PURPOSE: Kienböck's disease affects the lunate bone, and osteoarthritic changes progress as the disease advances; however, relatively few studies have reported the arthroscopic findings of Kienböck's disease, and these reports have been in small populations. The purpose of this study was to review arthroscopic findings in a larger population than studied in previous reports.
METHODS: We retrospectively reviewed 57 patients who underwent radial osteotomy for Kienböck's disease after arthroscopy of both the radiocarpal and midcarpal joints. All arthroscopic findings were classified as follows i…

We report a case of complete palmar dislocation of the lunate in a rheumatoid patient. X-rays showed a normal bone structure of the lunate without sclerosis or collapse and on MRI an almost normal signal intensity was found. The lunate was removed. Histological examination showed complete necrosis of both marrow and bone cells, and tetracycline labelling showed no fluorescence. This case illustrates that neither X-ray nor magnetic resonance imaging (MRI) can detect complete bone necrosis. When X-ray or MRI changes do occur, these are indications of cellular events following some degree of spon

Kienböck's disease is known for its difficulty in being diagnosed and treated at early stages; option treatments are few and most of them quite aggressive. The author describes his experience with arthroscopic assisted lunate bone grafting. Three patients with diagnosis of stage I avascular necrosis of the lunate (average age: 45 years), were treated. Before surgical procedure, the patients underwent to a conservative treatment. After harvesting the bone graft from the volar surface of the radius, arthroscopic bone grafting was performed. At an average follow-up of 13.5 months (9-15), all the …

In cases of distal radius fractures, the fixation of the volar lunate facet fragment is crucial for preventing volar subluxation of the carpal bones. This study aims to clarify the sex differences in the volar morphology of the lunate facet of the distal radius and its relationship with the transverse diameter of the distal radius. Sixty-four CT scans of healthy wrists (30 males and 34 females) were evaluated. Three-dimensional (3D) images of the distal radius were reconstructed from the CT data. We defined reference point 1 as the starting point of the inclination toward the distal volar edge

PURPOSE: Diagnosis and treatment remain controversial for Kienböck disease. A few reports have correlated magnetic resonance imaging (MRI), which is essential for early diagnosis, and histopathology of Kienböck biopsy specimens, but histopathological correlations of whole lunate bones or histological slices compared with MRI images are lacking. The purpose of this study was to compare presurgical MRI scans with corresponding histological slices of Kienböck-diseased lunates.
METHODS: We excised whole lunates at the time of surgery from 6 patients with Kienböck disease (stage IIIB) undergoing t…

Kienböck disease (KD) is a progressive condition and difficult to manage. A number of treatment algorithms exist but there is no clear consensus as to which method produces the best outcome, particularly for Lichtman stage III disease. The majority of the current treatment options for KD emphasize lunate unloading and restoration of biology with bone graft. However, less attention has been given to the protection of the lunate during revascularization process. We report a novel technique of lunate decompression and bone grafting, combined with wrist distraction with or without radial shortenin

The intraosseous and extraosseous vascularity of carpal bones was studied in 75 cadaver limbs. Vascular patterns were correlated with the clinical incidence of avascular necrosis, and at-risk patterns of vascularity were identified. The carpal bones were found to fall into three groups. Group 1, which consisted of the scaphoid, the capitate, and eight percent of the lunates, had either vessels entering only one surface or large areas of bone that were dependent on a single vessel. This group was the most vulnerable to posttraumatic avascular necrosis. The bones of Group 2, the hamate and the t

BACKGROUND AND AIM: Lunate traumatic fractures represent between 0.5% and 1% of carpal bone fractures. Transverse fracture through the body of the lunate (type V) are the least frequent and need surgical treatment if displaced, because it has a high risk of necrosis or pseudoarthrosis. The objective of this paper is to present our experience with arthroscopic treatment in patients with a delay in consolidation after a transverse fracture through the body of the lunate.
METHODS: Two clinical cases of delay in consolidation in transverse fracture through the body of the lunate are presented. Bo

