Nourish and nurture: development of a nutrient ocular lubricant.

PURPOSE: The authors aimed to produce a new tear substitute capable of providing both lubrication and nutrition, based on a novel nutrient-containing therapeutic ocular surface medium (TOSM). METHODS: Viscous substances, including hypromellose (HPMC), carbopol, and sodium hyaluronate (SH) were added to the TOSM at various concentrations. Three commercial preservative-free artificial tear substitutes, Hypromellose (Pharmacy of Moorfields Eye Hospital, London, UK), Thilo-Tears (a carbomer; Alcon Pharma GmbH, Freiburg, Germany), and Vislube (a hyaluronate; Chemedica AG, Munich, Germany) were use

Anaphylaxis to an urethral lubricant: chlorhexidine as the "hidden" allergen.

We report on a severe allergic reaction during urethral catheterisation, initially erroneously attributed to natural rubber latex (NRL). However, the negative investigations for latex, the uneventful further applications of NRL, the positive skin test and basophil activation test for chlorhexidine strongly support diagnosis of chlorhexidine anaphylaxis. This case illustrates that it is not widely appreciated that "innocent" products such as urethral lubricants may elicit serious life-threatening reactions. Careful inspection of the medical records is once again re-emphasised to be mandatory in…

Comparative analysis of condom lubricants on pre & post-coital vaginal swabs using AccuTOF-DART.

In this study we demonstrate the use of Direct Analysis in Real Time Mass Spectrometry (DART) as a powerful tool for detection of nonoxynol in vaginal fluid post contact with a condom, enabling rapid tracing and added evidences in sexual assault crimes. Vaginal fluid was sampled using cotton swabs and glass rods and measured directly with DART. Sample preparation using water, hexane, methanol, and dichloromethane extraction, was explored for comparison and optimization of signals. Nonoxynol was detected up to eight hours after sampling. Optimal sampling conditions and mass spectrometry paramet

Effects of lubricants and wash solutions on semen evaluation in a fertility clinic laboratory.

OBJECTIVE: The complex nature of semen components and the various collection procedures make standardization of semen analysis (SA) challenging. Therefore, the main goal of this study was to optimize and improve the quality and utility of the SA report. METHODS: Semen samples (n = 20) were split into 2 aliquots to compare 2 isolation gradients. Samples incubated in the different wash medium were evaluated for motility, forward progression, morphology, and suitability for intrauterine insemination (IUI). Another group (n = 20) was evaluated for IUI utility without and with a lubricant to compa

Effects of steroids and lubricants on electrical impedance and tissue response following cochlear implantation.

The present study examined the effects of steroids and lubricants on electrical impedance and tissue response following cochlear implantation in animal models. Guinea pigs were implanted following either no treatment, or intrascalar injection with dexamethasone, triamcinolone, sodium hyaluronate or saline. Cats were implanted following either no treatment, or intrascalar injection with dexamethasone, triamcinolone or a mixture of triamcinolone with sodium hyaluronate. In guinea pigs, impedance changes and intracochlear tissue response were less for the hyaluronate and saline groups. In cats, i…

Pain during Female Urethral Catheterization: Intraurethral Lubricant Injection versus Catheter Tip Lubrication--A Prospective Randomized Trial.

PURPOSE: Urethral lubrication during catheterization can be performed by instilling the gel directly in the urethra or by pouring the gel on the catheter tip. In this study we compared the pain level associated with each technique during female urethral catheterization in the setup of a multichannel urodynamic study. MATERIALS AND METHODS: A total of 94 women with a mean ± SD age of 55 ± 14 years who were referred for a multichannel urodynamic study were prospectively randomized into 2 groups according to lubrication technique, including 1) instillation of 5 ml 2% lidocaine gel in the urethra

Slippery when charged: hydration lubrication in hydrogels.

Hydrogels are hydrated polymer networks with promising biomedical applications due to their microstructural similarity to biological tissues at sliding interfaces, such as the articular cartilage. These tissues are highly hydrated, charged, and operate in ionic environments. A fundamental understanding of lubrication at charged hydrogel interfaces is thus essential to advance practical applications. Hydration lubrication has recently been proposed to govern hydrogel lubrication. Here, a thin film of water remains confined under pressure, enabling friction coefficients below 0.01, a condition r

Optimizing Solid Lubricity in Complex Particle Flow.

Granular matter flow is a complex many-body problem, especially for complex particles. Particle flow has been enhanced using solid lubricants, more recently redefined as network disruptors (NDs), given their role in in situ disrupting of force chains. Herein, we evaluate attributes of a recently reported ND to establish a criterion that can be adopted to enhance the fluency. We evaluate the effect of fiber shape, curl, lumen(s), mechanical strength, and surface heterogeneity on rheological flow energy. Cylindrical fibers showed enhanced fluency relative to flat fibers, while carbonization (pyr…

Reduced Sliding Friction of Lubricant-Impregnated Catheters.

During urethral catheterization, sliding friction can cause discomfort and even hemorrhaging. In this report, we use a lubricant-impregnated polydimethylsiloxane coating to reduce the sliding friction of a catheter. Using a pig urethra attached to a microforce testing system, we found that a lubricant-impregnated catheter reduces the sliding friction during insertion by more than a factor of two. This suggests that slippery, lubricant-impregnated surfaces have the potential to enhance patient comfort and safety during catheterization.

Lubricating drops for contact lens discomfort in adults.

BACKGROUND: Contact lens discomfort is a symptom-based clinical diagnosis that affects 13% to 75% of contact lens wearers. The Tear Film and Ocular Surface Society defines contact lens discomfort as "a condition characterized by episodic or persistent adverse ocular sensations related to lens wear either with or without visual disturbance, resulting from reduced compatibility between the lens and ocular environment, which can lead to decreased wearing time and discontinuation from lens wear." Signs of the condition include conjunctival hyperemia, corneal and conjunctival staining, altered blin

Bioinspired Lubricity from Surface Gel Layers.

Surface gel layers on commercially available contact lenses have been shown to reduce frictional shear stresses and mitigate damage during sliding contact with fragile epithelial cell layers in vitro. Spencer and co-workers recently demonstrated that surface gel layers could arise from oxygen-inhibited free-radical polymerization. In this study, polyacrylamide hydrogel shell probes (7.5 wt % acrylamide, 0.3 wt % N,N'-methylenebisacrylamide) were polymerized in three hemispherical molds listed in order of decreasing surface energy and increasing oxygen permeability: borosilicate glass, polyethe

Osteochondral junction leakage and cartilage joint lubrication.

BACKGROUND AND OBJECTIVES: Previous studies have shown that there is potentially interstitial fluid exchange between cartilage tissue and the subarticular spongiosa region in the case of injury or disease (e.g., osteoarthritis and osteoporosis). Interstitial flow is also required for cartilage lubrication under joint load. A key question then is how cartilage lubrication is modified by increased interstitial fluid leakage across the osteochondral junction. Thus, the purpose of this study is to develop a numerical model to investigate changes in cartilage lubrication with changes in osteochondr

Lubricating effect of sialomucin and hyaluronan on pleural mesothelium.

Coefficient of kinetic friction (μ) between rabbit visceral and parietal pleura, sliding in vitro at physiological velocities and load, increases markedly after blotting mesothelial surface with filter paper; this increase is only partially reduced by wetting blotted mesothelium with Ringer solution. Given that mesothelial surface is covered by a thick coat with sialomucin and hyaluronan, we tested whether addition of sialomucin or hyaluronan solution after blotting lowers μ more than Ringer alone. Actually, these macromolecules lowered μ more than Ringer, so that μ was no longer significantly
