BACKGROUND: Because of denervation supersensitivity, a miotic pupil in a sympathetically-denervated eye dilates in response to a dilute or weak alpha-1-agonist drug. A reversal of anisocoria after topical apraclonidine is considered as a positive test result that diagnoses a unilateral Horner syndrome.
HISTORY AND SIGNS: Two women aged 34 and 46 years with a cocaine-confirmed oculosympathetic defect (Horner syndrome) were tested with 1 % topical apraclonidine on separate days.

Normalization of intraocular pressure in primary open-angle glaucoma and narrow-angle glaucoma by single iridencleisis was demonstrated in 551 eyes between six months and seven years postoperatively. The effects of epinephrine on the permeability of the blood-aqueous barrier, stimulated by paracentesis of the anterior chamber, were studied in rabbits following alpha-adrenergic blocking by means of topical or systemic thymoxamine. The alpha-adrenergic block was tested by continuous recording of the systemic arterial blood pressure. The reversal of the vascular effects of epinephrine after administration of thymoxamine was demonstrated at the bloodaqueous barrier. The miotic property of thymoxamine is believed to have had no influence on our results.

Twenty patients with untreated primary open angle glaucoma (POAG) were tested for the presence of cholinergic supersensitivity of the iris using a 0.1% solution of pilocarpine. Nineteen patients had a significant miotic response compared to six of a group of twenty control subjects. We believe that an ocular parasympathetic neuropathy may be an important factor in promoting or perpetuating elevated intra ocular pressure in POAG. The mechanism of this is suggested and several possible areas of further study discussed.

In eyes undergoing vitreoretinal surgery up to now dilatation of a miotic pupil by the pupillary stretching technique could be performed only in aphakic eyes. Now a new needle with a tiny eye at its tip has been developed. With the help of this needle, 4 iris loop sutures can be led around the iris only passing the anterior chamber. This permits pupillary stretching also in phakic eyes. The modified technique was used on 3 phakic and 2 aphakic eyes.

This 75-year-old woman had phacomorphic angle closure, dense nuclear sclerosis, deep set eye, miotic pupil and tight corneal wound during phacoemulsification. Phacoemulsification wound burn was noted at the end of surgery. Tenon was harvested from the inferior conjunctiva, placed over the gape and anchored by two radial corneoscleral 10-0 nylon. Ten days later, anterior optical coherence tomography showed good wound apposition and sutures were removed with visual recovery to 20/25 (6/7.5) without astigmatism.

An 18-year-old woman suffered cardiac arrest and became comatose after ingesting a large amount of proxyphene and salicylates. Six days after admission, her pupils, initially miotic, became fixed and dilated. On the seventh day disc edema developed. This was due to optic neuritis from acute Darvon overdose. Within eight weeks the disc edema resolved into a pale white atrophy with severe loss of vision. This is the first reported case of optic atrophy after propoxyphene overdose.

After the presentation of a newborn with congenital glaucoma, four additional members in two generations of a family were found to be affected with megalocornea. The absence of the disorder in the parents of three affected siblings can be explained by autosomal recessive inheritance, autosomal dominant inheritance or germ-line mosaicism.
