We evaluated whether lower-limb length discrepancy (LLLD) is a risk factor for curve progression in adolescent idiopathic scoliosis (AIS). A systematic review of observational studies was conducted. On April 18, 2025, two independent reviewers searched Medline (PubMed and Ovid), Embase, PsycINFO, BIREME, PEDro, and Google Scholar using a PECO framework: population (AIS), exposure (LLLD), comparator (AIS without LLLD), and outcome (Cobb angle progression). The search identified 730 records; after removing 75 duplicates, 655 titles and abstracts were screened. Of these, 644 were excluded, and 11

STUDY DESIGN: Systematic review, PROSPERO ID: (CRD42023112392).
OBJECTIVE: We aim to provide physicians with an evidence-based stepwise approach to distinguishing the primary cause of plane pelvic obliquity (PO) to guide treatment.
SUMMARY OF BACKGROUND DATA: PO is a frequent clinical finding in both spinal deformity (SD) and leg length discrepancy (LLD) but distinguishing between these etiologies remains challenging due to overlapping clinical and radiographic features and potential multifactorial origins. Misdiagnosis may delay care and worsen patient outcomes.
MATERIALS AND METHODS: PubM

We report three infants with a poorly known form of congenital anterolateral angulation of the tibia with distinctive features seen on lateral roentgenograms. In these films the affected tibia appears to be divided into two segments, one proximal and the other distal, which taper as they approach each other at the site of the angulation, and end separately at the apex of the curve with an intervening radiolucent gap in the anterior tibial cortex. The two tibial segments are originally bridged and held firmly in that position by a well-defined triangular osseous structure located in the concavi

Over a 25-year period, 12 patients had from 2.5 to 5.1 cm operative shortening of the tibia and fibula for leg length discrepancy at between four and 18 years of age. All recovered normal function and there was minor cosmetic impairment in only two cases. The only vascular complication was temporary delay in return of the circulation to the foot after tourniquet removal in one patient. The procedure is valuable for discrepancy of tibial length in patients when they present too late for epiphyseal arrest, when there is doubt as to the appropriate timing for epiphyseal arrest, or when it is unce

AIM: When a leg-length discrepancy (LLD) is severe enough, it can result in lumbar scoliosis and other postural defects. To our knowledge, no study has demonstrated associations between LLD and lumbar curvature using full-length standing radiographs of the lower limbs and lumbar spine. This study aimed to examine the correlations between LLD and lateral curvature of the lumbar spine using standing radiographs.
MATERIALS AND METHODS: Full-length standing radiographs of the lower limbs and spinal column of 113 participants (age range: 10-65 years) obtained between November 2006 and September 20

BACKGROUND: Patients with ankylosing spondylitis often have fusions in the spine and sacroiliac joints, such that it is difficult to compensate for leg length discrepancy (LLD).
METHODS: We retrospectively measured the LLD after total hip arthroplasty (THA) in 89 patients with ankylosing spondylitis from June 2004 to February 2021 at our institute. Patients were divided into two groups based on an LLD of 5 mm. Clinical outcomes were investigated using the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) and Harris Hip Score (HHS). In addition, these points are investigat

Congenital and acquired conditions presenting with leg length discrepancy, leg deformity or short stature are not only a challenge for paediatric orthopaedic surgeons in terms of treatment options but may also involve a number of medical specialties due to the complex clinical manifestations of the diseases. Various surgical treatment options are available for these rare genetic diseases, including bone lengthening and growth inhibition techniques for lower limb discrepancy, as well as guided growth and other surgical procedures for correction of angular deformities. Surgical techniques may be

This study was performed to determine whether overgrowth occurs when treating infantile femoral shaft fractures with a Pavlik harness. Twenty-three patients, aged less than 1 year, treated with a Pavlik harness for a femoral shaft fracture were included. Shortening at the time of injury was recorded on radiographs and compared with those obtained at the follow-up visit 18 months or later. Average overgrowth was 5 mm (1-18 mm). Overgrowth occurs in infants following treatment of femoral fractures with a Pavlik harness. Initial shortening of up to 18 mm was acceptable and resulted in a leg-lengt

