Understanding the long-term impact of flooding on the wellbeing of residents: A mixed methods study.

As the effects of climate change become more visible, extreme weather events are becoming more common. The effects of flooding on health are understood but the long-term impact on the well-being of those affected need to be considered. This mixed methods secondary analysis of a cross-sectional survey examined the extent to which being flooded in the past is associated with ongoing concerns about flooding. Survey data were collected from residents in Hull 11 years after the initial flooding event. Respondents were asked about the floods in 2007 and their current level of concern about flooding.

Facilitating translational science in anxiety disorders by adjusting extinction training in the laboratory to exposure-based therapy procedures.

Extinction learning is suggested to be a central mechanism during exposure-based cognitive behavioral psychotherapy. A positive association between the patients' pretreatment extinction learning performance and treatment outcome would corroborate the hypothesis. Indeed, there is first correlational evidence between reduced extinction learning and therapy efficacy. However, the results of these association studies may be hampered by extinction-training protocols that do not match treatment procedures. Therefore, we developed an extinction-training protocol highly tailored to the procedure of ex

Community Therapist Response to Technology-Assisted Training in Exposure Therapy for Childhood Anxiety Disorders.

Despite the efficacy of exposure for childhood anxiety disorders (CADs), dissemination has been unsuccessful. The current study examined community-therapist response to a brief (90-minutes) training in technology-assisted exposure therapy for CADs. The results indicated that therapists found the training in the therapy approach and technology acceptable, despite endorsing mainly non-exposure-based practice prior to the training. Training also increased positive beliefs about exposure, t (23) = 4.32, p < .000, that persisted 6 months later, t (23) = 4.56, p < .000. In addition, the number of th

Psychological Interventions for Complex Post-traumatic Stress Disorder Symptoms: A Systematic Review.

BACKGROUND: Complex post-traumatic stress disorder (CPTSD) is characterized by the presence of post-traumatic stress disorder symptoms combined with disturbances in self-organization. However, treatment strategies for this newly defined condition remain lacking. METHODS: In this systematic review of international studies exploring psychological interventions for CPTSD in children, adolescents, and adults, we screened PubMed and Google Scholar for relevant literature while adhering to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. RESULTS: In total

Impact of the 2016 US presidential election on OCD symptom presentation: A case illustration.

A recent survey of Americans found that the majority experienced stress during and after the 2016 United States presidential election. Psychosocial, environmental stressors can have a unique impact on symptom presentation in mental health disorders. This manuscript details a case illustration of an individual whose symptoms of obsessive-compulsive disorder were directly linked to the election, as well as how these symptoms were addressed via treatment with intensive exposure and response prevention.

Updating Watson & Marks (1971): How Has Our Understanding of the Mechanisms of Extinction Learning Evolved and Where Is Our Field Going Next?

J. P. Watson and I. M. Marks published a seminal article in Behavior Therapy entitled "Relevant and Irrelevant Fear in Flooding-A Crossover Study of Phobic Patients" in 1971 that paved the way for important theoretical developments and empirical studies that examined the mechanisms underlying extinction learning. Indeed, in the 44 years since their article was published, our knowledge about how exposure therapy works has increased considerably. In this review, we explore the progress our field has made in understanding extinction learning and how Watson and Marks' important work has influenced

The treatment of extreme shyness in maladjusted children by implosive, counselling and conditioning approaches.

This article deals with the reason why little research has been conducted into the area of timidity or shyness in children, and the effect this has on their socialisation and learning. The study attempts to define and treat the problem of timidity by: reducing anxiety both general and specific; developing greater assertiveness in the child's behaviour; promoting the ability to communicate effectively with other people; discovering and treating the areas which are creating feelings of inadequacy by reducing or eliminating them; investigating parental background and trying to influence this, and

Use of Virtual Reality Techniques to Rehabilitate Military Veterans with Post-Traumatic Stress Disorder (Review).

