Degree of Compression of the Inferior Alveolar Canal on Cone-Beam Computed Tomography and Outcomes of Postoperative Nerve Injury in Mandibular Third Molar Surgery.

PURPOSE: Cone-beam computed tomography (CBCT) offers the advantage of a 3-dimensional representation of the anatomic relationship of the mandibular third molar tooth and the inferior alveolar canal (IAC), as compared to a panoramic radiograph. We hypothesized that a novel method of categorizing the degrees of compression of the IAC were reliable predictors for postoperative nerve injuries. METHODS: We conducted a retrospective analysis of the outcomes in third molar surgery for patients who obtained a CBCT scan in additional to a plain film radiograph over a 12 months period and underwent sur

Risk of inferior alveolar nerve injury with coronectomy vs surgical extraction of mandibular third molars-A comparison of two techniques and review of the literature.

The removal of mandibular third molar teeth is one of the most common oral surgical procedures. In a significant number of patients, it carries a degree of associated morbidity, including damage to the inferior alveolar nerve (IAN). For this reason, practitioners desire the most up-to-date guidance on the most appropriate technique, informed by the best available evidence that will produce the lowest incidence of iatrogenic complications. The aim of this study was to perform a systematic review comparing the effect of coronectomy vs complete surgical extraction of mandibular third molar teeth

Does the Lag Time Between Injury and Treatment Play a Role in Recovery of Inferior Alveolar Nerve Neurosensory Disturbances Following Mandibular Body Fracture?

BACKGROUND: The lag time between injury and treatment (LTIT) plays an important role in reduction of complications in mandibular fractures. The aim of this study was to measure the effect of LTIT on recovery of the inferior alveolar nerve (IAN) neurosensory disturbances (NSDs) following surgical management of mandibular body fractures. METHODS: This was a prospective cohort study. Patients who had a unilateral mandibular body fracture with paresthesia were studied. Paresthesia was evaluated by 2-point discrimination (TPD) test, brush stroke test and self-reporting before and 6 months after th

Management of a deeply impacted mandibular third molar and associated large dentigerous cyst to avoid nerve injury and improve periodontal healing: case report.

As dentigerous cysts increase in size, the risk of postsurgical complications increases as well. Emphasis is usually placed on the risk of neurologic damage caused by trauma during and after surgical removal and of mandibular fracture resulting from the large residual bone defect. However, influence on the periodontal status of neighbouring teeth should also be taken into account. In this article, we present an interdisciplinary, safe, minimally invasive approach to treating large dentigerous cysts associated with deeply impacted third molars. A dentigerous cyst is an epithelial-lined patholog

Radiographic evaluation of root apex to mandibular canal distance in the posterior mandible for assessing risk of inferior alveolar nerve injury during immediate implant placement.

BACKGROUND: Immediate implant placement in the posterior mandible is associated with a risk of inferior alveolar nerve (IAN) injury due to anatomical variations such as lingual concavities and variability in mandibular canal positioning. Accurate three-dimensional assessment of these structures is therefore essential. OBJECTIVES: This study aimed to evaluate the root apex-mandibular canal distance (RAC), prevalence and morphology of lingual concavities, and cross-sectional ridge patterns in the posterior mandible to identify high-risk sites for IAN injury during immediate implant placement.

Beyond the white line: a multicentre retrospective analysis of radiographic and clinical predictors of inferior alveolar and lingual nerve injury following mandibular third molar surgery.

BACKGROUND: Inferior alveolar nerve (IAN) and lingual nerve (LN) injury remain among the most significant complications of mandibular third molar surgery. Earlier studies relied primarily on panoramic predictors such as interruption of the white line (IoWL), often without three dimensional validation. This multicentre retrospective analytical study aimed to identify independent predictors of neurosensory injury and to develop and internally validate a composite risk prediction model. METHODS: Records of 2,850 third molar extractions performed between January 2019 and December 2024 across three

Risk factors associated with inferior alveolar nerve injury after extraction of the mandibular third molar--a comparative study of preoperative images by panoramic radiography and computed tomography.

In this study we investigated the relationships among the risk factors for inferior alveolar nerve injury (IANI), and the difference between preoperative imaging findings on panoramic radiographs and computed tomography (CT), by univariate and multivariate analyses. We determined the following to be significant variables by multivariate analysis: panoramic radiographic signs, such as the loss of the white line of the inferior alveolar canal or the diversion of the canal; excessive haemorrhage during extraction; and a close relationship of the roots to the IAN (type 1 cases) on CT examination.

The effect of root orientation on inferior alveolar nerve injury after extraction of impacted mandibular third molars based on propensity score-matched analysis: a retrospective cohort study.

BACKGROUND: The injury of the inferior alveolar nerve (IAN) is one of the most serious complications of impacted mandibular third molars (IMTMs) extraction. The influence of the root orientation of IMTMs on IAN injury is still controversial. A deeper understanding of the risk factors of IAN injury conduces to better prevention of IAN injury. This study aims to explore whether root orientation is an independent risk factor of IAN injury during IMTMs extraction using the statistical strategy of propensity score matching (PSM). METHODS: This retrospective cohort study included 379 patients with

Histological evaluation of inferior alveolar nerve injury after osteotomy of mandibular buccal cortex using piezoelectric versus conventional rotary devices: a split-mouth randomised study in rabbits.

Up until now, only a limited number of evidence-based studies with different results has evaluated traumatic nerve injury after maxillofacial surgery using piezoelectric devices versus rotary instruments. The present experiment was performed to evaluate damage to the inferior alveolar nerve (IAN), histologically, after osteotomy of the buccal cortex of the mandible using piezoelectric devices versus surgical handpieces. Forty rabbits underwent bilateral osteotomy of the mandibular buccal cortex. For the osteotomy of one side, piezoelectric devices were used, and for the other, conventional rot

Inferior alveolar nerve injury: Correlation between indicators of risk on panoramic radiographs and the incidence of tooth and mandibular canal contact on cone-beam computed tomography scans in a Western Australian population.

AIM: The aim of the present study was to assess risks prior to third molar removal. A 2-D panoramic radiograph or a 3-D cone-beam computed tomography (CBCT) scan can be used to visualize the proximity of the third molar to the mandibular canal. We aimed to correlate panoramic indicators of risk with the incidence of contact between these two structures on CBCT scans. METHODS: Patients were selected from a Western Australian population if they had a panoramic radiograph that illustrated signs of risk of inferior alveolar nerve injury and had a CBCT scan on file. Statistically-significant relat

Effectiveness of concentrated growth factor and laser therapy on wound healing, inferior alveolar nerve injury and periodontal bone defects post-mandibular impacted wisdom tooth extraction: A randomized clinical trial.

Источник пометил публикацию как отозванную

The extraction of wisdom teeth with mandibular impact frequently results in complications including damage to the inferior alveolar nerve (IAN) and malformations of the bone. The objective of this research endeavour was to assess the efficacy of low-level laser therapy and concentrated growth factor (CGF) in facilitating nerve recovery and wound healing in such instances. A total of thirty-one patients (mean age 27.52 ± 5.79 years) who presented with IAN injury after extraction were randomly assigned to one of three groups: control group (which received oral mecobalamin), CGF group (which rece
