Hip ultrasound.

In newborns, US has an established role in the detection and management of developmental dysplasia of the hip. Later in childhood, when the limping child is a major diagnostic dilemma, US is extremely helpful in the identification of the varied disease processes underlying this condition, as transient synovitis, septic arthritis, Perthes disease and slipped femoral capital epiphysis. In adolescent practicing sporting activities, US is an excellent means to identify apophyseal injures about the pelvic ring, especially when avulsions are undisplaced and difficult-to-see radiographically. Later o…

Hip instability.

Hip instability is becoming a more commonly recognized source of pain and disability in patients. Traumatic causes of hip instability are often clear. Appropriate treatment includes immediate reduction, early surgery for acetabular rim fractures greater than 25% or incarcerated fragments in the joint, and close follow-up to monitor for avascular necrosis. Late surgical intervention may be necessary for residual symptomatic hip instability. Atraumatic causes of hip instability include repetitive external rotation with axial loading, generalized ligamentous laxity, and collagen disorders like Eh…

Hip ultrasound.

Sonography has been widely utilized for several decades in the diagnosis and follow-up of developmental dysplasia of the hip due to its ability to visualize the nonossified femoral head and portions of the acetabulum in neonates, dynamic capabilities, accuracy, and lack of nonionizing radiation. Furthermore, hip sonography is also used throughout parts of the world to diagnose and guide intervention for both children and adults. The hip joint, tendons, and periarticular structures can be delineated with sonography in a variety of congenital, developmental, infectious, inflammatory, arthritic, …

'Hip' pain.

'Hip' pain is usually located in the groin, upper thigh or buttock and is a common complaint. Slipped capital femoral epiphysis, avascular femoral head necrosis and apophyseal avulsion are the most common diagnoses in childhood and adolescents. Strains and fractures are common in sport-active adults. Osteoarthritis occurs in middle-aged and older adults. Trauma may result in femoral head fracture or typical muscle and tendon sprains and bursitis. Septic or inflammatory arthritis can occur at every age. Septic arthritis, fractures and acute epiphyseal slipping are real emergency cases. Congenit…

Hip pain.

Hip pain can be categorized as either anterior groin pain, lateral trochanteric pain, or posterior buttock pain. Reproduction of the pain during passive and active range of motion of the hip against resistance provides the most accurate localization. The very important intra-articular diagnoses with poorer prognoses and more urgent requirement for treatment, such as a septic joint and hip fracture, often present with anterior groin pain. Trochanteric bursitis is the most common cause of lateral pain next to osteoarthritis. Buttock pain occasionally represents a radiation from lumbar disc or fa…

Hip Pain and Mobility Deficits-Hip Osteoarthritis: Revision 2025.

The 2025 Hip Pain and Mobility Deficits - Hip Osteoarthritis Clinical Practice Guideline (CPG) is a revision of the 2017 CPG and represents the second update for this CPG from APTA Orthopedics. In preparation for this update, a review was conducted on the topic of hip osteoarthritis (OA) to identify articles published after March 2016. The topics addressed in this 2025 CPG revision will specifically attempt to answer the question: What is the evidence to support physical therapy interventions directed at patients with hip OA? Prevalence, pathoanatomical features, and clinical course were discu…

Total hip arthroplasty in hip tuberculosis: a systematic review.

OBJECTIVES: Tuberculosis of the hip joint is a common form of bone tuberculosis that can cause severe joint destruction and affect quality of life. Total hip arthroplasty (THA) is an important way to treat hip joint-related diseases. In recent years, THA has been applied to treat tuberculosis of the hip joint and has achieved certain results. To this end, we conducted a systematic review of total hip arthroplasty (THA) for the treatment of tuberculosis of the hip. METHODS: We searched six electronic databases, including the CNKI Database, Wanfang Database, PubMed, Web of Science, Embase, and …

How the hip-spine relationship influences total hip arthroplasty.

INTRODUCTION: Motion in the spine, pelvis and hips which make up the spinopelvic femoral complex (SPFC) implies mechanical relationships that help maintain trunk balance and optimize hip functionThe aim of this study was to understand the physiology of the SPFC and evaluate the dysfunctions of the SPFC and their implications for total hip arthroplasty considering the hip-spine relationship. METHODS: A review of relevant and comprehensive studies on this subject is reported in order to highlight a pathophysiology that integrates the description of the evaluations of the spine-pelvic and hip pa…

Applying the hip-spine relationship in total hip arthroplasty.

