Genetic defects in molecules expressed at the neuromuscular junction (NMJ) cause congenital myasthenic syndromes (CMSs), which are characterized by muscle weakness, abnormal fatigability, amyotrophy, and minor facial anomalies. Muscle weakness mostly develops under 2 years but is also sometimes seen in adults. Mutations identified to date include (i) muscle nicotinic acetylcholine receptor (AChR) subunits, (ii) rapsyn that anchors and clusters AChRs at the neuromuscular junction, (iii) agrin that is released from the nerve terminal and induces AChR clustering by stimulating the downstream LRP4…

Autoantibodies directed against voltage- or ligand-gated ion channels and their associated proteins at the neuromuscular junction give rise to a family of neurological autoimmune diseases. Antibodies to acetylcholine receptors or muscle-specific kinase present on the postsynaptic muscle membrane are associated with different forms of myasthenia gravis (MG). Antibodies to the presynaptic voltage-gated potassium and calcium channels are responsible for acquired neuromyotonia and Lambert-Eaton myasthenic syndrome (LEMS), respectively. The patients respond to immunotherapies and their plasma can t…

This is a review of the neurophysiological techniques used clinically to demonstrate abnormalities of neuromuscular transmission. The use of these tests in myasthenia gravis, Lambert-Eaton myasthenic syndrome, congenital myasthenic syndromes, and other conditions with disturbed neuromuscular transmission is presented.

The effect of subcutaneous administration of pyridostigmine or neostigmine for 7 to 15 days on neuromuscular transmission has been studied in the rat phrenic nerve-hemidiaphragm preparation. The quantal contents of end-plate potentials at different stimulus rates and the amplitude and frequency of miniature end-plate potentials were compared with those of untreated controls. The rate of release of acetylcholine quanta at high stimulus rates, and the frequency of miniature end-plate potentials, were reduced by pretreatment with both pyridostigmine and neostigmine. Presynaptic effects differed i

In myasthenia gravis and Lambert-Eaton myasthenic syndrome, antibodies against ion channels are clearly related to pathogenesis; however, in the inflammatory myopathies, a link to disease causation is not as evident, except as a marker of autoimmunity. Autoantibody testing in patients with neuromuscular junction and skeletal muscle disorders is useful in diagnosis and, in some diseases, prognosis. This review briefly summarizes methods of autoantibody testing and then proceeds to a clinician-friendly guide to autoantibody testing of neuromuscular junction disorders, hyperexcitability syndromes…

The neuromuscular junction (NMJ), the connection between nerve and muscle, is an essential anatomic site that warrants careful consideration. Dysfunction at this junction often leads to skeletal muscle weakness with a multitude of clinical manifestations. Disorders of the NMJ primarily encompass myasthenia gravis, Lambert-Eaton syndrome, botulism, and organophosphate poisoning. This article will review diseases and disorders of the NMJ, their clinical presentations, and approach to management.

The objective of this work was to extend the evaluation of a recently proposed method for estimating neuromuscular jitter within motor unit potential (MUP) trains extracted from muscles suffering neuromuscular junction disease. The method detects, within the MUP duration, "single-fiber" intervals that have likely been produced by single muscle fibers. Jitter is then estimated between pairs of these "single-fiber" intervals using an algorithm which incorporates the traditional mean consecutive difference (MCD) parameter. Electromyographic (EMG) recordings from facial muscles of 15 patients with…

The neuromuscular junction (NMJ) is an essential synapse whose loss is a key hallmark of the neurodegenerative disease spinal muscular atrophy (SMA). Here, we show that activity of the SMA-determining SMN protein in the assembly of U7 small nuclear ribonucleoprotein (snRNP)-which functions in the 3'-end processing of replication-dependent histone mRNAs-is required for NMJ integrity. Co-expression of U7-specific Lsm10 and Lsm11 proteins selectively enhances U7 snRNP assembly, corrects histone mRNA processing defects, and rescues key structural and functional abnormalities of neuromuscular patho…

Channelopathies, neuromuscular junction disorders, and myopathies represent multiple mechanisms by which toxins can affect the peripheral nervous system. These toxins include ciguatoxin, tetrodotoxin, botulinum toxin, metabolic poisons, venomous snake bites, and several medications. These toxins are important to be aware of because they can lead to serious symptoms, disability, or even death, and many can be treated if recognized ear.

BACKGROUND: human exposure to mixtures of chemicals of toxicological interest, typically found in industrial contaminated sites (ICSs), has been associated with a broad range of different health outcomes. Deprived population groups endure most of the burden of disease and premature death associated to the exposure to those pollutants. Characterising the impacts on health of an ICS is a challenging process. Currently the two main methodological approaches used are Human Health Risk Assessment (HHRA) and Environmental Epidemiological (EE) studies.
OBJECTIVES: review existing guidance and scient

The neuromuscular junction (NMJ) is a synapse between a motor neuron and skeletal muscle. The contraction of skeletal muscle is controlled by the neurotransmitter acetylcholine (ACh), which is released from the motor nerve terminal. To achieve efficient neuromuscular transmission, acetylcholine receptors (AChRs) must be densely clustered on the muscle membrane of the NMJ. Failure of AChR clustering is associated with disorders of neuromuscular transmission such as congenital myasthenic syndromes (CMS) and myasthenia gravis (MG). Motoneuronal agrin and muscle-specific receptor tyrosine kinase (…

OBJECTIVE: Neuromuscular junctions, which are responsible for voluntary motor function, are the chemical synapses between motor neurons and skeletal muscle fibers. In this study, we performed a scientometric analysis that can be used to construct and visualize networks of neuromuscular junction model (NMJM) using bibliometric methods.
MATERIALS AND METHODS: A literature survey for NMJM was conducted using "TS= (neuromuscular junction model)," including literature titles, abstracts, and keywords, under the Web of Science Core Collection (WoSCC). Document production was analyzed by document cit…

In amyotrophic lateral sclerosis (ALS), Guillain-Barré syndrome (GBS), and neuromuscular junction disorders, three mechanisms may lead, singly or together, to respiratory emergencies and increase the disease burden and mortality: (i) reduced strength of diaphragm and accessory muscles; (ii) oropharyngeal dysfunction with possible aspiration of saliva/bronchial secretions/drink/food; and (iii) inefficient cough due to weakness of abdominal muscles. Breathing deficits may occur at onset or more often along the chronic course of the disease. Symptoms and signs are dyspnea on minor exertion, ortho…
