Teleophthalmology Programs in Underserved Communities in the United States: A Narrative Review.

Underserved communities in the United States continue to face substantial barriers to accessing eye care, leading to preventable vision loss. This review synthesizes United States-based teleophthalmology programs designed to expand eye care access among under-resourced populations, including older adults, Indigenous communities, children, veterans, people experiencing homelessness, and patients in safety-net systems. Most programs used asynchronous nonmydriatic fundus photography captured by trained technicians in community or primary care settings, with remote specialist interpretation and fr

Responding to measles outbreaks in underserved Roma and Romanian populations in England: the critical role of community understanding and engagement.

Since 2016, the European Region has experienced large-scale measles outbreaks. Several measles outbreaks in England during 2017/18 specifically affected Romanian and Romanian Roma communities. In this qualitative interview study, we looked at the effectiveness of outbreak responses and efforts to promote vaccination uptake amongst these underserved communities in three English cities: Birmingham, Leeds and Liverpool. Semi-structured in-depth interviews were conducted with 33 providers involved in vaccination delivery and outbreak management in these cities. Interviews were analysed thematicall

Is dapsone still relevant in immune thrombocytopenia in resource limited settings?

Immune thrombocytopenia is an autoimmune disorder characterised by autoantibody production against platelets, increased platelet destruction and impaired thrombopoiesis. Steroids are the first-line agents whenever treatment is indicated; however, some patients may not respond and the responders may as well relapse while the dose is being tapered. Side effects of steroids prohibits their long-term use and patients often have to be switched to other agents. Standard drug management with intravenous immunoglobulins and thrombopoietin receptor analogues is difficult to administer in patients from

Examination of the relative importance of hospital employment in non-metropolitan counties using location quotients.

INTRODUCTION: The US Health Care and Social Services sector (North American Industrial Classification System 'sector 62') has become an extr … Budget Control Act 2011 (BCA). RESULTS: Although a very high percentage of rural counties contain medically underserved areas, an examination of location quotients indicates that the percentage of the county workforce employed by hospitals in the most rural counties tends to be higher than for the nation as a whole, a counterintuitive finding. Further, when location quoti …

States' experiences with loan repayment programs for health care professionals in a time of state budget cuts and NHSC expansion.

PURPOSE: The landscape of education loan repayment programs for health care professionals has been turbulent in recent years, with doubling of the funding for the National Health Service Corps (NHSC) and cuts in funding for some states' programs. We sought to understand how this turbulence is being felt within the state offices involved in recruiting clinicians to rural and urban underserved communities. METHODS: We conducted key informant telephone interviews with staff of state offices of rural health, primary care organizations, and/or related organizations within 28 diverse states to answ

Task Sharing in the Diagnosis, Prevention, and Management of Rheumatic Heart Disease: A Systematic Review.

BACKGROUND: Globally, rheumatic heart disease (RHD) is a major contributor to the burden of cardiovascular disease. Major gaps in RHD prevention and treatment have been documented at all levels of health systems in low- and middle-income countries. Task sharing is an approach that could prove effective in remediating bottlenecks in RHD-related care. OBJECTIVES: This study conducted a systematic review to assess the state of the evidence for the use of task sharing in the diagnosis, prevention, and management of RHD. METHODS: Guided by a previously published protocol, we searched various data

Can Geographic Bridging Social Capital Improve the Health of People Who Live in Deprived Urban Neighborhoods?

The growing number of people living in deprived urban neighborhoods, which often have unhealthy environments, is of growing concern to inequality researchers. Social capital could be a resource to help such communities get ahead. In this study, we examined the differential effects of bonding and bridging social capital on self-rated health using two operational definitions, which we call personal and geographic social capital. Bonding and bridging social capital were operationally distinguished as respondents' perceived similarity to other members of a group with respect to personal characteri

Radiotherapy for breast cancer in countries with limited resources: program implementation and evidence-based recommendations.

Radiotherapy is an essential part of the multimodality treatment of breast cancer. Applying safe and effective treatment requires appropriate facilities, staff, and equipment, as well as support systems, initiation of treatment without undue delay, geographic accessibility, and completion of radiotherapy without undue prolongation of the overall treatment time. Radiotherapy can be delivered with a cobalt-60 unit or a linear accelerator (linac). In early stage breast cancer, radiotherapy is an integral part of breast-conserving treatment. Standard treatment includes irradiation of the entire br

Responding to the needs of low-income and homeless women who are survivors of family violence.

