Medicare's 2026 to 2030 Transforming Episode Accountability Model (TEAM): Implications for Anesthesiologists.

Medicare's Transforming Episode Accountability Model (TEAM) represents a significant shift in surgical care reimbursement. TEAM's bundled payment introduces episode-level accountability for cost and outcomes, replacing fee-for-service for discrete activities. TEAM's risk-sharing payments will reshape the clinical and financial environment in which anesthesiologists practice, as their reimbursement is included within the bundle. Their role in perioperative decision-making can strongly influence the performance of hospitals operating under TEAM's risk-sharing arrangements. This article outlines

Creating a new drug service in Edinburgh.

After one year Edinburgh's Community Drug Problem Service has shown that if psychiatric services offer consultation and regular support for drug users many general practitioners will share the care of such patients and prescribe for them, under contract conditions, whether the key worker is a community psychiatric nurse or a drug worker from a voluntary agency. This seems to apply whether the prescribing is part of a "harm reduction" strategy over a long period or whether it is a short period of methadone substitution treatment. Given the 50% prevalence of HIV infection among drug users in the

[Liaison programs in drug abuse-AIDS].

AIDS has acquired a growing importance for psychiatry because of the number of infected people and the increasing attention that these patients need. The major risk group in Spain is the drug users one. These patients share management problems when they are in general hospitals settings, both for their psychiatric aspects and for the consequences of drug abusing. A consultation liaison psychiatry service system improves the attention to these patients and reduces the emergency interventions and the hostility against the staff. The consultation liaison psychiatry system has generated an overall

New paradigms for organ allocation and distribution in liver transplantation.

PURPOSE OF REVIEW: The 'Final Rule,' issued by the Health Resources and Service Administration in 2000, mandated that liver allocation policy should be based on disease severity and probability of death, and - among other factors - should be independent of a candidate's residence or listing. As a result, the Organ Procurement Transplantation Network/United Network for Organ Sharing (UNOS) has explored policy changes addressing geographic disparities without compromising outcomes. RECENT FINDINGS: Major paradigm shifts are underway in U.S. liver allocation policy. New hepatocellular carcinoma

The pathway to establishing and implementing opioid agonist therapy (OAT) for people who inject drugs in Nairobi, Kenya.

Sub-Saharan Africa (SSA) is experiencing a burgeoning population of people who inject heroin. This has created an urgent need for the scale-up of comprehensive harm reduction services including medication for opioid use disorders. The rise in illicit drug use threatens the gains made in the fight against HIV in this high HIV burden region. People who inject drugs (PWID) have an increased risk of HIV acquisition and transmission due to factors such as needle sharing and engaging in high-risk sexual behaviors. Estimates show that PWID have a prevalence of HIV of 18.7%-nearly four times the preva

Interhospital Care Transitions: Coordinating Care Across Institutions.

Interhospital transfers (IHT) are indispensable for providing timely access to specialized services, particularly for rural populations and high-acuity conditions such as trauma, stroke, and myocardial infarction. However, IHTs introduce substantial risks, including transport complications, care discontinuity, increased costs, prolonged hospitalization, and elevated mortality. Although Emergency Medical Treatment and Labor Act and Centers for Medicare & Medicaid Services regulations establish baseline safeguards, coordination deficiencies persist. Centralized transfer centers, standardized doc