Three hundred twenty-one males, ages 18-55, had standardized tests to determine height, weight, Davenport Index, leg length inequality, determination of flexion and extension torques, flexion/extension balance, range of motion, straight leg raising, and lumbar lordosis. A total of 106 (33.0%) had never experienced low-back symptoms; 144 (44.9%) had or were having moderate low-back pain (LBP); and 71 (22.1%) had or were having severe low-back symptoms. These three subgroups showed no significant differences in height, weight, Davenport Index, lumbar lordosis, or leg length inequalities. LBP pat…

We present a technique for reconstruction of the legs in patients with soft-tissue loss and formation of large scars with retraction of this tissue in the pretibial region. In such patients, a subcutaneous tissue expander is placed in the region adjacent to the scar tissue. With expansion, we obtained sufficient skin for use in the reconstruction, and the resulting asymmetry in leg diameter was compensated for by means of one or two calf prostheses, depending on the patient.

Linear scleroderma is an unusual form of localised scleroderma, mainly affecting the legs and occurring primarily in children. Sometimes the linear lesions may extend to involve the underlying muscles and bones, with severe disturbances in growth and possibly flexion deformities of the legs. In this study, two cases suffering from linear scleroderma of the legs are presented.

Congenital diastasis of the inferior tibiofibular joint presents clinical problems of talipes equinovarus, ankle diastasis, and leg-length inequality. In the past, foot disarticulation and prosthetic fitting has often been chosen as a solution because of the difficulty involved in functional reconstruction. The authors report three additional cases of congenital diastasis of the inferior tibiofibular joint that were satisfactorily treated by lengthening of the tibia and fibula and foot repositioning by the Ilizarov technique. After treatment, all patients had a plantigrade, functional foot wit

BACKGROUND: Limb lengthening procedures are performed for various indications, including limb length discrepancies (LLDs) and short stature. This systematic review and meta-analysis compares the efficacy and safety of the newer motorized intramedullary lengthening nails (MILNs) with the traditional alternative techniques (ATs) for femoral limb lengthening.
METHODS: We conducted a comprehensive literature search in the Medline, Embase, Cochrane, Web of Science, and Scopus databases, inclusive of all dates through July 1, 2023, and without language restrictions. Factors mediating outcomes inclu

Limb length discrepancy (LLD) is a frequent complication following total hip arthroplasty (THA) often associated with patient dissatisfaction. Radiographic landmarks are commonly used to determine limb length, but their reliability and accuracy remain to be validated. One-hundred and sixty-two preoperative standing pelvic radiographs from patients undergoing THA were measured using four common landmarks (teardrop, ischial tuberosity, obturator foramen, and iliac crest.) LLD and angular differences between measurements were obtained. Comparison of these landmarks for measuring leg lengths showe

Distraction osteogenesis (DO) is a technique that can be successfully used to treat leg length discrepancies and to reconstruct long defects of bone. By gradual distraction of the bone fragments by approximately 1 mm per day, hypervascularization and an increased bone healing potential (growth stimulus) occur. So far, DO has almost exclusively been used longitudinally to generate new biologically active bone. Chronic wounds, ulcers and osteitis, especially in the region of the foot, are always a challenge for the treating surgeon, since treatment is tedious, the outcome uncertain and associate

Limb length inequality or discrepancy (LLD) occurs when there is a difference in length between 2 limbs or when deviation exists from a normally expected length for a given age. The magnitude of the discrepancy is defined as the difference between the 2 extremities. Aside from congenital etiologies, LLD can also arise from infection, paralysis, tumors/neoplasm, and surgery. Approximately 70% to 90% of the world's population has some elements of LLD with compensation allowing for tolerance and potentially masking the extent to which one limb could be significantly shorter either functionally or
