[An examination of the self-reported scale of brief psychopathological symptoms to detect malingering in forensic psychiatric subjects].

OBJECTIVE: To examine the self-reported scale of brief psychopathological symptoms (SBPS) to detect malingering in forensic psychiatric cases. METHODS: Two hundred and six cases with different types of psychiatric problems were tested by SBPS. All cases were separately evaluated by two experts. RESULTS: About 34.5% cases (71/206) were classified as malingering by the cut-off 13 scores of SBPS. Compared with expert's evaluation, SBPS showed a false negative rate of 19.8% and a false positive rate of 1.7%, respectively, with a total accuracy rate of 90.8%. Cases involved in compensations inclu…

Guardianship as a back-door insanity strategy: Parricide, antisocial personality disorder, and malingered psychosis in Brazil.

Intrafamilial homicide, particularly parricide, sits at a critical intersection of forensic psychiatry, criminal justice, and civil rights p … symptoms were vague, internally inconsistent, and strategically framed, raising strong suspicion of malingering; there was no evidence of persistent delusions, formal thought disorder, negative symptoms, or psychosis-driven functional decline independent of substance use and antisocial behavior. The final formulation supported antisocial personality disorder …

Failed Performance on the Test of Memory Malingering and Misdiagnosis in Individuals with Early-Onset Dysexecutive Alzheimer's Disease.

OBJECTIVE: Individuals with early-onset dysexecutive Alzheimer's disease (dAD) have high rates of failed performance validity testing (PVT), which can lead to symptom misinterpretation and misdiagnosis. METHOD: The aim of this retrospective study is to evaluate rates of failure on a common PVT, the test of memory malingering (TOMM), in a sample of clinical patients with biomarker-confirmed early-onset dAD who completed neuropsychological testing. RESULTS: We identified seventeen patients with an average age of symptom onset at 52.25 years old. Nearly fifty percent of patients performed below…

A brain origin for factitious disorder (Munchausen's) with malingering? A single case with an old frontal lobe lesion.

We describe a patient who presented with gender identity dysphoria and stroke-like symptoms who we diagnosed with Munchausen's syndrome (factitious disorder). We discuss whether a brain lesion in the left frontal cortex is a possible risk factor, and support this hypothesis through neuropsychological investigation, EEG abnormalities, and a personality assessment. This case report supports previous suggestions that underlying brain disease/lesions might be risk factors for Munchausen's syndrome (factitious disorder).

Diagnostic utility of the Structured Inventory of Malingered Symptomatology (SIMS) in detecting feigned cognitive impairment in older adults.

OBJECTIVE: The primary aim of this study was to evaluate the diagnostic utility of the Structured Inventory of Malingered Symptomatology (SIMS) in detecting feigned cognitive impairment, specifically through self-reported cognitive and neurological symptoms, among older adults. METHOD: The study was carried out at the University Hospital Kralovske Vinohrady in Prague, Czech Republic. The study included 172 participants: 56 patients with cognitive impairment (33 patients with MCI and 23 patients with dementia), 54 healthy elderly volunteers as a simulation group, and 62 healthy elderly honest

Impact of Demographic and Deployment Related Factors on Structured Inventory of Malingered Symptomatology Performance in Veterans and Service Members.

BACKGROUND: Screening for feigning and exaggeration in military populations is necessary for accurate interpretation of findings in clinical and research settings. The Structured Inventory of Malingered Symptomatology (SIMS) is a commonly used symptom validity measure, but little is known about the impact of non-clinical factors on its performance. The primary objective of this study was to examine relationships among demographic and deployment-related characteristics and SIMS performance in a cohort of veterans and reservists. METHODS: One hundred and sixty-two participants with a history of

Gaslighting in alleged assault ascertained as Munchausen syndrome transcending to malingering in highly counterintuitive self inflicted acid burns.

A factitious disorder leading to the self-infliction of highly counter-intuitive burns was diagnosed in a middle-aged female. The injuries w … a guarded self-immersion of the hands and feet in a corrosive by an illiterate female, followed by malingering to earn livelihood is unprecedented in factitious disorders. The delayed presentation which required amputation of all the limbs to save the life of the patient is a glaring highlight of this case.

An Inventory of Problems-29 Sensitivity Study Investigating Feigning of Four Different Symptom Presentations Via Malingering Experimental Paradigm.

The Inventory of Problems-29 (IOP-29) is a newly developed, self-administered test designed to assist practitioners evaluating the credibility of various symptom presentations. In a recent simulation study comparing real patients against experimental feigners, its classification accuracy compared favorably with that of the Structured Inventory of Malingered Symptomatology and generalized better to different types of symptom reports (i.e., mood- or trauma-related vs. psychosis-related). Extending on these findings, this sensitivity study tested whether the IOP-29 would be similarly accurate in

Neuropsychiatric Disturbances, Self-Mutilation and Malingering in the French Armies during World War I: War Strain or Cowardice?