Post-traumatic stress disorder (PTSD) is a mental and behavioral disorder developing after a traumatic event like participation in combat activities. Objective diagnosis of combat PTSD and effective rehabilitation of war veterans is a current multifaceted problem with particularly high social costs. This review aims to evaluate the potential of virtual reality technique used as exposure therapy tool (VRET) to rehabilitate combat veterans and service members with PTSD. The review was written following the guidelines of Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA).

What good are positive emotions for treatment? A replication test of whether trait positive emotionality predicts response to exposure therapy for social anxiety disorder.

BACKGROUND: Positive valence emotions serve functions that may facilitate response to exposure therapy - they encourage approach behavior, diminish perceived threat reactivity, and enhance assimilation of new information in memory. Few studies have examined whether positive emotions predict exposure therapy success and extant findings are mixed. METHODS: We conducted a secondary analysis of an exposure therapy trial for social anxiety disorder to test the hypothesis that patients endorsing higher trait positive emotions at baseline would display the greatest treatment response. Effects of confronting the feared outcome during exposure therapy on the return of fear: An analogue study.

BACKGROUND AND OBJECTIVES: Although exposure therapy is an efficacious treatment for anxiety disorders, fear often returns after treatment. From an inhibitory learning perspective, long-term improvement depends not only on learning that feared stimuli are safe, but also that it is safe to experience the emotional response triggered by these stimuli. Accordingly, the current study examined the effects of occasional threat reinforcement during repeated exposure to multiple cues on the return of fear in snake phobia by incorporating reminders of the feared outcome. METHODS: Snake fearful communi

Exposure therapy in a virtual environment: Validation in obsessive compulsive disorder.

Exposure and response prevention (ERP) is the current first-line psychological treatment for Obsessive-compulsive disorder (OCD). However, substantial inter-individual variability exists in treatment outcomes, including inadequate symptom improvements, and notable refusal and attrition rates. These are driven, in part, by impracticalities in simulating intrusive thoughts within clinical settings. Virtual reality (VR) offers the potential of overcoming these limitations in a manner that allows for finely controlled anxiety-provoking scenarios to be created within supportive clinical settings. Mathematical Characterization of Changes in Fear During Exposure Therapy.

BACKGROUND: During exposure therapy, patients report increases in fear that generally decrease within and across exposure sessions. Our main aim was to characterize these changes in fear ratings mathematically; a secondary aim was to test whether the resulting model would help to predict treatment outcome. METHODS: We applied tools of computational psychiatry to a previously published dataset in which 30 women with spider phobia were randomly assigned to virtual-reality exposures in a single context or in multiple contexts (n = 15 each). Patients provided fear ratings every minute during expo

Exposure therapy for emetophobia: a case study with three-year follow-up.

Emetophobia, also referred to as a specific phobia of vomiting, is a largely under-researched and poorly understood disorder with prevalence estimates of ranging between 1.7 and 3.1% for men and 6 and 7% for women (Hunter & Antony, 2009; Philips, 1985). The current case study, therefore, sought to methodically apply exposure-based behavioral treatment to the treatment of a 26 year-old, Hispanic, female suffering from emetophobia. Although not as powerful as a randomized design, this description may still add to the existing emetophobia literature through the illustration of adaptation of publi

A Whole System Effort: Implementing Narrative Exposure Therapy in Early Intervention for Psychosis Services.

Post-traumatic stress disorder (PTSD) and psychosis may present as part of a continuum of responses to traumatic events. Despite emerging evidence on effectiveness of trauma interventions for PTSD in psychosis, implementation remains poor in Early Intervention for Psychosis (EIP) services around the world, partly due to scarcity of trauma‑adapted protocols and complex training requirements. Narrative Exposure Therapy (NET), a short intervention with minimum training requirements, offers scalability potential. This paper reports preliminary Patient and Public Involvement work with key stakehold
