Human parvovirus B19 is a single-stranded DNA virus with a predilection for infecting rapidly dividing cell lines, such as bone marrow erythroid progenitor cells. People with defective cell-mediated immunity (eg, severe combined immunodeficiency syndrome; acquired immunodeficiency syndrome; and patients receiving immunosuppressive therapy, ie, post organ transplant) can develop pure red cell aplasia, in which suppression of erythroid precursors is permanent. Identification of parvovirus inclusions in marrow biopsies and subsequent confirmation of infection by in situ hybridization is important

Oral candidiasis is a significant infection in patients being treated with chemotherapy and radiotherapy for cancer, and in patients who are immunocompromised because of HIV infection and AIDS. Candida albicans is the most common fungal pathogen and has developed an extensive array of putative virulent mechanisms that allows successful colonization and infection of the host under suitable predisposing conditions. The purpose of this review of the literature was to assess the effectiveness of interventions for the prevention of oral candidiasis in immunocompromised patients and in patients trea

Orofacial viral infections are common in immunocompromised patients. Herpes simplex virus (HSV) infections are the most common. Varicella-Zoster virus (VZV) infections are less common, but usually more severe. Epstein-Barr virus (EBV) may produce ulcers, lymphoproliferative syndromes or oral hairy leukoplakia (HL). Human herpes virus 6 (HHV 6) may be the etiology of recurrent aphthous stomatitis, while human herpes virus 7 (HHV 7) has an unclear role. Human herpes virus 8 (HHV 8) is the etiologic agent of Kaposi sarcoma. Human papilloma viruses (HPVs) cause warts, papillomas and epithelial hyp

Histoplasma infections in Europe are rare, and acute disseminated histoplasmosis has only been observed in immunocompromised persons. We describe a case of acute disseminated histoplasmosis in a young, nonimmunocompromised European woman. The probable source of infection was Sri Lanka or the Maldives. At presentation she was severely ill with fever, lymphadenopathy, anemia, thrombocytopenia, hepatosplenomegaly, and polyserositis. Histologically, myelofibrosis and osteosclerosis were observed with extramedullary hematopoiesis. Histoplasma capsulatum yeasts were detected in bone marrow trephine

A report of the species Fusarium proliferatum causing systemic infection in a child with acute lymphoblastic leukemia is presented, with a review of the clinical and laboratory features relating to outcome in disseminated fusarial infections. Thirteen cases of disseminated infection due to Fusarium species have been reported, all but one of which were fatal. Hematologic malignancy is the commonest underlying illness. The frequent occurrence of multiple skin lesions, orbitofacial involvement, and fungemia is in contrast to the otherwise similar disseminated aspergillosis or mucormycosis. Fungem

Strongyloidiasis and aseptic meningitis occurred in a renal transplant recipient who was receiving immunosuppressive drugs. Cure followed therapy with antibiotic administration for 3 days combined with oral thiabendazole, suggesting parasitic infection as the most likely cause of meningitis. Awareness of this disease and need for prompt institution of therapy is stressed. Prophylactic administration of thiabendazole is probably preventive.

The immunocompromised host is at an increased risk for pulmonary and extrapulmonary NTM infections. Where data are available in these specific populations, increased mortality is observed with NTM disease. Prior to starting therapy for NTM disease, providers should ensure diagnostic criteria are met as treatment is long and often associated with significant side effects and toxicities. Treatment should involve 2 to 4 agents and be guided by cultures and antimicrobial susceptibilities. Drug interactions are important to consider, especially in those with HIV or transplant recipients. Whenever p…

Leptospirosis is typically a self-limited febrile illness; when it occurs, meningitis usually develops early in the course. Here, we describe a patient who had engaged in freshwater activities in Kauai that was immunocompromised due to a history of mantle cell lymphoma, autologous hematopoietic cell transplant, and hypogammaglobulinemia. He developed leptospiral meningoencephalitis 11 weeks after illness onset and persistently detectable Leptospira DNA in blood and cerebrospinal fluid along with ongoing clinical illness, despite appropriate treatment.

OBJECTIVE: Owing to the increasing number of patients with acquired immunodeficiency syndrome and immunosuppressed transplant patients, disseminated Toxoplasma gondii has emerged as a potentially fatal pathogen. Common presentations include encephalitis, pneumonia, and myocarditis. The objective of this report is to describe the clinical course, histologic features, and outcome in two immunocompromised patients with disseminated toxoplasmosis presenting with parasitemia and panniculitis.
MATERIALS AND METHODS: Two cases of disseminated toxoplasmosis presenting with parasitemia (patient 1) and

PURPOSE OF REVIEW: To summarize current approaches to isolation precautions and infection prevention and control (IPC) practices in immunocompromised hosts, including patients with hematologic malignancies, those receiving cellular therapies, and transplant recipients, with attention to variability in evidence and practice.
RECENT FINDINGS: Standard and transmission-based precautions remain foundational but often require adaptation for immunocompromised populations due to increased infection severity, prolonged pathogen shedding, and heightened transmissibility. Protective environments, inclu…

Patients with myeloproliferative disorders such as primary myelofibrosis are at an increased risk of opportunistic infections including Noca … dia infections, the limitations of our diagnostic tools, the atypical presentation of empyema in an immunocompromised host, and Hickam's dictum; the idea that a patient can have multiple diagnoses occurring simultaneously.

PURPOSE OF REVIEW: A wide spectrum of heterogeneous conditions can render a patient immunocompromised. Recent years have seen an increase in the number of immunocompromised patients given the earlier detection of conditions that require immunosuppressive therapies, changes in immunosuppressive regimens leading to increased survival or novel therapeutic advancements in oncologic care. Acute respiratory failure (ARF) is the leading cause of critical illness and mortality in this population. This review highlights the spectrum of causes of ARF in immunocompromised patients with a particular focus

BACKGROUND: Strongyloidiasis, endemic in tropical areas, may be asymptomatic in immunocompetent subjects or may cause potentially fatal hyper-infection in immunocompromised patients.
METHODS: Of the 13,885 patients referred to the parasitology laboratory at our tertiary care referral center for stool microscopy, 15 were diagnosed as strongyloidiasis over a 6 year period. We assessed these patients retrospectively.
RESULTS: Most patients were young (median age 32 years, range 3-66) males (12, 80%). Seven patients (46.6%) were immunocompromised. All patients were symptomatic, and symptoms incl

Multidrug-resistant viruses remain a clinically challenging complication in the immunocompromised host. This review discusses mechanisms of viral resistance and treatment options in this context with particular emphasis on the immunocompromised haematology patient. We also outline some of the newer agents and treatment strategies being developed to treat multidrug-resistant viruses.

Pulmonary infections are among the most common causes of morbidity and mortality worldwide, and contribute substantially to annual medical e … t women. This article discusses some of the important causes of acute lung infections in normal and immunocompromised hosts. Because there often is considerable overlap, infections are categorized by the host immune status that is most likely to be associated with a particular pathogen.

Exophiala species are rarely implicated in clinical diseases. In the past 2 years, we have treated phaeohyphomycosis caused by Exophiala species in three immunocompromised patients. Two of these patients presented with subcutaneous abscess or cutaneous verrucous lesions due to Exophiala jeanselmei. The former, an 81-year-old woman, had pulmonary tuberculosis and the latter, a 62-year-old man, had undergone heart transplantation and was receiving immunosuppressive treatment. The third patient, a 62-year-old woman, had acute lymphoblastic leukemia and developed lymphadenitis due to Wangiella (Ex
