Occurrence of a pneumothorax secondary to malpositioned nasogastric tube: a case report.

We report on the case of a 79-year-old man undergoing a Whipple operation with postoperative unstable respiratory status. Non-invasive ventilation therapy was started and a nasogastric tube (NGT) was placed to lower the risk of gastric accumulation of air. The NGT was placed following recommended clinical procedures. For further examination of the respiratory status, an X-ray was done, which presented the NGT entering the bronchial tree, following the main bronchus down and a newly occurring pneumothorax in the right lung. The NGT was removed immediately and the pneumothorax was treated by dra

Tube decompression of the dilated colon.

With the technique described herein, decompression of a massively dilated colon is effectively accomplished, allowing resection to proceed safely. This method of decompression has allowed us to perform colonic resection in all patients with toxic megacolon seen in recent years. It is desirable to remove the infected and inflamed colon in such a circumstance. We have not resorted to blowhole colostomies in cases of toxic megacolon, as this leaves the colon, which is the septic source, within the abdominal cavity. This technique has also been used successfully to expedite subtotal colectomy and

The effective placement of Miller-Abbott-tubes under endoscopic guidance. Technical improvements.

In an earlier publication, the intestinal placement of Miller-Abbott-tubes using routine gastroscopic equipment was proposed for the conservative treatment of intestinal obstruction. In addition to the technical advices presented at this early stage of personal experience, we now recommend to 1. insert the tube primarily through the nostrils, 2. extract the tip through the mouth, 3. grasp the leading ligature of the tube tip extracorporally by means of the biopsy instrument and 4. guide the tube tip to the target area by orogastric approach. This technical improvement excludes difficulties in

Navigating the complexities of medicating paediatric patients: Insights from an enteral feeding tube workshop.

Enteral feeding tubes (EFT) are used to administer nutrition into the gastrointestinal tract of patients who are unable to take nutrition via mouth. A wide range of children may require enteral feeding through EFT which are also used to administer medication. However, many medicines are not licensed for administration via this route. Numerous factors can impact EFT medicine administration, including for example, dosage form properties and composition, EFT size, design and material, and operational aspects such as tube flushing. As a result, the risk of sub-optimal dosing and medication error i

[Nasointestinal intubation in the treatment of acute adhesive small bowel obstruction].

Proximal gastrointestinal decompression is essential in the treatment of acute adhesive small bowel obstruction. Improvement of endoscopic methods, surgical instruments and radiological methods of control makes it possible to timely assess lavage and intra-intestinal therapy. The review presents current views on upper gastrointestinal decompression and other options for the treatment of adhesive small bowel obstruction.

Gastric tubes and airway management in patients at risk of aspiration: history, current concepts, and proposal of an algorithm.

Rapid sequence induction and intubation (RSII) and awake tracheal intubation are commonly used anesthetic techniques in patients at risk of pulmonary aspiration of gastric or esophageal contents. Some of these patients may have a gastric tube (GT) placed preoperatively. Currently, there are no guidelines regarding which patient should have a GT placed before anesthetic induction. Furthermore, clinicians are not in agreement as to whether to keep a GT in situ, or to partially or completely withdraw it before anesthetic induction. In this review we provide a historical perspective of the use of

Stomach absorption of intubated insecticides in fasted mice.

Eight 14C-labeled insecticides representing diverse chemical classes were intubated into fasted mice whose stomachs were ligated at the pylorus. Absorption through the stomach was measured at 3 time intervals over a 60-min period and compared to similar absorption studies in the entire gastrointestinal tract. The percent of stomach absorption (as contrasted to total gastrointestinal absorption) varied from 29% (carbaryl) to 10% (nicotine). Distribution following stomach absorption was found to be similar to that in the gastrointestinal tract.

Inadvertent Nasogastric Tube Placement into the Left Bronchial Tree.

Nasogastric tube insertion is a routine procedure that is utilized on a daily basis in our hospitals and long-term care facilities. Blind insertion of these tubes is usually performed by nurses, residents and other health care providers. In this report, we present an incident of misplacement of the nasogastric tube into the bronchial tree causing parenchymal injury and pneumothorax. We also highlight the importance of nasogastric tube placement confirmation and possible pitfalls in interpreting confirmation radiographs.

Nasogastric Tube Syndrome: The Size and Type of the Nasogastric Tube May Contribute to the Development of Nasogastric Tube Syndrome.

Nasogastric tube syndrome (NGTS) is a rare but life-threatening complication associated with nasogastric tube (NGT) placement. The effect of the NGT size and type on the development of NGTS has not yet been fully elucidated. We herein report the case of a 77-year-old man with cerebral infarction who was complicated with NGTS. The immediate removal of the NGT improved the symptoms of NGTS. Although the NGT was passed through the same route during reinsertion, the use of a softer and smaller-sized NGT did not cause any NGTS recurrence. To prevent the development of NGTS, using a NGT that is appr

Pneumothoraces Prevented With Use of Electromagnetic Device to Place Feeding Tubes.

BACKGROUND: A US Food and Drug Administration safety letter warned about the risk for pneumothoraces during feeding tube insertion despite the use of electromagnetic placement devices that provide real-time visualization of feeding tube position. OBJECTIVES: To systematically assess pulmonary placement and pneumothoraces in CORTRAK-assisted feeding tube insertions. METHODS: CINAHL, MEDLINE, and Cochrane databases were searched for studies of CORTRAK-assisted feeding tube insertion. Thirty-two studies documenting pulmonary placement and/or complications of feeding tube insertion were found.

[Project to Reduce the Incidence Rate of Nasogastric Tube Slippage in the Rehabilitation Ward].

BACKGROUND: Nasogastric (NG) tubes are used for providing nutrition to patients who have difficulties swallowing. However, the difficult process of repeatedly inserting NG tubes may lead to gastric erosions. PURPOSE: The rehabilitation ward in this study had an NG tube slippage rate of 2.26% in 2012, significantly higher than the same rate in 2011. A proposal was developed to design better techniques for nurses to perform NG tube insertions in order to decrease the rate of NG tube slippage. RESOLUTION: The following were identified as the major causes of NG tube slippage: (1) caretaker is un
