Selecting Medical Conditions Relevant to Exploration Spaceflight to Create the IMPACT 1.0 Medical Condition List.

INTRODUCTION: Medical conditions occurring in spaceflight pose risks to the crew and the mission and these risks will be exacerbated during exploration-class missions. Probabilistic risk assessment is a method used at NASA to quantify this risk for low-Earth orbit operations. Informing Mission Planning via Analysis of Complex Tradespaces (IMPACT) is a next-generation tool suite that will perform these assessments for exploration-class missions. It will require a robust list of medical conditions of significant likelihood and/or consequence to exploration-class missions to accurately inform the

The experience of Teesside helicopter emergency services: doctors do not prolong prehospital on-scene times.

BACKGROUND: The benefits of helicopter emergency medical services (HEMSs) attending the severely injured have been documented in the past. The benefits of doctors attending HEMS casualties have been demonstrated in particular in inner-urban and metropolitan areas. However, for UK regions with potentially less major trauma, concerns have been raised by ambulance services that a willingness of doctors to "stay and play" may lead to unnecessary delays on-scene without any additional benefit to the patient. AIMS: To identify factors that do prolong on-scene time, establish whether doctors "stay a

Ethical concerns regarding operations by volunteer surgeons on vulnerable patient groups: the case of women with obstetric fistulas.

By their very nature, overseas medical missions (and even domestic medical charities such as "free clinics") are designed to serve "vulnerable populations." If these groups were capable of protecting their own interests, they would not need the help of medical volunteers: their medical needs would be met through existing government health programs or by utilizing their own resources. Medical volunteerism thus seems like an unfettered good: a charitable activity provided by well-meaning doctors and nurses who want to give of their time, skills, and resources to help those who would not otherwis…

Sustainable Health Programs: Challenges and Pathways to Success.

Outreach medical programs refer to medical or surgical care initiatives provided by volunteer teams typically from higher income countries and last from a few days to 8 weeks. To appreciate the development and empowerment of the onsite team, the use of "Sustainable Health Programs" (SHP) has become the preferred terminology at Global Smile Foundation. This study aims to review the challenges faced with implementing SHPs and propose a framework with the goal of implementing successful and sustainable programs. The cornerstone of such success is forging long-term, bidirectional partnerships with

Utility of portable ultrasound in a community in Ghana.

OBJECTIVE: In many developing countries, access to medical imaging is limited by availability of resources. Portable ultrasound shows great promise to meet the needs in these countries because it is transportable and relatively inexpensive, and it has a wide range of applications. As part of the Ghana Health Mission, Sekondi-Takoradi, Ghana, we explored the utility of ultrasound in primary care and hospital settings during March 2004. Our objective was to evaluate the clinical utility of a portable ultrasound machine in a variety of physical conditions and multiple clinical scenarios. METHODS

The Development of a Surgical Mission in the Peruvian Amazon: A 15-Year Expedition.

Musculoskeletal disorders and injuries represent a substantial proportion of the global burden of disease. This burden is particularly prevalent in low and middle-income countries that already have insufficient health-care resources. The purpose of this paper is to highlight the vision, the history, the implementation, and the challenges in establishing an orthopaedic surgical mission in a developing nation to help address the epidemic of musculoskeletal trauma.Scalpel At The Cross (SATC) is a nonprofit Christian orthopaedic surgical mission organization that sends teams of 10 to 20 members to

[Emergency in-hospital anaesthesia assistance].

BACKGROUND: Anesthesiologists and nurse anaesthesists have important roles in the management of in-hospital medical emergencies, and in trauma management. We investigated the extent of these services provided by the department of anaesthesiology at St. Olav University Hospital in Trondheim in the year 2002. MATERIAL AND METHODS: Missions involving emergency medical assistance were identified by a computer search in the department's database. We assessed medical aspects, place, time, and type of intervention. RESULTS: We identified a total of 646 missions, of which 501 occurred in the emergen

The value of a spaceflight clinical decision support system for earth-independent medical operations.

As NASA prepares for crewed lunar missions over the next several years, plans are also underway to journey farther into deep space. Deep space exploration will require a paradigm shift in astronaut medical support toward progressively earth-independent medical operations (EIMO). The Exploration Medical Capability (ExMC) element of NASA's Human Research Program (HRP) is investigating the feasibility and value of advanced capabilities to promote and enhance EIMO. Currently, astronauts rely on real-time communication with ground-based medical providers. However, as the distance from Earth increas

Trends in the Application of Invasive Prehospital Interventions by Paramedics: A Six-Year Descriptive Analysis of the German Emergency Medical Services System.

BACKGROUND: The Emergency Paramedic Act (EPA; Notfallsanitätergesetz), implemented in 2014, expanded the legal authority of German paramedics to independently perform defined invasive and pharmacological interventions under specific conditions. However, real-world implementation of these competencies within the physician-based German Emergency Medical Services (EMS) system remains insufficiently described. OBJECTIVE: The aim of this study was to evaluate the frequency, type, temporal development, and regional variation of invasive and pharmacological/therapeutic interventions performed indepe
