A double-blind crossover study of sodium valproate and placebo was conducted in five patients with Meige syndrome. CSF neurotransmitter studies were performed at the end of each treatment period. GABA levels were not influenced by the administration of sodium valproate. An increase in HVA levels was observed in every patient, which may reflect an increase in central dopaminergic activity. This finding may explain the trend towards clinical deterioration which was observed during treatment with sodium valproate. Sodium valproate appears to be ineffective in Meige syndrome.

VP and MS are distinct clinical entities arising from the peripheral vestibular system and central extrapyramidal pathways, respectively. We report a rare case of a 69-year-old woman with coexistence of VP and MS. Individualized pharmacotherapy resolved vertigo and markedly improved blepharospasm. We report a patient with Meige syndrome (segmental cranial dystonia) who had neuropathologic changes of Parkinson's disease on postmortem examination. Neuropathologic examination showed typical and atypical Lewy bodies in the pigmented nuclei of the brainstem (substantia nigra, locus ceruleus), the nucleus basalis of Meynert, and the nucleus ambiguus. Three cases of Meige syndrome with severe bilateral facial spasm were reported. All patients suffered from abnormal facial movement characterized by blepharospasm and oromandibular dystonia, and had atrophic changes of bilateral basal ganglia which were recognized by brain CT scan. Bilateral facial nerve block at the foramen stylomastoideum was applied to those three patients. Excellent results were obtained by this block technique in all the patients. The technique is not so sophisticated and is recommended for the treatment of the patient with Meige's syndrome.

In 34 patients with idiopathic blepharospasm, 5 with Meige syndrome, and 11 with hemifacial spasm, local injection of botulinum toxin gave temporary relief from the involuntary movements of the eyelids. Local ophthalmologic side effects were minimal. There are no known systemic side effects. The procedure had to be repeated every 12 weeks. Tolerance to the toxin did not appear.

We present a rare case of a 45-year-old woman with pseudo-Meigs' syndrome and eosinophilic pleural effusion (EPE). She experienced cough, sputum, and dyspnea with a large right pleural effusion. Laboratory tests showed eosinophilia in the blood and pleural fluid. An ovarian tumor and ascites were also detected.
