Horner syndrome with transient visual impairment.

A 57-year-old female presented with headache, miosis, and ptosis diagnosed as Horner syndrome (HS). After delaying the recommended diagnostic imaging, she experienced transient, unilateral visual impairment in bright light. The patient was subsequently determined to have a spontaneous internal carotid artery dissection (ICAD) and secondary retinal ischemia with minimal cardiovascular risk factors and no history of preceding trauma. She wore dark glasses, received gabapentin for pain control, and was anticoagulated for a total of 4 months at which time the ICAD resolved despite a residual bleph…

Horner Syndrome Secondary to Thyroid Surgery.

Horner syndrome (HS), caused by an interruption in the oculosympathetic pathway, is characterised by myosis, ipsilateral blepharoptosis, enophthalmos, facial anhydrosis, and vascular dilation of the lateral part of the face. HS is a rare complication of thyroidectomy. A 15-year-old female patient presented with solitary solid and large nodule in the right thyroid lobe. Ultrasound-guided fine-needle aspiration was performed and the cytological examination results were undefined. The patient underwent a total thyroidectomy. On postoperative day 2, she developed right-sided myosis and upper eyeli…

Neuroimaging experience in pediatric Horner syndrome.

BACKGROUND: Horner syndrome in children is rare. The frequency and spectrum of malignancy as the cause of Horner syndrome in children remains unclear. Also unclear is whether the imaging work-up should include the entire oculo-sympathetic pathway or should be more targeted. In addition, the value of cross-sectional angiographic imaging in Horner syndrome is uncertain. OBJECTIVE: To review imaging pathology in a cohort of children with Horner syndrome at a major academic pediatric medical center. MATERIALS AND METHODS: We reviewed a 22-year period of CT and MR imaging studies in children with…

Horner syndrome in neurofibromatosis type 1.

The authors report a rare case of Horner syndrome in a patient with neurofibromatosis type 1 (NF-1). A 31-year-old man visited the clinic with drooping left eyelid. The physical examination revealed ptosis of the left eyelid, miotic pupil, facial anhidrosis, and several skin masses on the chest. The radiological examination of the chest demonstrated a well-defined left posterior mediastinal mass close to the vertebral bodies of the upper thoracic spine at the level of T1-T5. The masses of mediastinum and skin were totally removed. They were diagnosed as neurofibromas. Neurofibromatosis type 1 …

Neuroimaging yield in isolated Horner syndrome.

PURPOSE OF REVIEW: To review the yield of imaging in isolated cases of Horner syndrome in order to better understand if and when imaging will ultimately be beneficial. RECENT FINDINGS: Imaging in Horner syndrome is commonly performed, however, imaging yield in isolated Horner syndrome has not been extensively studied. A few studies have tried to look into the yield of imaging in such cases and derive conclusions from their findings. SUMMARY: With limited evidence-based data, the general recommendation for evaluation in pediatric cases of isolated Horner syndrome is physical examination, urin…

A rare cause of Horner syndrome.

OBJECTIVES: To demonstrate the importance of thorough investigation of patients with Horner syndrome, and to explain the relevant anatomy. CASE REPORT: A 46-year-old man presented with right-sided Horner syndrome. No other abnormality was found. Magnetic resonance imaging showed calcification of the stylohyoid ligament, with a pseudoarthrosis in the mid-portion of the ligament. This pseudoarthrosis was displacing and compressing the internal carotid artery and the adjacent sympathetic chain, causing Horner syndrome. CONCLUSION: In this case, magnetic resonance imaging was invaluable in eluci…

Neck procedures resulting in Horner syndrome.

PURPOSE: To report cases of Horner syndrome caused by interventions in the neck region. METHODS: Retrospective chart review and focused literature search. RESULTS: Six cases of Horner syndrome caused by neck procedures were identified. The study included 5 women and 1 man, ranging in age from 37 to 79 years old. Procedures included coronary artery bypass surgery with neck catheter, carotid endarterectomy, cervical and thoracic sympathectomy, parathyroid surgery, and 2 cases of cervical spine fusion surgery. CONCLUSIONS: Physicians should be aware that procedures involving the neck, even rel…

Postoperative Horner Syndrome After Lung Transplantation.

BACKGROUND: Horner syndrome arises from a disruption along the oculosympathetic efferent chain and can be caused by a variety of pathological and iatrogenic etiologies. We present 3 cases of postoperative Horner syndrome after bilateral lung transplantation. METHODS: The electronic health records of 3 patients with iatrogenic Horner syndrome after lung transplantation were examined, including notes from each patient's medical history, operative and postoperative records, and ophthalmology consultation results. A literature review was performed. RESULTS: All 3 of our patients displayed anisoc…

Horner Syndrome in Birds of Prey.

Horner syndrome, which results from oculosympathetic denervation, has rarely been reported in birds. A retrospective study was conducted in a raptor rehabilitation program to gain further insight into Horner syndrome in birds. Data from 5128 live raptors admitted over 20 years were reviewed. Horner syndrome was identified in 22 individuals from 13 different species. Raptors from the orders Strigiformes, Accipitriformes, and Falconiformes were affected, with the last order being underrepresented. Ptosis of the upper eyelid was the most commonly reported clinical sign in the birds diagnosed with…
