Cranioplasty in acoustic neuroma surgery.

OBJECTIVES: To measure the incidence of postoperative headaches after retrosigmoid resections of acoustic neuromas and to evaluate the impact of cranioplasty on the prevention and management of these headaches. STUDY DESIGN: A prospective evaluation was performed on 30 consecutive patients who underwent a cranioplasty after retrosigmoid excision of their acoustic neuroma. The results were compared with 30 historical control patients who underwent the same procedure but did not have reconstruction with a cranioplasty. The patients were evaluated by review of office records and via telephone qu…

[Neurootologic study of acoustic neuroma].

OBJECTIVE: To summarize the outcome of neurootologic examination of acoustic neuroma and study of diagnostic significance of neurootologic examination on acoustic neuroma. METHOD: 13 patients were examined with pure tune test, acoustic immittance, electronystagmography, acoustic brainstem evoked response, electrocochleogram, otoacoustic emissions and facial electroneurography. RESULT: All patients had subjective hearing loss. One patient had normal pure tone audiogram, other 4 patients had normal pure tone test at the frequency < 2000 Hz. The outcome of acoustic immittance is various. 5 pati…

Pneumocephalus after acoustic neuroma surgery.

OBJECTIVE: The study was conducted to describe the diagnosis and management of pneumocephalus after acoustic tumor resection. STUDY DESIGN: The study design was a retrospective chart review. SETTING: The study was conducted as a tertiary otologic referral to the senior authors' practices (PAF, MDA). PARTICIPANTS: Three patients operated on for acoustic tumor in St. Vincent's General Hospital and the Scottish Hospital, Sydney, Australia, were studied. INTERVENTION: Observation in both cases and surgery in a case in which tension pneumocephalus was progressive were performed. MAIN OUTCOME M

Suboccipital surgery for acoustic neuroma.

A retrospective study was performed on 106 consecutive patients with an acoustic neuroma who underwent suboccipital surgery for the first time between 1980 and 1992. Complete follow-up data were available for all the patients, including regular imaging up to 1 January 1994. Incomplete removal of the tumours was chosen in preference to radical surgery in a large proportion of the patients, to avoid the risk of post-operative morbidity. The recurrence rate in this series was 20%. Comparison of the results of suboccipital surgery to those obtained using other surgical approaches, while taking the…

[Our experience in neuroma surgery].

Our 24-year experience with acoustic neuroma surgery is reported. Three approaches were used in 34 patients: translabyrinthine (11 cases), transtemporal (2 cases), and retrosigmoid (21 cases). Prolonged facial paralysis occurred in 27% of patients with the translabyrinthine approach and in 15% with the retrosigmoid approach. Facial nerve monitoring was used in all operations with the retrosigmoid approach. In the translabyrinthine approach, 5 patients had CSF leak, 2 required secondary surgery, and 2 developed meningitis. With the retrosigmoid approach, 5 patients had CSF leak, 2 required seco…

Acoustic neuroma standard of care.

Patients who have undergone acoustic neuroma surgery require expert, comprehensive nursing care. This article includes a standard of care and tables about neurological assessment, potential complications, home safety and discharge instructions. A holistic, multidisciplinary approach can enable nurses to promote the recovery of acoustic neuroma patients and ultimately improve their outcomes.

[Early diagnosis of acoustic neuroma].

The diagnostic findings in 31 patients (32 ears) with acoustic neuromas are reported. In patients suspicious of acoustic neuroma, case history and clinical examination are important. In our material of 31 patients with acoustic neuromas, hearing loss was presented in 94% (slow onset 87%, sudden onset 10% and fluctuating 3%), tinnitus in 91%, vestibular symptoms (dizziness or vertigo) in 62%. It should be stressed that in patients with unilateral hearing loss (or tinnitus) or vestibular symptoms, it is necessary to exclude the presence of an acoustic neuroma. In addition, the trigeminal never w…

MR imaging of stump neuroma.

Painful stump neuromas in the postamputation patient are difficult to diagnose and treat. We report a case in which magnetic resonance allowed precise preoperative diagnosis and localization of such a lesion. Magnetic resonance, with its ability for long axis imaging in the extremity, is ideally suited for the workup of stump neuromas.

Otosclerosis masking coexistent acoustic neuroma.

The coexistence of an acoustic neuroma with otosclerosis can prove to be a great diagnostic dilemma. In such cases, the diagnosis of acoustic neuroma is usually delayed while more common causes of sensorineural hearing loss associated with otosclerosis or its surgery are considered. Any asymmetric progression of a sensorineural loss or shift in discrimination after stapes surgery, whether sudden or gradual, should provoke suspicion of a second pathologic process. These include perilymph fistula, labyrinthine otosclerosis or ischemia, and acoustic neuroma. The use of auditory brainstem response…

Conservative treatment of Morton's neuroma.

Morton's metatarsalgia (plantar digital neuritis) is a syndrome caused by the development of a neuroma in a digital nerve just proximal to its point of bifurcation. Severe neuralgic pain in the distal part of the sole is referred to the corresponding two toes. The treatment of this syndrome with the injection of a local anesthetic and methylprednisolone is discussed.

[Subarachnoid hemorrhage in acoustic neuroma].

A 32-year-old patient is reported with two subarachnoid bleedings occurring at an interval of 5 years. The cause of bleeding was a richly vascularized neurinoma of the acoustic nerve with focal necrosis. Operation was successful.

Neuroma formation following digital amputations.

One hundred consecutive patients with 144 digital amputations performed at the University of Colorado Health Sciences Center between 1978 and 1980 were retrospectively reviewed. In this group, four patients demonstrated painful amputation stumps. Two of these were treated by surgical excision of neuromas and two patients, who had more vague complaints, were treated by desensitization which decreased the sensitivity of their stumps and allowed them to return to work. It is felt that this low incidence of painful neuromas and amputation stumps is due to the positive postoperative effort to send …
