Immunotherapy in food allergy.

Food allergies are caused by immune responses to food proteins and represent a breakdown of oral tolerance. They can range from mild pruritus to life-threatening anaphylaxis. The only current consensus for treatment is food avoidance, which is fraught with compliance issues. For this reason, there has been recent interest in immunotherapy, which may induce desensitization and possibly even tolerance. Through these effects, immunotherapy may decrease the potential for adverse serious reactions with accidental ingestions while potentially leading to an overall health benefit. In this review, we …

Trends in cancer immunotherapy.

Modulation of the immune system for therapeutic ends has a long history, stretching back to Edward Jenner's use of cowpox to induce immunity to smallpox in 1796. Since then, immunotherapy, in the form of prophylactic and therapeutic vaccines, has enabled doctors to treat and prevent a variety of infectious diseases, including cholera, poliomyelitis, diphtheria, measles and mumps. Immunotherapy is now increasingly being applied to oncology. Cancer immunotherapy attempts to harness the power and specificity of the immune system for the treatment of malignancy. Although cancer cells are less immu…

Immunotherapy in acute leukemia.

Recent advances in immunotherapy of cancer may represent a successful example in translational research, in which progress in knowledge and technology in immunology has led to new strategies of immunotherapy, and even past failures in many clinical trials have led to a better understanding of basic cancer immunobiology. This article reviews the latest concepts in antitumor immunology and its application in the treatment of cancer, with particular focus on acute leukemia.

Progress in immunotherapy rituximab.

Rituximab is an anti-CD20 chimeric monoclonal antibody that has shown substantial activity. Since its discovery, rituximab has been used with great success in a variety of hematological malignancies. Its success in the management of aggressive lymphomas led to expansion of its use in other conditions such as stem cell transplantation, post-transplant lymphoproliferative disorder, and other non-malignant conditions where B cell activation is thought to be important, such as idiopathic thrombocytopenic purpura and rheumatoid arthritis. The side effects have been remarkably few, particularly, inf

Immunotherapy of myeloid leukaemia.

The treatment of myeloid leukaemia has progressed in recent years with the advent of donor leukocyte infusions (DLI), haemopoietic stem cell transplants (HSCTs) and targeted therapies. However, relapse has a high associated morbidity rate and a method for removing diseased cells in first remission, when a minimal residual disease state is achieved and tumour load is low, has the potential to extend remission times and prevent relapse especially when used in combination with conventional treatments. Acute myeloid leukaemia (AML) and myelodysplastic syndrome (MDS) are heterogeneous diseases whic

Topical immunotherapy in dermatology.

Topical immunotherapy of skin diseases has been used since the 1970s to treat conditions thought to result from an altered immunological state, mainly extensive alopecia areata and resistant viral warts. Despite its effectiveness, only a handful of dermatology departments in the UK currently provide such treatment. Any of three universal contact sensitisers may be used for topical immunotherapy; but diphencyprone (DCP) has advantages over dinitrochlorobenzene (DNCB) and squaric acid dibutyl ester (SADBE). Sensitisation of medical, nursing and pharmacy staff may be avoided by careful handling o…

[Immunotherapy in childhood asthma].

It was accomplished a prospective study with 100 pediatric patients (60 children- and 40 girls) with the diagnoses of chronic rhinitis and bronchial asthma and reactivity allergic to three or more antigens environmental related to exacerbation of disease and whose symptoms were impeding a normal life. It was then included in a program of immunotherapy in compliance with the plan that has in the Centro Regional para la Prevención y el Tratamiento de las Enfermedades Alérgicas (CRPTEA) of the Hospital Universitario de la Universidad Autónoma de Nuevo León, as well as an education and rehabilitat…

[Endocrine involvement in immunotherapy].

Thyroid dysfunction and abnormalities in corticosteroid regulation were observed after clinical use of cytokines (IL2, IFN alpha), molecules with immunostimulating properties. Interleukin 2 administration induces hypothyroidism often associated with anti-thyroglobulin and anti-thyroid microsomal antibody. All these features recall Hashimoto's thyroiditis. Frequency of thyroid dysfunction during immunotherapy varies according to authors between 20 and 40 percent of all patients treated. A correlation between thyroid dysfunction and response to IL2 therapy was reported. Hypothyroidism and hypert…

Immunotherapy of habitual abortion.

Many cases of habitual abortion have been assumed to be due to hyporesponsiveness to the spouse's antigens encountered in pregnancy. Immunization by paternal leukocytes has been used to potentiate the immune response and prevent further miscarriages. This treatment has been highly controversial in terms of efficacy, mode of action, and side effects. More recently immunoglobulin has been used as passive immunization for similar indications. In our experience immunotherapy is effective; 80% of patients have subsequent live births. The most significant results are seen in patients with five or mo…

Cytokines: a new immunotherapy.

Numerous abnormalities in neonatal host-defense mechanisms have been documented over the past decade. Profound disturbances in myeloid progenitor proliferation, bone marrow neutrophil storage pools, a tendency to peripheral neutropenia and significant in vitro abnormalities of mature neutrophil cell effector function all predispose the neonate to a high mortality rate during bacterial sepsis. The recent use of hematopoietic CSFs and other cytokines to enhance host defense mechanisms in the adult has suggested a role for this new form of immunotherapy in the newborn. Several studies over the pa…

NK cell-based immunotherapy.

NK cells have rapidly become a leading platform for cancer immunotherapy. Unlike T cells, NK cells recognize transformed cells through integrated signals from inhibitory and activating receptors, enabling allogeneic "off-the-shelf" therapies without risk of graft-versus-host disease and low rates of cytokine release syndrome and neurotoxicity. Refinement in sourcing of allogenic NK cells, especially umbilical cord blood and induced pluripotent stem cells, ex vivo activation and expansion, CAR engineering, and combinations with novel engagers have resulted in clinically applicable products with…

Immunotherapy for Pleural Mesothelioma.

Immune checkpoint inhibitors (ICIs) have changed the treatment landscape of pleural mesothelioma (PM) over the past decade. This review summarizes the history of immunotherapy in PM, covering studies of single-agent immunotherapy, combination checkpoint blockade, the integration of ICIs into frontline therapeutic strategies, and ICI-VEGF combinations. Also discussed are the emerging role of biomarkers in guiding first-line therapy selection for patients with PM and novel therapeutic approaches. Dual checkpoint blockade targeting PD-1 and CTLA-4 demonstrated clinical efficacy in early-phase stu…

Adoptive T-Cell Immunotherapy.

Epstein-Barr virus (EBV) contributes to diverse malignancies defined by distinct patterns of viral latent antigen expression, which shape tumor immunogenicity and influence responsiveness to T-cell therapies. Adoptive transfer of EBV-specific T cells (EBVSTs) has demonstrated exceptional efficacy in posttransplant lymphoproliferative disease (PTLD), particularly after hematopoietic stem cell transplantation (HSCT), when donor-derived EBVSTs restore antiviral immunity, persist long term, and induce high response rates with minimal toxicity. Advances in manufacturing and the development of multi
