Percutaneous nephrolithotomy in the spinal cord injury population.

We evaluated 23 male spinal cord injury patients who underwent percutaneous nephrolithotomy for the success rate of stone removal and the incidence of operative complications. There were 18 quadriplegic and 5 paraplegic patients, and 5 had bilateral procedures. Of the kidneys 7 had staghorn calculi, 8 had pelvic and caliceal combinations, 6 had large multiple caliceal stones, 4 had large (more than 2.5 cm.) pelvic stones and 3 had pelvic stones less than 2.5 cm. Placement of a nephrostomy tube and stone extraction were performed as a single procedure with the use of general anesthesia in all b

Urinothorax: rare complication of percutaneous nephrolithotomy. Case report.

Urinothorax is an unusual cause of pleural effusion. To describe a case with urinothorax secondary to urinary system trauma. 41-year-old male readmitted to our hospital 16 days after a right percutaneous nephrolithotomy with fever, shortness of breath, chest pain and multiloculated pleural effusion confirmed by CT scan, resolved with surgical treatment. Readmitted twenty-one days later with recurrent empyema and diagnosis of reno-pleural fistula, treated with new thoracotomy and double J and urinary catheter placement. The suspected diagnosis and the multidisciplinary approach allowed to solve

Are Routine Laboratory Investigations Necessary Following Percutaneous Nephrolithotomy?

OBJECTIVE: To determine how effective routine postoperative blood work is in identifying complications after percutaneous nephrolithotomy (PCNL), the gold standard treatment for large volume stone disease. Although major complication rates are low, hemorrhagic and sepsis-related complications are serious and can occur. Routine post-PCNL complete blood count is routinely performed by most endourologists but may be a low-value practice. METHODS: A retrospective review was performed of all PCNL procedures at our center over a 3-year period. Patient demographic, stone characteristics and postoper

Factors Associated With Postoperative Infection After Percutaneous Nephrolithotomy.

Numerous studies have investigated risk factors for the development of postoperative infection in percutaneous nephrolithotomy (PCNL) patients. Herein, we describe our meta-analysis of the risk factors for the prediction of post-PCNL infectious complications. We searched electronic databases using a combination of the terms percutaneous nephrolithotomy, risk factors, infection, and sepsis. The primary outcome was post-PCNL infection as defined by fever >38°C or sepsis as defined by the Sepsis Consensus Definition Committee. Risk factors for infection in each study were identified and included

Delayed massive haemothorax 10 days following percutaneous nephrolithotomy.

A 56-year-old man presented with massive right haemothorax 10 days following percutaneous nephrolithotomy (PCNL) for complex, large-bulk, right renal stones. Antiplatelet medication started following coronary stenting 7 months ago was stopped 5 days prior and resumed 2 days following surgery. Stones were cleared through two tracts, one supracostal, with placement of ureteral stent but no nephrostomy. He was discharged the next day with an unremarkable chest X-ray. He developed cough and fever after 1 week. Three days later he presented with acute dyspnoea, blood pressure of 100/60 mm Hg, pulse

Endovascular management of vascular complications after percutaneous nephrolithotomy.

BACKGROUND: Bleeding is one of the most common and most important complications of percutaneous nephrolithotomy (PCNL), which is mainly controlled with conservative treatment options. Transcatheter arterial embolization is required in less than 1 % of the patients undergoing PCNL. There are only a few studies about endovascular treatment of vascular complications of PCNL. The purpose of this study was to evaluate renal arterial complications of PCNL and treatment outcomes with endovascular coil embolization. PATIENTS AND METHODS: This retrospective study evaluated 16 patients who underwent en

Tubeless percutaneous nephrolithotomy: first 200 cases in Israel.

BACKGROUND: Tubeless percutaneous nephrolithotomy is defined as PCNL without postoperative nephrostomy tubes. It is reported to reduce postoperative pain, hospital stay and recovery time. To date the procedure has been reserved for selected patients. OBJECTIVES: To assess our initial experience in extending the implementation of tubeless PCNL without preoperative patient selection. METHODS: All consecutive PCNLs performed during 2004-2008 were evaluated. Tubeless PCNL was performed when residual stones, bleeding and extravasation were excluded intraoperatively. Staghorn stones, stone burden,

Modified "tubeless" percutaneous nephrolithotomy using a tail-stent.