Total hip arthroplasty dislocations that occur inside Lewinnek's anatomical safe zone represent a need to better understand the hip-spine relationship. Unfortunately, the use of obtuse and redundant terminology to describe the hip-spine relationship has made it a relatively inaccessible topic in orthopaedics. However, with a few basic definitions and principles, the hip-spine relationship can be simplified and understood to prevent unnecessary dislocations following total hip arthroplasty. In the following text, we use common language to define a normal and abnormal hip-spine relationship, pres…

[Challenges of primary hip arthroplasty with high hip dislocation].

BACKGROUND: Endoprosthetic care of high hip dislocation is a surgical challenge. The hip anatomy is greatly altered in these patients, including a rather flat and small acetabulum with impaired bone quality and a relevant chance of a bony defect of the acetabular roof. Additionally, the front coverage and in some cases even the dorsal coverage of the hip are missing. The proximal femur is characterized with an increased antetorsion, a coxa valga position and an enlarged greater trochanter. The medullary cavity is narrowed, the offset is reduced, and the absolut leg length can be enlarged. Furt…

[Development of hip preservation in treating adult hip dysplasia].

During the past decades, the field of hip preservation in China has achieved great improvements. Theoretically, the blood supply of the femoral head and the acetabulum along with the mechanism of early-stage hip osteoarthritis has been gradually cleared. The knowledge and application of the periacetabular osteotomy, surgical hip dislocation, and hip arthroscopy has been vigorously promoted. Improved understanding of the mechanism, pathology, and prevention protocols of the secondary hip dysplasia have been obtained, despite the fact that lack of awareness of some mechanism and detail, for exam…

[Effectiveness of total hip arthroplasty for hip infection sequelae].

OBJECTIVE: To evaluate hip function and reinfection of the patients with hip infection sequelae after total hip arthroplasty (THA) treatment. METHODS: A clinical data of 31 patients (31 hips) with hip infection sequelae, who were treated with THA between June 2010 and May 2017, was retrospectively analyzed. There were 18 males and 13 females, with an average age of 46.1 years (range, 20-71 years). There were pyogenic infection in 17 cases, tuberculous infection in 9 cases, and unknown source of infection in 5 cases. The average time from infection to cure was 13.9 months (range, 6-25 months).…

Hip Arthroplasty After Surgical Treatment of Intertrochanteric Hip Fractures.

BACKGROUND: Although use of intramedullary hip screws (IMHS) for intertrochanteric (IT) hip fractures has become more common, limited data have suggested difficulties in conversion to hip arthroplasty. The present study investigates whether conversion of failed IT fracture fixation with an intramedullary vs extramedullary device leads to different rates or types of complications or decreased arthroplasty survivorship. METHODS: One hundred eleven patients were converted to hip arthroplasty after previous surgical treatment of an IT fracture from 2000 to 2010. Seventy hips had been treated with…

Acetabular considerations during total hip arthroplasty for hip dysplasia.

The wide spectrum of anatomic abnormalities that characterize hip dysplasia dictate the need for different reconstructive techniques when hip replacement is required. Multiple factors such as young age and high activity level of this patient population, coupled with the increased complexity of surgery, explain the somewhat elevated historical failure rate of hip arthroplasty in DDH and emphasize the need for careful analysis of each case and selection of the most appropriate reconstruction options. One particular problem specific to acetabular reconstruction is the deficient bone stock that ma…

Total hip arthroplasty in chronic unreduced hip fracture-dislocation.

We treated 15 patients with chronic unreduced hip fracture-dislocations at our hospital; all patients sustained the fracture-dislocations in motor vehicle accidents. All presented to our institution more than 6 months after initial treatment at their local hospitals with uncontained femoral heads; all underwent 1-stage total hip arthroplasty with bone grafting. These patients were monitored for a mean of 71.5 months (range, 36-96 months). All patients had significantly decreased pain, increased function, and increased range-of-motion scores using the Merle d'Aubigné scoring system. All grafts …

Do hip replacements improve outcomes for hip fracture patients?

BACKGROUND: Hip fracture is a common problem among older Americans. Two types of procedures are available for repairing hip fractures: hip replacement and open or closed reduction with or without internal fixation. The assumption has been that hip replacement produces better functional outcomes. Although that is the common wisdom, outcome studies evaluating hip replacement for treatment of hip fracture are few and have not clearly documented its superiority. OBJECTIVES: To compare outcomes of hip fracture patients who receive hip replacement versus another stabilizing procedure (open or close…
