Four patients with continuous muscle fibre activity (Isaacs-Mertens syndrome) are reported in this communication. Electrophysiological evidence of peripheral neuropathy was present in two patients. Impulse induced repetitive CMAPs were noted in two patients. In one case the repetitive activity increased upon proximal stimulation. Single fibre EMG recordings of the spontaneous activity revealed pathological discharges with variable frequencies. Each pathological discharge comprised of a primary discharge and 1-4 extradischarges. There was marked variability of the interval between the primary d

Autoimmune disorders of neuromuscular transmission are caused by antibodies (abs) directed against membrane proteins at the motor end-plate. Myasthenia gravis (MG) is due, in most cases, to abs against the nicotinic acetylcholine receptor (AChR). Anti-AChR-positive MG actually includes different disease entities: weakness can be confined to extrinsic ocular muscles or can be generalized; patients with generalized MG (G-MG) can be subdivided on the basis of age of onset, HLA association and thymic pathology. About 15% of G-MG patients are anti-AChR-negative; in a proportion of these cases serum

Peripheral nerve hyperexcitability (PNH) typically presents with complaints of muscle twitching, cramps, and muscle stiffness. Symptoms and signs indicating central and/or autonomic nervous system dysfunction also may be reported. An electroclinical spectrum exists, spanning from the milder cramp-fasciculation syndrome to more severe syndromes characterized by continuous muscle fiber activity. It is important to recognize that PNH may be an autoimmune phenomenon associated with antibodies targeting proteins of the voltage-gated potassium channel-complex and, in some patients, a paraneoplastic

BACKGROUND: Spontaneous muscle twitching observed on clinical examination can be challenging to distinguish between myokymia and fasciculation, particularly in peripheral nerve hyperexcitability syndromes where both phenomena may coexist. Because both fasciculations and myokymia may appear in Isaacs syndrome, ultrasonography alone can be misleading when discharge frequency is low, and correlation with needle EMG is essential for accurate differentiation.
CASE PRESENTATION: We report a 53-year-old man with an 11-year history of progressive lower-limb twitching, cramps, and nocturnal muscle pai…

Antibodies to voltage-gated potassium channels (VGKCs) appear likely to be the effector mechanisms in many patients with acquired peripheral nerve hyperexcitability (APNH) syndromes, a group of disorders that include neuromyotonia, cramp-fasciculation syndrome, and Isaacs' syndrome. They may contribute to the associated autonomic changes. Through a central action, they may also be the effector mechanism in those with Morvan's syndrome and in some patients with limbic encephalitis. Evidence supporting this hypothesis includes the increased association of APNH with autoimmune diseases (in partic

Peripheral nerve hyperexcitability (PNH) syndromes are a rare set of neuromuscular disorders that include cramp-fasciculation syndrome (CFS) and Isaacs syndrome (IS). Successful treatment of these diseases has been achieved with antiepileptic medications; however, chronic pain symptoms can persist. We provide a case report of a 25-year-old female who has suffered from painful severe muscle spasms and fasciculations since childhood. With CFS as our working diagnosis, a treatment regimen using interventional pain techniques, including sympathetic chain blocks, ketamine infusions, and trigger poi…

CASE REPORT: We describe a 58-year-old man, without history of radiation therapy, who presented with transient episodic diplopia following sustained lateral gaze lasting around 30-50 seconds. He developed a large angle exotropia of the left eye that gradually returned to normal after 50 seconds. Between episodes he was asymptomatic. Partial resolution of his symptoms was obtained after treatment with carbamazepine.
DISCUSSION: Neuromyotonia must be considered when evaluating a patient with intermittent diplopia.

Spontaneous electrical activity of muscle originates either from one, a few, or a large number of muscle fibers; the responses from the latter are similar to those of motor units. Responses from one or a few fibers are those arising in the end-plate zone of normal muscle and the electrical activity is associated with denervation and myotonia. The motor unit-like activity may either be discharged at randomly varying intervals, as in fasciculations, or at intervals similar to the discharges during voluntary effort, as in hemifacial spasm, Isaacs' syndrome, and in infantile spinal muscular atroph

We studied two patients with abnormal spontaneous muscular activity. The first had widespread fasciculations, painful spasms, delayed muscular relaxation, and hyperhidrosis. Improvement occurred after several years. The second case had generalized paresthesia, mild stiffness, a positive result from Trusseau's test, and was relieved by administration of carbamazepine. Both patients had abnormal conduction velocity. Examination of muscle biopsy specimens disclosed fiber type grouping and increased collateral ramification of motor axons. These observations exemplify symptoms and signs that resemb

A 27-year-old woman noticed diplopia when gazing left and paresthesia of the left face and headache of the left side for 8 months. Abduction and supraduction of the left eye were moderately restricted. Hypoesthesia of the face innervated by the ophthalmic branch of the left trigeminal nerve was detected. Visual disturbance due to optic neuropathy developed 5 months later. MRI with gadolinium revealed a mass lesion extending from the left cavernous sinus to the orbital apex. Ocular neuromyotonia and corresponding diplopia were induced by sustained right gaze. Such episodes occurred almost every

Neuromyotonia or Isaacs' syndrome is a rare peripheral nerve hyperexcitability disorder caused by antibodies against potassium channels of myelinated axons. We present the high-density surface electromyographic (EMG) recordings of a patient with fasciculations and cramps due to neuromyotonia. To characterize the time course of hyperexcitability, we analyzed the interspike intervals (ISIs) between fasciculation potentials, doublet, and multiplet discharges. ISI duration increased within each burst. The ISI histograms found can be explained by the recovery cycle of the myelinated axon and its de

Neuromuscular hyperexcitability is a characteristic of Isaacs' syndrome. Autoantibodies specific for voltage-gated potassium channels (VGKC) or ganglionic nicotinic acetylcholine receptors (AChR) are markers of this disorder. To determine the frequency of these ion channel antibodies and of related neuron- and muscle-specific antibodies in patients with acquired neuromuscular hyperexcitability, we tested serum specimens from 77 affected patients (35 neuromyotonia, 32 cramp-fasciculation syndrome, 5 rippling muscle syndrome, and 5 focal neuromuscular hyperexcitability) and 85 control subjects.

Assessment of spontaneous waveforms recorded in a resting muscle during needle electromyography is important to determine the type of underlying neuromuscular disorder, temporal course of a disease, and severity and prognosis. A variety of different spontaneous waveforms may be recorded. Some waveforms may be recording in patients without neuromuscular disorders, such as end-plate activity or fasciculation potentials, while others occur only in abnormal muscles. Fibrillation potentials are the most common abnormal spontaneous waveform and are encountered in a wide variety of neuromuscular diso

OBJECTIVE: To describe a patient who presented with nonfixed swan neck deformity of the fingers and generalized body aches.
DESIGN: Case report.
SETTING: Tertiary care teaching hospital.
PATIENT: A 38-year-old woman who presented with swan neck deformity of the fingers.
MAIN OUTCOME MEASURES: Electromyographic finding and electromyographic and clinical response to intravenous immunoglobulin.
RESULTS: Needle electromyography revealed continuous motor unit activity in the 50- to 70-Hz frequency during the resting state for all the muscles sampled, which suggests the possibility of neuromyot
