Pulmonary actinomycosis is described in 17 South Australian koalas necropsied between 2016 and 2019. From these cases, four koalas had secondary hypertrophic osteopathy. Plain radiographical and computed tomography images demonstrated periosteal reaction on multiple appendicular skeletal bones in all cases, including scapula, humerus, ulna, radius, ilium, femur, tibia, fibula, metacarpus, metatarsus and phalanx. Grossly, periosteal surfaces of the metaphyses and diaphyses of long bones were thickened and roughened; microscopically, this was characterised by bi-layered proliferation of well-dif…

A 10-month-old male maremma shepherd dog was presented with chronic diarrhoea, moderate polyuria/polydipsia, lethargy, dysorexia and stiffness. Pain was elicited in the distal parts of all four limbs. Radiographs of the limbs showed increased endomedullary radiopacity and lysis, with thick periosteal proliferations at the metadiaphyseal areas of each radius-ulna and tibia and of the distal metacarpus on one side. Polyclonal hypergammaglobulinaemia was documented and a similar electrophoretic protein pattern was also found in the synovial fluid. Leishmania amastigotes were found in the macropha…

Metastases to the hand and wrist are rare. The lung, breast and kidneys are the most common sites of primary lesions that metastasize in the hand. Phalanges are more commonly involved than metacarpals and wrist. We report the case of a neglected, large metastatic tumour involving a patient’s left wrist and metacarpus originating form renal adenocarcinoma cancer, which appeared 2 years prior to the diagnosis of the primary neoplasm. The tumour was resected, but without oncologic margins. After obtaining histological veri- ication (clear cell renal cell carcinoma) the patient had been proposed …

Secrétan's disease was first described in 1901 as "hard edema and traumatic hyperplasia of the dorsum of the metacarpus." Other names for this process include peritendinous fibrosis, posttraumatic hard edema, and factitious lymphedema. Over the years, many theories have been proposed as to the etiology of the disease--trauma that leads to peritendinous fibrosis, hypersympathetic stimulus after an injury, and self-inflicted trauma. There is no consensus regarding pathophysiology, classification, prognosis, or treatment, but the literature suggests that Secrétan's disease is an injury that is se…

The authors present a congenital arm- and hand deformity in a four year old boy. This "mirror deformity" includes aplasia of the radius, doubled carpus and metacarpus, polydactyly (eight fingers) and aplasia of the thumb. The hypoplastic radial four fingers show brachysyndactyly, the whole upper extremity is hypoplastic but both shoulders are normally developed. The elbow has an extension contraction with a range of movement of 20 degrees in the double olecranons. The wrist has a flexion contracture. At first, pollicisation of the third finger of the hypoplastic radial part of the left hand wi…

Stable wound coverage after extensive soft-tissue loss of the upper extremity remains a difficult problem in the management of large defects … We report a series of 12 emergency and delayed early reconstructions of the forearm, wrist, carpus, metacarpus, and hand using the free rectus abdominis muscle flap with split-skin coverage, demonstrating the versatility of this flap within this special context. Emergency free rectus muscle flap transfer is safe, technically easy, and expandable.

The authors have examined 220 maxillary and mandibular panoramic roentgenograms of 55-55, all 110, between 21-34 years of age alcohol addict males and females with a complete dentitions and intact parodontium. In 19 per cent of patients developed osteoporomalacia, cystic rarefications and multiple pathologic microfractures of the alveolar spongy bone tissues. The calcipenic osteopathia was verified by the routine laboratory disorders of calcium and phosphor metabolisms, besides by the diminution of roentgenmorphometrical index of right 2nd metacarpus and tibia. The osteoporomalacia of alveolar…

The walking cast was used to repair 46 fractures of long bones (distal radius and tibia, metacarpus and metatarsus) and 10 fractures of first phalanx of 35 horses and 21 ponies. The walking cast proved to be highly effective. Even heavily comminuted and 'hopeless' fractures, which could not be treated with osteosynthesis, were treated successfully, because the weight of the animal, a common failure factor in large animal orthopaedics, was relieved from the injured leg. Immediate full weight bearing was possible post operatively. In five cases the walking cast was used in combination with osteo…

