Primary hyperparathyroidism.

Primary hyperparathyroidism is a common endocrine disorder caused by overactivation of parathyroid glands resulting in excessive release of parathyroid hormone. The resultant hypercalcemia leads to a myriad of symptoms. Primary hyperparathyroidism may increase a patient's morbidity and even mortality if left untreated. During the last few decades, disease presentation has shifted from the classic presentation of severe bone and kidney manifestations to most patients now being diagnosed on routine labs. Although surgery is the only curative therapy, many advances have been made over the past de…

Primary hyperparathyroidism.

Primary hyperparathyroidism (PHPT) is a common disorder in which parathyroid hormone (PTH) is excessively secreted from one or more of the four parathyroid glands. A single benign parathyroid adenoma is the cause in most people. However, multiglandular disease is not rare and is typically seen in familial PHPT syndromes. The genetics of PHPT is usually monoclonal when a single gland is involved and polyclonal when multiglandular disease is present. The genes that have been implicated in PHPT include proto-oncogenes and tumour-suppressor genes. Hypercalcaemia is the biochemical hallmark of PHPT…

[Primary hyperparathyroidism].

For the past 40 years, primary hyperparathyroidism has been recognized as a common endocrine disease which is, most often, "non-symptomatic", without the occurrence of nephrolithiasis or osteitis fibrosa cystica. Our knowledge in the pathophysiology has increased largely and diagnosis of primary hyperparathyroidism is usually easy. The only radical treatment is surgery and the surgical indications have been codified by several consensus conferences. For patients who do not undergo surgery, prolonged medical monitoring is needed.

[Primary hyperparathyroidism].

Primary hyperparathyroidism is frequently an incidental finding in asymptomatic patients. Often the diagnosis of primary hyperparathyroidism is made in evaluation for osteoporosis, rarely in the context of hypercalcemic crisis, myopathy, kidney stones, nephrocalcinosis, and osteitis fibrosa. The most frequent cause for primary hyperparathyroidism is benign parathyroid adenoma, reminders have hyperplasia. Primary hyperparathyroidism is defined as hypercalcemia with inappropriately high parathyroid hormone levels. Surgery is the definitive treatment for patients with symptomatic primary hyperpar…

Primary hyperparathyroidism.

Primary hyperparathyroidism is the most common cause of hypercalcemia in the outpatient setting. Phenotypically, it has evolved from a disease of overt symptomatology to one of vague complaints and biochemical diagnosis. Preoperative localization and intraoperative parathyroid hormone have revolutionized the surgical management of these patients. Minimally invasive operations are now common worldwide with low morbidity and high patient satisfaction.

[Primary hyperparathyroidism].

Primary hyperparathyroidism is a common endocrine disease. Since introduction of automated routine measurement of serum calcium the detection of primary hyperparathyroidism has increased significantly. The classical symptoms such as kidney stones, gastrointestinal and bone manifestation are rarely seen nowadays. For this reason, an elevated parathyroid hormone level in a patient with hypercalcemia confirms primary hyperparathyroidism in 95 % of cases. Currently 80 % of these patients are asymptomatic, and therefore criteria for diagnosis and treatment have been discussed on several consensus c…

[Primary hyperparathyroidism].

Primary hyperparathyroidism (PHPT) is characterized by excessive PTH secretion in respect to calcium homeostasis needs, due to parathyroid adenoma (80% of cases), hyperplasia (15-20%), or carcinoma (1-2%). In familial forms of PHPT, several mutations have an established role: menin gene for MEN type 1, RET for MEN type 2a, calcium-sensing receptor gene for familial hypocalciuric hypercalcemia, parafibromin gene for PHPT-jaw tumour and carcinoma. Etiology of sporadic adenomas (80% of PHPT cases) is less defined, being most commonly found a mutation of menin gene or activation of PRAD1 oncogene.…

Primary hyperparathyroidism.

PURPOSE OF REVIEW: This contribution presents research progress concerning primary hyperparathyroidism (pHPT), and the background for recent notable changes in treatment policy. RECENT FINDINGS: Research has clarified that most patients with pHPT require surgery due to risk for osteoporosis, renal stones, and possibly silent complications of renal impairment, cardiovascular disease, and common psychiatric disability. Genetic studies have advanced, but the cause of the disease remains unclear for most patients. Localization methods for parathyroid tumors have improved and increased the interes…

Primary hyperparathyroidism.

Treatment of primary hyperparathyroidism depends on a clear diagnosis based on biochemical confirmation. Most patients have an elevated serum total or ionized calcium level in association with an elevated or inappropriate serum intact parathyroid hormone level. The serum calcium level can be lowered by hydration and by a variety of pharmacologic agents. However, none of these agents is effective in the long-term management of primary hyperparathyroidism. The extraordinarily high success rate of surgery, combined with its low morbidity and the ever-increasing acceptance of minimally invasive te…

[Primary hyperparathyroidism].

Primary hyperparathyroidism is the third most frequent endocrine disorder. The condition required for diagnosis is inappropriately elevated secretion of parathyroid hormone (PTH) with respect to calcemia. Most often, the disease is due to a parathyroid adenoma, i.e. a monoclonal benign parathyroid tumor, less often to a parathyroid hyperplasia. The main tumorogenic mechanisms currently proposed are a DNA rearrangement in the PTH locus (transposition of the PTH promoter upstream to Cyclin D1/PRAD 1 gene) and a mutation of the gene responsible for multiple endocrine neoplasia type I. The clinica…
