A saddle-horn injury of the pelvis can be considered a separate category of injuries. They occur during horse riding, when a horse rider hits the horn of the saddle and suffers from anteroposterior compression of the pelvic ring. The hits can cause symphyseolysis or even sacroiliac joint injury, and thus also pelvic ring instability. So far, just a few articles have been published dealing with horse saddle-related injuries. A 73-year-old Japanese woman presented with cutaneous horn on the right cheek. The resected tumor was 9 mm in diameter, with 14 mm protrusion, and showed exophytic growth with marked papillomatosis. Histopathology showed proliferation of atypical melanocytes with melanin pigments in the epidermis and dermis under the cutaneous horn. These cells were confined to the base of the cutaneous horn, and did not spread to the surrounding epidermis. The final diagnosis was cutaneous horn malignant melanoma. Occipital horn syndrome, formerly known as Ehlers-Danlos syndrome type IX or X-linked cutis laxa, is an allelic variant of Menkes' syndrome. OBJECTIVE: To evaluate clinical implications of the unicornuate uterus and rudimentary horn.
DESIGN: A retrospective study.
SETTING: A university hospital.
PATIENT(S): Forty-two women who had a unicornuate uterus with or without rudimentary horn.
INTERVENTION(S): The rudimentary horn was removed in 21 cases.

The authors report a case of pregnancy in a rudimentary horn diagnosed by laparotomy. The location could not be suspected by any clinical sign. This pregnancy stopped at 19 weeks of amenorrhoea. Any attempts of induction of labour failed. Laparotomy was necessary to assess the diagnosis and to allow the resection of the uterine horn. The authors present a review of literature.

A rare case of rupture of an noncommunicating rudimentary horn pregnancy is reported. The pregnancy proceeded to 25 gestational weeks, when the rudimentary horn ruptured. The patient had signs and symptoms of massive hemoperitoneum. An emergency exploratory laparotomy revealed rupture of the gravid rudimentary horn and the fetus was lying free in the peritoneal cavity, was delivered breech and died 2 hours later. This reproductive complication was treated successfully by prompt exploratory laparotomy. Some of the factors associated with pregnancy in a rudimentary horn are discussed.

Five cases of rudimentary horn pregnancy are discussed. Two presented as acute ruptured ectopic pregnancy, whereas in 3 the pregnancy was advanced and intact with intrauterine death of the fetuses. The diagnostic difficulties are discussed and in centres where ultrasound facilities are not available, a high index of suspicion in nonresponders to oxytocin induction is emphasized.
