Amylin induces hypoglycemia in mice.

Amylin is a 37-aminoacid pancreatic protein that exerts control over several metabolic events such as glycemia and lacticemia. Amylin has long been shown to induce increases in arterial plasma glucose. We decided to investigate whether amylin plays additional roles in the glucose metabolism. We evaluated glucose homeostasis using whole blood from the tail tip of fasting, conscious, unrestrained normal and streptozotocyn-induced diabetic mice following subcutaneous administration of mouse amylin. Subcutaneous injection of 1 μg mouse amylin caused a transient decrease in whole blood glucose in b

Oral aripiprazole-induced severe hypoglycemia.

This case report highlights a very rare adverse drug reaction caused by oral aripiprazole resulting in severe hypoglycemia. A 72-year-old-male patient suffering from Parkinson disease on prolonged carbidopa plus levodopa combination therapy (carbidopa 25 mg plus levodopa 100 mg, thrice daily) for 1.3 years was recently diagnosed with psychosis and was initiated 10 mg/day oral aripiprazole. After 10 days of aripiprazole therapy, the patient experienced symptoms of hypoglycemia and on the 21st day, he was hospitalized for severe hypoglycemia. Other long-term concomitant medications taken by this…

Persistent hyperinsulinemic hypoglycemia of infancy.

Congenital hyperinsulinism (CHI) is the most frequent cause of severe, persistent hypoglycemia in infancy and childhood. We report a case of CHI with diffuse pancreatic abnormality diagnosed preoperatively using the 68Ga octreotide (DOTA NOC) positron emission tomography scan. Genetic analysis revealed homozygous ABCC8 splicing mutation. Subtotal (95%) pancreatectomy was done, and the infant remained euglycemic and was discharged on breast feeds. The patient is continuously followed up and is asymptomatic until 9 months.

[Dizziness: hypoglycemia, hypotension or spasmophilia?].

The consultation for dizziness is a common problem in clinical practice. Because of the apparent lack of specificity of the complaints, there is a rather high risk to prescribe a variety of sophisticated exams, which will not be very helpful in absence of a well oriented anamnesis and a pertinent clinical examination. The present paper aims at describing a global medical approach, essentially based upon a detailed anamnesis (semiological, chronological and therapeutical arguments), to which may be added a few simple clinical and technical complementary data. This strategy should allow obtainin

[Pleural fibroma revealed by hypoglycemia].

Pleural fibroma or solitary fibrous tumor of the pleura (SFTP) is a rare tumor generally discovered fortuitously or in patients with non-specific respiratory symptoms, more rarely with hypoglycemia. We report the case of a 51-year-old man with a history of smoking (30 pack-years) and no other disease history whose chest x-ray revealed an enormous opacity occupying the lower two-thirds of the left hemithorax. This radiological anomaly had led to episodes of lipothymia three months earlier together with mental confusion in the morning as well as an episode of hypoglycemia (0.48 g/l) which rapidl…

Hypoglycemia in multiple trauma victims.

The typical presentation of hypoglycemia involves a diaphoretic patient with a history of diabetes mellitus who is found with an altered mental status. The hypoglycemic patient's presentation may lead the physician to believe that the altered mentation may have been caused by some other condition. Hypoglycemia occurs rarely in the traumatic setting, yet is easily and rapidly diagnosed with bedside testing. A retrospective review was conducted in a university hospital emergency department (ED) (level 1 trauma center) of adult trauma patients with a Glasgow Coma Scale (GCS) score of <15 who had …

Hypoglycemia secondary to endocrine deficiencies.

Hypoglycemia secondary to endocrine deficiencies is uncommon, but it occurs. The endocrine deficiency may be of hypothalamic-pituitary origin or may be due to primary failure of the adrenal gland or the thyroid gland. If hypoglycemia is suspected, the diagnosis should be established immediately by measurement of the blood sugar level. Blood should also be obtained for subsequent use in confirming the diagnosis of the endocrine disease responsible for causing the hypoglycemia. A 50% dextrose in water solution should immediately be injected, and fluid therapy consisting of 5% dextrose in normal …

Hypoglycemia associated with renal failure.

Hypoglycemia associated with renal failure is more common than generally thought. Its occurrence is often a marker of multisystem failure and has an ominous prognostic implication. Its pathogenesis is frequently complex and involves one or several mechanisms. In the evaluation of uremic hypoglycemia, the first step should be the exclusion of obvious causes such as insulin, oral hypoglycemic agent therapy, and the use of drugs known to cause hypoglycemia. Propranolol, salicylates, and disopyramide are among the most commonly implicated agents. Additional triggering events are alcohol consumptio…

Evaluation of death from hypoglycemia.

Hypoglycemia involving a serum glucose of 40 mg/dl or less may lead to death from hypoglycemic coma. Hypoglycemia may be a factor in sudden or unexpected deaths investigated by a medical examiner. We reviewed the data from 54,850 autopsies in a large acute-care medical center to investigate the range of underlying conditions leading to hypoglycemic coma and found 123 cases, which included complications of alcoholism in 41 (33%), drugs in 26 (21%), neoplasia in 26 (21%), conditions producing chronic passive congestion of liver in eight (7%), debilitating neurologic disease in eight (7%), endocr…

Glucose-resistant hypoglycemia in inanition.

Three patients had starvation-related hypoglycemia that was not responsive to the infusion of glucose. The patients resembled fatal cases of hypoglycemia seen very rarely in victims of kwashiorkor. None of the patients had a history of drug ingestion, nor was there any evidence of endocrine insufficiency. Two of the patients had low serum insulin levels. Complete postmortem examinations were done for all three patients and there was no evidence of islet cell abnormalities or of malignant neoplasms. A syndrome of severe hypoglycemia refractory to therapy, that was occasionally seen, in years pa…

[Intrathoracal tumor with accompanying hypoglycemia].

The number of the Doege-Potter-syndrome cases described above is less than one hundred. Even rarer are thoracic tumours with associated hypoglycaemia. The pathophysiologic mechanisms of the syndrome have not been fully elucidated as yet. There are also divergencies about the etiology of hypoglycaemia. Several hypotheses are under discussion. Even our observation shows the difficulty of discovering the genesis of tumour-conditioned hypoglycaemia.

Incidence of hypoglycemia in newborns at risk and an audit of the 2011 American academy of pediatrics guideline for hypoglycemia.

BACKGROUND: Hypoglycemia is low blood glucose level that may negatively affect neurological and developmental prognosis. The American Academy of Pediatrics (AAP), Committee on Fetus and Newborn defined the safe glucose concentrations in the 2011 guideline for newborns at risk for hypoglycemia. This study aimed to investigate the incidence and associated risk factors for hypoglycemia in newborn infants having risk and to assess compliance with the AAP guideline. METHODS: According to 2011 AAP guideline for hypoglycemia, the newborns at risk for hypoglycemia included in this study were divided …

Cerebral glycogen in humans following acute and recurrent hypoglycemia: Implications on a role in hypoglycemia unawareness.

Supercompensated brain glycogen levels may contribute to the development of hypoglycemia-associated autonomic failure (HAAF) following recurrent hypoglycemia (RH) by providing energy for the brain during subsequent periods of hypoglycemia. To assess the role of glycogen supercompensation in the generation of HAAF, we estimated the level of brain glycogen following RH and acute hypoglycemia (AH). After undergoing 3 hyperinsulinemic, euglycemic and 3 hyperinsulinemic, hypoglycemic clamps (RH) on separate occasions at least 1 month apart, five healthy volunteers received [1-13C]glucose intravenou…
