As summer is upon us, we thought a discussion of tennis leg would be beneficial to our fellow clinicians. Tennis leg is a relatively common clinical condition, classically manifesting as acute, sports-related pain in the mid-calf. First described in 1883, the pathogenesis has been debated--tennis leg was first attributed to rupture of the plantaris tendon, though more recent investigations have implicated rupture of the medial head of the gastrocnemius at its myotendinous junction. For simplicity, many authors use the term tennis leg to describe all such acute muscle injuries in the superficia

All muscles can be injured during sports activities, certain specific muscles in the lower extremities are particularly vulnerable. The spectrum of these injuries is wide. Muscle strain injuries frequently involve polyarticular muscles which contain a high percentage of fast-twitch fibres (Type II). Disruption of muscle fibres occurs near the myotendinous junction. In most cases, the diagnosis can be based upon history and physical examination. Ultrasound and Magnetic Resonance Imaging can be useful to determine the severity of a specific injury. The treatment of these injuries is basically co

The muscle-tendon junction (MTJ) is a specialised interface between muscle and tendon and transmits muscle-generated force to the tendon. The MTJ is particularly vulnerable to injuries compared to muscle and tendon and becomes more injury prone with age. Despite its clinical importance, the mechanisms driving MTJ ageing and age-related functional deterioration remain poorly understood. In this study, young (3-month-old) and old (23-month-old) male mice were used to provide the first comprehensive three-dimensional characterisation of age-related structural and cellular changes at the mouse Ach

CASE: We report on the rare occurrence of first extensor compartment tendon injury at the myotendinous junction after fractures of the radial styloid and both-bone forearm in a healthy 59-year-old man. Here, a novel technique for repair of this unique injury and an additional proposed mechanism of tendon rupture are discussed.
CONCLUSION: We advocate for the suspicion of tendon rupture in the first dorsal compartment with fractures of the radial styloid and both-bone forearm. Tubularization and transfer of the abductor pollicis longus and extensor pollicis brevis tendons to the brachioradiali

PURPOSE: It has been promoted that disturbance of ocular proprioception may play a role in the pathogenesis of concomitant strabismus and other types of oculomotor anomalies. The aim of the study was to obtain knowledge about how surgical foreshortening of the myotendinous region potentially affects the proprioceptors that resides in this area of the muscles and to test the hypothesis that avoiding disruption of ocular proprioceptors result in a more favorable long term postoperative result.
PATIENTS AND METHODS: The distal end of the lateral and medial rectus muscles from patients with manif

: Injuries to the hamstring muscle are common among athletes, especially strains at the myotendinal junction that respond well to nonoperative treatment. Proximal hamstring avulsion injuries can be severely debilitating, and the role of endoscopic treatment for these injuries has not been explored. This article describes the case report of a 16-year-old girl who was diagnosed with incomplete proximal hamstring avulsion showing no improvement despite extensive nonoperative treatment. The patient was treated by endoscopic repair in the prone position using a novel technique. Her preoperative pai

PURPOSE: The purpose of our study was to compare biomechanically a long head biceps tenodesis using an all soft tissue biceps sling technique versus an interference screw technique.
METHODS: Six paired fresh frozen shoulder specimens were separated into 2 groups. One group used an all soft tissue biceps sling technique for tenodesis. The other group used the interference screw technique for subpectoral tenodesis of the long head biceps tendon. Specimens in both groups were sequentially loaded for 200 cycles, and the difference between the initial and final displacements were recorded. Specime

BACKGROUND: Muscle strains are one of the most common complaints treated by physicians. High-force lengthening contractions can produce very high forces resulting in pain and tissue damage; such strains are the most common cause of muscle injuries. The hamstring muscles are particularly susceptible as they cross two joints and regularly perform lengthening contractions during running. We describe a patient with return to full function after a large hamstring tear.
CASE DESCRIPTION: We report the case of a 26-year-old man who presented 1 year after a noncontact, left-sided proximal hamstring t

The chaetognaths have a multilayered epidermis, which is not covered by cuticle, except in the head region. Two kinds of cells are found in the epidermis: the filament-rich cells, adjacent to the basement membrane, and superficial cells, which are filament poor. The filament-rich cells, which are linked by gap junctions and columnar junctions, are highly developed in the collarette region, which joins the head and the trunk. As elsewhere in the epidermis these cells are covered by the filament poor cells which are linked by zonulae adhaerentes, gap junctions and septate junctions. The filament

Snapping knee syndrome on the medial side is rare. Here, we report the case of a patient with snapping knee syndrome of the sartorius with knee osteoarthritis. A large osteophyte at the posteromedial femoral condyle impinged on the sartorius myotendinous junction, causing painless snapping. The patient was successfully treated with osteophyte removal and total knee arthroplasty while preserving the tendon. Hence, tendon release or resection to treat snapping syndrome is not always necessary if the underlying cause can be eliminated. Furthermore, we found that while tendon tension is important

