute respiratory failure was used to determine whether necrotizing tracheobronchitis (NTB) produced during high-frequency pneumatic flow interrupter (HFFI) ventilation could be attenuated by prior administration of 2 mg/kg hydrocortisone IV. Fourteen piglets (means age 3.6 days, means wt 1.4 kg) were anesthetized and paralyzed before saline lung lavage. The animals were randomly assigned to either placebo (P) or hydrocortisone (H) group. Continuous HFFI (10 Hz) was interrupted five times per minute by a 1-second deflationary pause. All animals were kept on 1.0 FI02 with vent

Convective mixing mechanisms in high frequency intermittent jet ventilation.

A liquid flow visualization technique was used to identify the location of neutrally buoyant bead clouds injected into airway models during flows simulating high frequency intermittent jet ventilation (HFIJV) in neonatal lungs. The motions of these bead clouds show that the convective or bulk mixing that occurs during HFIJV is made up of two parts; a turbulent convective exchange with the atmosphere caused by the jet in the trachea and a streaming motion along the airways driven by an interaction between the jet and the expansion and contraction of the airways due to their compliance. These co

Pulmonary interstitial emphysema treated by high-frequency oscillatory ventilation.

Twenty-seven low birth weight infants who developed pulmonary interstitial emphysema (PIE) and respiratory failure while on conventional ventilation were treated with high-frequency oscillatory ventilation (HFOV). The mean birth weight was 1.2 kg (range 0.55 to 2) with gestational age of 28 wk (range 25 to 34). Ten patients died, six of whom had documented sepsis with shock and were therefore excluded from analysis. All patients showed initial improvement on HFOV. Surviving patients showed continued improvement in oxygenation and ventilation at increasingly lower fraction of inspired oxygen an

Neonatal high-frequency oscillatory ventilation: where are we now?

High-frequency oscillatory ventilation (HFOV) is an established mode of respiratory support in the neonatal intensive care unit. Large clinical trial data is based on first intention use in preterm infants with acute respiratory distress syndrome. Clinical practice has evolved from this narrow population. HFOV is most often reserved for term and preterm infants with severe, and often complex, respiratory failure not responding to conventional modalities of respiratory support. Thus, optimal, and safe, application of HFOV requires the clinician to adapt mean airway pressure, frequency, inspirat

Benefits of high frequency oscillatory ventilation for premature infants.

BACKGROUND: High frequency oscillatoryventilation based on optimal lung volume strategy is one of the accepted modes of ventilatory support for respiratory distress syndrome in very low birth weight infants. In 1999 itwas introduced in our unit as the primary ventilation modality for RDS. OBJECTIVES: To evaluate if the shift to HFOV influenced the outcome of ventilated VLBW infants in the neonatal intensive care unit of Carmel Medical Center. METHODS: Data were obtained from the medical charts of VLBW infants born at Carmel Medical Center, and late mortality data from the Israel Ministry of

[High Frequency Jet Ventilation and laryngeal surgery, clinical practice].

High Frequency Jet Ventilation is one in a group of alternative ventilatory techniques characterized by the application of small tidal volumes delivered at higher than physiological rates, followed by the passive expiration. There are two groups of indications: first, High Frequency Jet ventilation was used as a special ventilation mode during diagnostic or surgical procedures in patients with airway pathology; and second, High Frequency Jet Ventilation was employed as a respiratory support technique to improve gas exchange during severe pulmonary failure in infants, children and adults. Successful use of high frequency oscillatory ventilation for pneumomediastinum.

An infant with pneumomediastinum due to mechanical ventilation was successfully treated using high frequency oscillatory ventilation (HFOV). The 3-month-old male had undergone ligation of a patent ductus arteriosus and suffered from barotraumatic pneumomediastinum in the postoperative period. Computed tomography of the chest confirmed the diagnosis. While using conventional mechanical ventilation the respiratory failure worsened. HFOV was instituted and the patient improved. A lower airway pressure by this mode of ventilation provided significant advantages in the patient with an air leak. He

Tidal volume and frequency dependence of carbon dioxide elimination by high-frequency ventilation.

Six patients with chronic respiratory failure received mechanical ventilation with tidal volumes less than or equal to the dead-space volume, at frequencies of 30 to 900 breaths per minute. The rate of elimination of carbon dioxide from the ventilator system during a brief trial of high-frequency ventilation accurately predicted the long-term effectiveness of a given combination of frequency and tidal volume. Below frequencies of about 200 breaths per minute, the volume of carbon dioxide eliminated from these patients was most strongly related to the product of frequency and tidal volume; at h

High-frequency oscillatory ventilation (HFOV) and airway pressure release ventilation (APRV): a practical guide.

Despite advances in ventilator management, 31% to 38% of patients with adult respiratory distress syndrome (ARDS) will die, some from progressive respiratory failure. Inability to adequately oxygenate patients with severe ARDS has prompted extensive efforts to identify what are now known as alternative modes of ventilation including high-frequency oscillatory ventilation and airway pressure release ventilation. Both modalities are based on the principles of the open-lung concept and aim to improve oxygenation by keeping the lung uniformly inflated for an extended period of time. Although a mor