INTRODUCTION: "Tubeless" percutaneous renal surgery has previously been described at our institution. Avoiding external nephrostomy tube drainage significantly decreases the pain and morbidity of percutaneous renal surgery. However, internal drainage with a double-J ureteral stent still results in bothersome bladder symptoms, especially in younger male patients. In addition, removal of the stent requires uncomfortable office cystoscopy. We report a technical enhancement of our "tubeless" technique that uses a novel tail-stent for internal drainage. TECHNICAL CONSIDERATIONS: A tubeless approac

Fluid absorption during percutaneous nephrolithotomy: does it matter?

BACKGROUND AND PURPOSE: Large amounts of irrigating fluid are used during percutaneous nephrolithotomy (PCNL). This use may be associated with migrating calculus debris, infection, and fluid absorption. This study evaluated the presence of fluid absorption during PCNL, its clinical and biochemical significance, and maneuvers to reduce it. PATIENTS AND METHODS: Fluid absorption during PCNL was evaluated in 148 patients by estimating the expired breath ethanol concentration. Factors thought to affect the amount of fluid absorbed were studied, including the amount of irrigating fluid used, the n

Antimicrobial Prophylaxis for Percutaneous Nephrolithotomy: Contemporary Practice Patterns.

Background: Updated in 2019, the American Urological Association's (AUA) Best Practice Statement on Urologic Procedures and Antimicrobial Prophylaxis outlines prophylaxis for percutaneous nephrolithotomy (PCNL). Recent studies have challenged these recommendations. We hypothesized that endourologists do not routinely follow the AUA's statement on antibiotic use during PCNL and assessed their prescribing patterns. Methods: A 24-question survey was distributed to members of the Endourological Society. The primary outcome was adherence to the AUA's recommendations. Two multiple logistic regressio

Unilateral diaphragmatic paresis following supracostal post-percutaneous nephrolithotomy.

Unilateral acquired diaphragmatic paresis is a known complication of thoracic surgeries. Direct mechanical injury to the phrenic nerve during surgery results in this complication. However its occurrence post-percutaneous nephrolithotomy (PCNL) has not been described. We report a 43-year-old man who underwent prone PCNL for endourological management of left complete staghorn calculus. Access via the 10th left intercostal space, under fluoroscopy, was carried out to remove the calculus. Post-operative, the routine chest radiograph revealed left hemidiaphragmatic blunting. Computerized tomography

Gallbladder injury after Percutaneous Nephrolithotomy: A case report.

UNLABELLED: Percutaneous Nephrolithotomy (PCNL) is the gold standard treatment of large kidney stones that has rare but serious complications such as gallbladder injury. In this article, a 62-year-old woman with a right lower kidney who was scheduled for PCNL was presented. The patient had acute abdomen on the first day after PCNL, so explorative laparotomy and cholecystectomy were done. The patient was discharged five days after surgery in good condition. SECTION HEADINGS: Endourology/Stone Disease.

Death due to atypical urinothorax following percutaneous nephrolithotomy.

Urinothorax is a cause of pleural effusion that is often missed due to its perceived rarity. Here, we present a case of urinothorax secondary to percutaneous nephrolithotomy resulting in lung collapse and death. This case highlights urinothorax as a cause of death, its biochemical profile, and diagnostic features on autopsy. A 39-year-old female presented to the ED with low back pain and dysuria. Abdominal and pelvic CT showed a large staghorn calculus in the right kidney which was then treated with percutaneous nephrolithotomy. On post-operative day (POD) 1, the patient was febrile, had decre

Mobile augmented reality for computer-assisted percutaneous nephrolithotomy.

PURPOSE: Percutaneous nephrolithotomy (PCNL) plays an integral role in treatment of renal stones. Creating percutaneous renal access is the most important and challenging step in the procedure. To facilitate this step, we evaluated our novel mobile augmented reality (AR) system for its feasibility of use for PCNL. METHODS: A tablet computer, such as an iPad[Formula: see text], is positioned above the patient with its camera pointing toward the field of intervention. The images of the tablet camera are registered with the CT image by means of fiducial markers. Structures of interest can be sup