The myotendinous junction (MTJ) is a critical interface connecting skeletal muscle and tendon, responsible for transmitting contractile forces and ultimately enabling musculoskeletal movement. Due to its complex architecture, the MTJ is particularly susceptible to injury under conditions of excessive stretching, high-impact loading, aging and neuromuscular disorders such as muscular dystrophies. Despite its significant physiological role, research on the MTJ remains limited, primarily due to the challenges associated with obtaining human tissue samples. This limitation underscores the urgent n

BACKGROUND: The fascia has been demonstrated to represent a potential force transmitter intimately connected to the underlying skeletal muscle. Sports-related soft tissue strains may therefore result in damage to both structures.
PURPOSE: To elucidate the prevalence of connective tissue lesions in muscle strain injury and their potential impact on return-to-play (RTP) duration.
STUDY DESIGN: Systematic review; Level of evidence, 3.
METHODS: Imaging studies describing frequency, location, and extent of soft tissue lesions in lower limb muscle strain injuries were identified by 2 independent

Mutation in the dystrophin gene results Duchenne Muscular Dystrophy (DMD), an X-linked fatal neuromuscular disorder. Dystrophin deficiency can be compensated by upregulation of utrophin, an autosomal homologue of dystrophin. But the expression of utrophin in adults is restricted to myotendinous and neuromuscular junctions. Therefore utrophin upregulation throughout the muscle fiber can only be achieved if we understand regulatory mechanisms behind its expression. Utrophin-A 5'UTR mediated repression of translation was reported earlier. In this article, we present evidences of two transcript va

HYPOTHESIS: Greater peak torque and higher myotendinous density at the ankle are associated with a more plate-like architecture at the distal tibia.
METHODS: In this cross-sectional study, women and men ≥ 50 years old with no metal implants, reconstructive surgery, muscular dystrophies, or tendinopathies in any leg were recruited by convenience. Isometric ankle dorsi-plantar flexion and inversion-eversion peak torques were measured using dynamometry. HR-pQCT distal tibia scans were completed. Both assessments were completed on the same day on the non-dominant leg. Integral and trabecular vBMD

Tears in the flexor digitorum superficialis muscle belly are rare injuries with one single case reported in the literature. In this article, we describe the first case of muscle tear of the flexor digitorum superficialis resulting from a wakeboarding trauma. The wakeboarder presented with pain localized in the anterior forearm, and early symptoms of carpal tunnel syndrome. Ultrasonography demonstrated an intramuscular hematoma of the flexor digitorum superficialis myotendinous junction, which compressed the median nerve. After conservative treatment, the patient was asymptomatic and returned t

BACKGROUND AND OBJECTIVE: Muscle injuries are one of the main daily problems in sports medicine, football in particular. However, we do not have a reliable means to predict the outcome, i.e. return to play from severe injury. The aim of the present study was to evaluate the capability of the MLG-R classification system to grade hamstring muscle injuries by severity, offer a prognosis for the return to play, and identify injuries with a higher risk of re-injury. Furthermore, we aimed to assess the consistency of our proposed system by investigating its intra-observer and inter-observer reliabil

Dystrophin is essential for muscle health: its sarcolemmal absence causes the fatal, X-linked condition, Duchenne muscular dystrophy (DMD). However, its normal, spatial organization remains poorly understood, which hinders the interpretation of efficacy of its therapeutic restoration. Using female reporter mice heterozygous for fluorescently tagged dystrophin (DmdEGFP), we here reveal that dystrophin distribution is unexpectedly compartmentalized, being restricted to myonuclear-defined sarcolemmal territories extending ~80 µm, which we called "basal sarcolemmal dystrophin units (BSDUs)." These

Competence in muscle biopsy evaluation is a core component of neuropathology practice. The practicing neuropathologist should be able to prepare frozen sections of muscle biopsies with minimal artifacts and identify key histopathologic features of neuromuscular disease in hematoxylin and eosin-stained sections as well as implement and interpret a basic panel of additional histochemical, enzyme histochemical, and immunohistochemical stains. Important to everyday practice is a working knowledge of normal muscle histology at different ages, muscle motor units, pitfalls of myotendinous junctions,

Morphological studies of the diaphragm have provided a detailed view of its architecture, which consists of parallel skeletal muscle fibers and a central ring of neuronal innervation that includes neuromuscular junction (NMJ) units. NMJs are disease-vulnerable synapses; thus, analysis of the NMJ is essential to understand its function in both healthy and disease-related conditions. The diaphragm was analyzed by spatial proteomics and 115 proteins were enriched at the NMJ. Comparison of the protein signatures of the NMJ and myotendinous junction (MTJ) revealed 31 shared proteins, suggesting par

Hamstring strains account for 25% to 30% of all muscle strains and are an exceedingly common injury in the athletic population. Although proximal hamstring avulsion injuries occur less commonly than strains at the myotendinous junction, they are more severe and debilitating. Proximal hamstring avulsions do not respond well to conservative treatment and are more likely to require surgical intervention. Surgical repair of proximal hamstring avulsions is indicated when the injury fails to respond to conservative treatment, in cases of osseous avulsion with retraction, and in cases of tearing of a