Kienbock disease, or avascular necrosis of the lunate, is an uncommon cause of a painful and stiff wrist. Management options range from conservative treatment in the form of immobilization and corticosteroid injections to a wide variety of surgical treatments that depend on the structural integrity of the lunate, intercarpal relationships, and the condition of the articular cartilage of the wrist. A particularly difficult problem lies in the management of young patient in whom vascularized bone grafting of the lunate has failed but in whom arthritis has not yet developed. Pyrocarbon lunate imp

Kienböck disease is a condition that typically occurs in the "at-risk" patient, in the "at-risk" aspect of the proximal condyle of the "at-risk" lunate. In the active male, repetitive loading causes the stress fracture that commences in the single layer proximal subchondral bone plate. The lunate fracture commences at the point the lunate cantilevers over the edge of the distal radius, and then takes on the shape of the radius. We postulate that the stress fracture violates the parallel veins of the venous subarticular plexus-leading to localized venous hypertension and subsequent ischemia and

BACKGROUND: A connection between lunate type has been described for medical conditions like Kienbock's disease as well as for wrist osteoarthritis, e.g. STT osteoarthritis. Aim of the Study to investigate the possibility of a relationship between lunate type and the type of wrist injury mainly localised to the scaphoid bone.
MATERIAL AND METHODS: 394 PA and lateral radiographs of the wrist (n = 394) were subjected to a detailed assessment by two independent surgeons. The X-rays were selected and analyzed from a computer and archive database run by St. Hedwig Hospital in Trzebnica, from the be

Background: The treatment strategy for distal radius fractures accompanied by volar lunate facet fragment is controversial. In most cases, only the bone fragment size was discussed and a plate for distal placement is selected due to the accompanying volar lunate facet fragment regardless of the direction of bone fragment displacement. In this study, we divided distal radius fractures accompanied by volar lunate facet fragment into dorsal and volar displaced fractures, and treated these surgically based on different treatment strategies. Methods: The subjects were 25 patients with distal radius

Kienböck's disease (KD), an avascular necrosis of the lunate bone, is a rare and progressive condition that may lead to chronic wrist pain and functional limitation. While avascular necrosis associated with COVID-19 has been reported in other skeletal sites, involvement of the lunate bone has not previously been reported. This case adds novel evidence that suggests a potential association between COVID-19 and Kienböck's disease. To demonstrate this potential association, this paper reports a case of Kienböck's disease developing following COVID-19 infection in a 50-year-old woman who was hospi…

We studied the three-dimensional (3-D) shape variations and symmetry of the lunate to evaluate whether a contralateral shape-based approach to design patient-specific implants for treatment of Kienböck's disease is accurate. A 3-D statistical shape model of the lunate was built using the computed tomography scans of 54 lunate pairs and shape symmetry was evaluated based on an intraclass correlation analysis. The lunate shape was not bilaterally symmetrical in (1) the angle scaphoid surface - radius-ulna surface, (2) the dorsal side and the length of the side adjacent to the triquetrum, (3) the

The bone necrosis of the hand is a common disease of the bone-cartilage complex. In principle, every bone of the hand can be affected. Compared to other osteonecroses of the hand the necrosis of the lunate occurs most frequently. Other diseases such as osteonecrosis of the Os scaphoideums (Morbus Preiser) are much rarer. Unfrequent diseases such as osteonecrosis of the metacarpal heads or all carpal bones are restricted to case descriptions. The lunate osteonecrosis leads to osteoarthritis of the wrist if untreated. A detailed clinical examination and adequate radiological diagnosis are essent

PURPOSE: The scapholunate ligament, linking the scaphoid and lunate bones, is crucial for wrist stability and motion. An injury to this ligament disrupts wrist biomechanics, leading to compromised functionality and a higher risk of osteoarthritis.
METHODS: Eight wrist specimens were used on a custom rig to study the effects of scapholunate ligament injury. The rig stabilized the hand while allowing controlled finger flexion. Sensors measured contact pressure, and motion sensors tracked scaphoid and lunate movements. Forces simulated finger flexion, and a finite element analysis model replicat…