Between 1914 and 1918, war strain appeared under a number of guises and affected, to varying extents, the majority of French soldiers. The m … . For many physicians, the psychological consequences of this first modern war were synonymous with malingering or cowardice in the face of duty. The stance of French military physicians in relation to their command was not unequivocal and remained ambivalent, swaying between medico-military collusion and empathy towards soldiers experiencing psychological di …

Factitious disorder and malingering in general hospitals in the United States: A retrospective analysis of the National Inpatient Sample 2017-2021.

OBJECTIVE: To examine socio-demographic characteristics, admission features, and comorbid diagnoses associated with hospitalizations assigned discharge codes for factitious disorder imposed on self (FDIS) and malingerer in United States acute care hospitals. METHOD: This retrospective observational cohort study analyzed data from the 2017-2021 editions of the National Inpatient Sample, including adults (≥18 years) discharged with ICD-10-CM codes for FDIS (F68.1) or malingerer (Z76.5). RESULTS: Across the five years, 15,620 acute care hospitalizations carried a discharge diagnosis of FDIS, co

The Utility of the Test of Memory Malingering Trial 1 in Differentiating Neurocognitive, Emotional, and Behavioral Functioning in a Pediatric Concussion Population.

OBJECTIVE: In concussion populations, suboptimal task engagement detected by performance validity tests (PVTs) has been associated with poorer neuropsychological scores and greater post-concussive symptoms (PCS). This study examined if Pass/Fail status on the Test of Memory Malingering-TOMM Trial 1-differentiated the neurocognitive, emotional, and behavioral profile of pediatric patients with concussion. METHOD: This study utilized archival data from 93 patients (mean age = 14.56 and SD = 2.01) with a history of concussion who were assessed at ~5-6 weeks post-injury (mean days = 40.27 and SD …

Assessing Axis I symptomatology on the SADS-C in two correctional samples: the validation of subscales and a screen for malingered presentations.

The Schedule of Affective Disorders and Schizophrenia-Change Version (SADS-C; Spitzer & Endicott, 1978b) is a brief, highly reliable structured interview with clinical applications to diverse populations. This investigation involved reanalyses of data from 2 earlier studies (Rogers, Grandjean, Tillbrook, Vitacco, & Sewell, 2001; Ustad, Rogers, & Salekin, 1998). Focusing on 2 clinical samples from a metropolitan jail, we investigated its subscales via exploratory and confirmatory factor analysis. A good model fit was found (comparative fit index =.92; robust comparative fit index =.94) for 4 su

Can brief measures effectively screen for pain and somatic malingering? Examination of the Modified Somatic Perception Questionnaire and Pain Disability Index.

BACKGROUND CONTEXT: Recent rise in fraudulent disability claims in the United States has resulted in psychologists being increasingly called upon to use psychological tests to determine whether disability claims based on psychological or somatic/pain complaints are legitimate. PURPOSE: To examine two brief measures, Modified Somatic Perception Questionnaire (MSPQ) and the Pain Disability Index (PDI), and their ability to screen for malingering in relation to the Bianchini et al. criteria for malingered pain-related disability published in The Spine Journal (2005). STUDY DESIGN: Examined brie…

Using the inventory of problems - 29 (IOP-29) with the Test of Memory Malingering (TOMM) in symptom validity assessment: A study with a Portuguese sample of experimental feigners.

This study tested whether combining the Inventory of Problems - 29 (IOP-29) with the Test of Memory Malingering (TOMM) would increase sensitivity in the detection of experimentally feigned mental health problems, compared to using either test alone. Additionally, it also evaluated (a) the effects of administration order of these two tests and (b) the cultural and linguistic applicability of these tests to a European Portuguese population. The IOP-29 and TOMM were administered to a community sample of 100 nonclinical, adult volunteers from Portugal, with the instruction to feign mental health p…

Recalled peritraumatic reactions, self-reported PTSD, and the impact of malingering and fantasy proneness in victims of interpersonal violence who have applied for state compensation.

The present study explores the associations between three types of peritraumatic reactions (dissociation, distress, and tonic immobility) and posttraumatic stress disorder (PTSD) symptoms in a sample of 125 victims of interpersonal violence who had applied for compensation with the Dutch Victim Compensation Fund (DCVF). In addition, the confounding roles of malingering and fantasy proneness are examined. Results indicate that tonic immobility did not predict PTSD symptom levels when adjusting for other forms of peritraumatic reactions, whereas peritraumatic dissociation and distress did. Howev…

Clinical and demographic associations of recorded feigning in functional neurological disorder.

Although functional neurological disorder (FND) is common, increasingly recognized, potentially disabling, and treatable, it remains stigmatized, and concerns about feigning persist among clinicians. We examined the prevalence of malingering and factitious disorder diagnoses in individuals with FND, their associated demographic and clinical characteristics, and evidence of clinician bias in the diagnosis of feigning. In this retrospective cohort and case-control study using the international TriNetX electronic health record network, we analysed diagnostic codes (International Classification of…
