Evaluation of the lower face enlargement following mandibular osteotomy: Bilateral sagittal split osteotomy (BSSO) vs supra basilar sagittal Split Osteotomy (SBSSO).

INTRODUCTION: The current most common technique for mandibular split is the sagittal split osteotomy described by Obwegeser and modified by Dalpont and Epker. Several studies have shown a lower face enlargement while employing these techniques. The supra-basilar sagittal split osteotomy described in 2017 seems to be an interesting alternative as mandibular angles remain in continuity with the internal valve. The aim of this study was to evaluate lower face enlargement by comparing two techniques: sagittal split osteotomy and supra-basilar sagittal split. MATERIALS AND METHODS: Cephalograms we

Maxillary Expansion and Mandibular Setback Surgery With and Without Mandibular Anterior Segment Osteotomy to Correct Mandibular Prognathism With Obstructive Sleep Apnea.

Mandibular prognathism is usually treated with mandibular setback surgery. However, this approach reduces the pharyngeal airway space, and can aggravate obstructive phenomena in patients with obstructive sleep apnea (OSA). While maxillary expansion is known to lead to an increase in the pharyngeal airway volume (PAS), its effect on the PAS in mandibular setback surgery has not yet been reported. The authors report a surgical approach with maxillary expansion in 2 patients with mandibular prognathism that was accompanied by OSA: maxillary midsagittal expansion with minimum maxillary advancement

Changes in the gonial angle following bilateral sagittal split osteotomy and vertical ramus osteotomy for mandibular excess.

AIM: The gonial angle plays an important role in ensuring a harmonious facial profile. Changes in this angle after surgery may be an esthetic concern for both the patient and the surgeon. The aim of the present study was to evaluate gonial angle changes after mandibular setback by the bilateral sagittal split osteotomy (BSSO) and vertical ramus osteotomy (VRO) techniques. METHODS: Fifty-eight male patients with mandibular prognathism only were treated from 2004 to 2006 (deformities such as discrepancy of jaws, mandibular setback of more than 10 mm, asymmetry, and vertical discrepancy were exc

Sagittal mandibular osteotomy in a patient with Eisenmenger's syndrome: A case report.

The various abnormalities of occlusion cause significant discomfort to the patient suffering from them. Currently, the surgical treatment of malocclusion in healthy patients is a routine process. The situation is completely different when the patient has a serious disease of the respiratory or cardiovascular system - a condition which may contraindicate such treatment. A 30-year-old female patient, with a class III skeletal defect (open bite and progeny) and Eisenmenger's syndrome was chosen as a clinical case. The DDS-Pro software was selected to plan the operation. The bilateral sagittal spl

Mandibular subsigmoid access osteotomy in the management of parapharyngeal space tumors.

Primary parapharyngeal space tumors are rare presentation in the head, face, neck region. Most of these tumors are benign in nature and significantly cast a challenge in terms of diagnosis and their surgical management. Anatomical location of these tumors, either prestyloid or poststyloid, size of the tumor, and proximity to the vital structures determine the appropriate surgical approach. The surgical approach to the parapharyngeal tumors should provide adequate visibility and access for complete removal, preserving the adjacent vital structures and also preventing recurrence. We report our e

The oblique mandibular chin-body osteotomy for the correction of broad chin.

BACKGROUND: The position and contour of the chin are important components in facial harmony and balance. In the aesthetic facial contouring of the lower face, the mandibular angle, body, and chin should be considered as 1 aesthetic unit. Any developmental broad chin or residual square chin after mandibular angle osteotomy can often make the lower face appear unattractive. For the correction of broad chin, the technique of horizontal-T genioplasty has been introduced; however, manipulation of this technique is sometimes complicated during repositioning and fixation because there is multiple ost

[Study on the changes in the upper airway following osteotomy for the mandibular prognathism].

This study was designed to compare pre-operation, immediate post-operation and over 6 month's follow-up for the mandibular prognathism in order to investigate the changes in the upper airway following mandibular osteotomy. The subjects were 18 patients. Their lateral cephalograms were taken pre-operatively, immediate post-operatively and over 6 months after operation and traced and analysed based on 10 selected items. The following conclusions were reached. 1) The downward displacement of the hyoid bone was recognized right after operation. (P less than 0.01) The hyoid bone tended to move to t…

[Horizontal osteotomy of the mandibular body with transfer of the inferior dental nerve].

In a patient with developmental facial asymmetry resulting from unilateral mandibular hyperplasia, irregular height of the mandibular body was strongly manifested on the affected side. Since a large portion of the inferior border of the mandible predicted to be cut off contained almost the whole of the lower alveolar nerve, transfer of the latter was employed. The nerve was surgically isolated along its course, and secured by being lifted while horizontal osteotomy of the mandibular body was performed. The nerve was accommodated in new groove created at a safe distance from the roots of the lo

The effect of mandibular osteotomy in three patients with hypersomnia sleep apnea.

Hypersomnia sleep apnea (HSA) is characterized by apneic episodes during sleep and daytime hypersomnolence. Patients afflicted as a result of upper airway obstruction have been treated traditionally with permanent tracheostomy. Three patients with HSA and mandibular retrognathism are presented. Each patient had a retrognathic mandible that stemmed from a different cause. Surgical advancement of their underdeveloped mandibles corrected the symptoms of HSA rapidly. The literature concerning HSA is reviewed and the advantages of mandibular surgery in selected cases are discussed.

Comparison of the aesthetic impact on the mandibular angle of two types of osteotomy.

The growing aesthetic demand for projection and definition of the mandibular angle has challenged oral and maxillofacial surgeons, requiring new approaches to osteotomies during orthognathic surgery. This study was undertaken to compare the conventional sagittal split osteotomy (SSO) with anterior oblique osteotomy (AOO) regarding their effects on the mandibular angle in both bone and soft tissue outcomes. A longitudinal observational cohort study was conducted with patients who underwent orthognathic surgery, including pre- and postoperative computed tomography and photographic examinations w

Customizable and Cost-effective Template for Fibular Osteotomies in Mandibular Reconstruction.

The free fibula flap is the gold standard for mandibular reconstruction in head and neck tumors. Traditionally, a titanium reconstruction plate is shaped to measure and align with the mandibular defect after ensuring proper occlusion. These prebent plates are matched against the fibula to estimate osteotomy sites and angles. This technique often requires repeated adjustments for optimal in-setting and heavily depends on the surgeon's experience. Advanced technology now allows virtual surgical planning with 3-dimensional simulation, enabling precise planning of resections and reconstructions. C
