In order to identify motorcycle accident cause factors and countermeasures in Thailand, a large prospective study was undertaken. Researchers conducted on-scene, in-depth investigation and reconstruction of 969 collisions involving 1082 motorcycle riders. Accidents were randomly sampled and included all levels of injury severity. Alcohol proved to be the most outstanding cause factor, with 393 drinking riders in crashes. BACKGROUND: Motorcycle crash victims form a high proportion of those killed or injured in road traffic accidents. Injuries to the head, following motorcycle crashes, are a common cause of severe morbidity and mortality. It seems intuitive that helmets should protect against head injuries but it has been argued that motorcycle helmet use decreases rider vision and increases neck injuries. This review will collate the current available evidence on helmets and their impact on mortality, and head, face and neck injuries following motorcycle crashes.

Since 1975, 26 states have repealed or modified their motorcycle (M/C) helmet laws. Louisiana (LA) reinstated the M/C helmet law in 1982. The medical and financial impact of repeal in Kansas (KS), reinstatement in LA (accident, fatality, and critical injury rates) have been studied through 1987. Current FARS data and studies from KS, LA, 10 states and 5 countries are compared and reported. Without M/C helmet legislation, the user rate drops from 99% to 50%. South Africa has the highest motorcycle accident and associated casualty rate in the world. In dealing with the problem South Africa lags far behind other countries where rider training programmes have had astonishing success. Accident statistics, problems relating to motorcycling and protective clothing are discussed. Recommendations are made which if implemented should reduce the accident and casualty rate by at least 50%; this indicates the size of the problem in this neglected field of automotive preventive medicine.

The reconstruction of traffic accidents involving powered two-wheelers (PTWs) frequently proves to be a challenging task. A case in which a fatal head-on crash of a PTW with a small truck where only minor vehicles damage was observed but resulted in isolated fatal chest trauma is discussed here. Mopeds' (MP) limited speed gives the impression that they are safer than motorcycles (MCs), but factors other than speed may contribute to crash outcome. Much of the information available in the literature on physiological responses during Enduro motorcycling is related to heart rate (HR) and blood lactate (BLa). The aim of this work was to investigate the hemodynamic changes that occur during a 10-min session of Enduro motorcycling. A 48-year-old male presented with a painful right knee after a motorcycle accident. Peripheral pulsations in the lower right leg were absent. X-ray indicated a dislocation of the knee. Additional CT-scan revealed a dissection of the popliteal artery. Surgical exploration revealed extensive vascular, neurological and ligamentar damage.

Death and injuries from riding motorcycles have been well documented. We describe a case of pyogenic granuloma that we feel was indirectly related to a motorcycle accident. Clinicians should be familiar with both the diagnosis and treatment of this entity.