Among 1140 patients treated in the last 16 years for any type of carpal fracture, dislocation, or subluxation, 16 (1.4%) patients seen with a traumatic axial disruption of both the carpus and metacarpus were identified. Most of these were crush or blast injuries. According to the direction of instability, the cases were classified into three groups: axial-ulnar disruptions (4 patients), axial-radial disruptions (11 patients), and combined axial-radial-ulnar disruption (1 patient). Treatment consisted of open reduction and percutaneous Kirschner-wire fixation in most cases. The results were les…

The four ulnar metacarpal bones are connected to each other at their proximal and distal ends by strong ligaments. The second and fifth metacarpal bones are the frame of the metacarpus. Surgically, this is of great importance. A fracture of these two bones often requires stabilization, whereas a fracture of the third and fourth metacarpal bones with intact ligaments seldom requires surgery except in cases of severe fragment dislocation. Using external fixation for these purposes it seems to be of utmost interest to define secure pintracks through the soft tissue. In an anatomical study using f…

We describe an infant who died shortly after birth, with Potter sequence and prune belly anomaly, omphalocele, single umbilical artery, imperforate anus and micropenis with empty scrotum. Fetopathological examination revealed multiple vertebral anomalies with rudimentary sacrum, hypoplasia o2 the first metacarpus on right hand, complex cardiovascular anomalies, malrotation of intestines, a dilated and blind-ending cloaca to which both ureters and a bicornuate uterus were connected, normal ovaries, hypoplastic kidneys with cystic renal dysplasia. The descending colon ended blindly and showed a …

A 5-year-old imported Zangersheide gelding was evaluated for SC swellings over both forelimbs and lameness localized to the distal metacarpus. Ultrasound examination of the SC masses was compatible with verminous granulomas. Linear hyperechoic foci were present within the suspensory ligament branches of both forelimbs, suggestive of ligamentous parasitic infiltrates. A diagnosis of onchocerciasis was confirmed on biopsy of a SC mass. The gelding was treated with ivermectin and a tapering course of PO dexamethasone but was eventually euthanized. Necropsy confirmed the presence of SC eosinophili…

Several accessory bones can be found around the wrist, which may pose a diagnostic challenge. Hereby, we report a unique case of a traumatic fracture of an aberrant os styloideum in a 66-year-old male. The patient presented with a swollen dominant right hand and a hematoma on its anterior and posterior aspects, as a result of a fall on an outstretched hand. The diagnosis was based on a CT scan, which showed a fractured accessory bone between the distal portion of the capitate bone and the base of the third metacarpus. Due to its typical location, the ossicle was identified as os styloideum. Th…

Disorders of calcium-phosphate-parathormone balance, are very important issues in ESRD patients, that may lead to severe complications, as dystrophic calcinosis cutis, a rare disease, caused by calcium salt deposits in cutaneous or subcutaneous tissues and many organs. We present the case of a 47 years old woman, in ESRD due to membranous glomerulopathy, treated by peritoneal dialysis, who, after 7 months of dialysis, developed painful masses on second finger and fifth metacarpus of the right hand. Laboratory and instrumental data showed hyperparathyroidism with a parathyroid mass consistent w…

The stability of the carpus is determined by a precise interaction of the osseous and ligamentous elements. The main load of the carpus responsible for the adaptation of concerned tissues is a longitudinal compression caused by the force of the muscles of the forearm with their insertions to the metacarpus and fingers. As a consequence, the proximal row is distracted due to the wedge-shaped form of the hamatum together with the capitatum. It is the function of the mainly oblique orientated ligaments to take these transverse tensional forces, supported by the circular arrangement of the retinac…

The non-cemented Ledoux trapeziometacarpal prosthesis has a high failure rate. In order to better understand the mechanism responsible for pain and loosening, we thoroughly analysed the membranes surrounding the prosthesis and the bone-prosthesis interface in two trapezia, each containing a Ledoux cup, which were resected after unsuccessful implantation. Serial sections, perpendicular to the longitudinal axis of the implanted cup, allowed histological examination of the interface and were used to quantify bone apposition. The tissues surrounding the prosthesis showed a foreign-body reaction to