Americans have started to recognize interpersonal violence as a major health care issue. Increasingly, clinicians are beginning to recognize both the high rate of victimization among extremely poor women and its health consequences. However, most clinical responses focus on the immediate effects of child abuse, partner abuse, and rape. The long-term medical and mental health consequences and the relationship between early victimization and adult problems are still largely ignored. This article focuses on medical and mental health needs of extremely poor women survivors of interpersonal violenc

Transforming Rural Health Through Strategic Affiliation: Lessons from St. Lawrence Health's Integration with Rochester Regional Health.

Rural hospitals across the United States face growing financial and operational pressures, with many at risk of closure due to workforce shortages, aging infrastructure, and limited access to capital. In response, many rural healthcare organizations are exploring strategic affiliations with larger health systems as a pathway to long-term sustainability. In January 2021, St. Lawrence Health, a three-hospital system serving a geographically large and medically underserved region in northern New York, formally affiliated with Rochester Regional Health. This article examines the experience of integ

Co-producing data-intensive research with an underserved group: a case study and evaluation identifying pathways to impact.

INTRODUCTION: Co-production of research, where researchers and experts by experience work as equal partners throughout a research project, can improve the quality, relevance, implementation and impact of research. However, there is limited evidence on methods for successful co-production in data-intensive research with underserved groups. In partnership with the charity Voice of Young People in Care (VOYPIC) and a group of care experienced young people, the Administrative Data Research Centre Northern Ireland (ADRC NI) piloted and evaluated a co-production approach in a research project that u

Breast cancer in limited-resource countries: diagnosis and pathology.

In 2002 the Breast Health Global Initiative (BHGI) convened a panel of breast cancer experts and patient advocates to develop consensus recommendations for diagnosing breast cancer in countries with limited resources. The panel agreed on the need for a pathologic diagnosis, based on microscopic evaluation of tissue specimens, before initiating breast cancer treatment. The panel discussed options for pathologic diagnosis (fine-needle aspiration biopsy, core needle biopsy, and surgical biopsy) and concluded that the choice among these methods should be based on available tools and expertise. Associations between student social position, training, and plans to care for underserved populations over the course of medical school.

There is a shortage of physicians ready to care for underserved communities in the United States. Given the American structure of Undergraduate Medical Education (UME) and Graduate Medical Education (GME), it is up to students to choose to care for underserved populations; medical school leaders may create programming to incentivize this choice, but that choice is ultimately up to students to make. Who, then, makes this choice? In this article, we examine a large, longitudinal dataset of multiple cohorts of medical students (2016-2020 through 2020-2024; N = 53,417) to identify patterns among w

Under the magnifying glass: a wave of drug price transparency.

OBJECTIVES: Underserved populations often have difficulty accessing care for diabetic eye disease, including screening, referral, and treatment. Although barriers to diabetes care management have been well documented, less is understood about barriers specific to rural and underserved populations. In this study, we interviewed health care professionals to identify barriers to and facilitators of screening for diabetic eye disease and specialty care among underserved patients in Alabama. METHODS: We conducted 12 semistructured interviews with experts in ophthalmology, diabetes care management,

"It's why I'm the doctor that I am": a qualitative study of the long-term outcomes of a longitudinal service-learning medical education program.

BACKGROUND: Service-learning (SL) is widely used in medical education to promote structural competency and engagement with underserved communities. However, most SL evaluations focus on short-term outcomes, leaving limited understanding of whether these experiences shape physicians' perspectives and practice over time. This study examined the long-term impact of the Urban Medicine (UMed) Program, a longitudinal service-learning program, on graduates' professional development and commitment to health equity. METHODS: A retrospective qualitative study was conducted using semi-structured intervi

Standing for Those With Intellectual/Developmental Disabilities.

It is time for people with intellectual/developmental disabilities (IDD), their families, the village that supports them, and the healthcare professionals that are responsible for their well-being to grab their pitchforks and storm the castle to end the purposeful exclusion by our government to designate them a medically underserved population (MUP). We (the authors) define IDD as people having a diagnosis of autism, cerebral palsy, Down syndrome, and hundreds of other genetic syndromes.

Liver abscess: diagnostic and management issues found in the low resource setting.

INTRODUCTION: Liver abscesses are mainly caused by parasitic or bacterial infection and are an important cause of hospitalization in low-middle income countries (LMIC). The pathophysiology of abscesses is different depending on the etiology and requires different strategies for diagnosis and management. This paper discusses pathophysiology and epidemiology, the current diagnostic approach and its limitations and management of liver abscess in low resource settings. SOURCES OF DATA: We searched PubMed for relevant reviews by typing the following keywords: 'amoebic liver abscess' and 'pyogenic
