BACKGROUND: Noma is a multifactorial and multibacterial opportunistic infection that initially causes necrotic gingivitis but rapidly spreads to the nearby orofacial tissue resulting in sloughing and severe deformation of the facial structures. The majority of cases are seen in young children under the age of 6 years. Noma is strongly associated with poverty, malnutrition and immunosuppression, and is often preceded by severe systemic infections such as measles and malaria. Only few cases of noma infection in adults have been described.
CASE REPORT: We present here a case report with a 32-yea…

OBJECTIVES: Noma is a gangrenous affliction that destroys the soft and hard tissue of the face. It affects exclusively young children from developing countries. Although the risk factors (malnutrition and debilitating diseases) are known, the cause remains a topic of debate. Etiological studies have tried to identify bacterial or viral agents responsible for the illness. We examine for the first time the possibility of a link between this illness and the social practices surrounding childbirth.
METHODS: This study took place in Niger, where we conducted 18 ethnographic interviews with mothers…

The authors describe a case of Noma or Cancrum Oris, an oral gangrenous disease, features more frequently found in children from developing countries. The clinical features, its ethiopathogenesis, and its particular link with different geographic and economic areas of the world, its clinical evolution as well as surgical treatment are all discussed. Underlined is the functional and organic aspect of this disease, in particular the distortion of the face, which commonly involves the full thickness of the cheek skin and bone, mandibular ankylosis and craniofacial dismorphisms, and the modern app…

This study showed that impoverished Nigerian children at risk for cancrum oris (noma) had significantly reduced plasma concentrations of zinc (< 10.8 micromol/L), retinol (< 1.05 micromol/L), ascorbate (< 11 micromol/L), and the essential amino acids, with prominently increased plasma and saliva levels of free cortisol, compared with their healthy counterparts. The nutrient deficiencies, in concert with previously reported widespread viral infections (measles, herpesviruses) in the children, would impair oral mucosal immunity. We postulate, subject to additional studies, that evolution of the …

A 68-year-old Japanese woman with sepsis developed a gangrene on her left cheek from a small wound in contact with a gastric tube. Klebsiella pneumoniae was cultured from the sputum, wound, and the blood and was assumed to be responsible for this condition, possibly through thrombosis of an artery.

INTRODUCTION: Noma is a severe gangrenous orofacial disease affecting malnourished children aged 2-6 years in extreme poverty, with 85-90% mortality if untreated. Recently classified as a Neglected Tropical Disease, it is managed mainly with empirical broad-spectrum antibiotics. Antimicrobial resistance (AMR) threatens to undermine treatment, yet AMR remains largely absent from Noma research and policy. This review examines AMR prevalence and drivers in Noma, identifies knowledge and policy gaps, and proposes recommendations.
METHODS: We reviewed literature on Noma microbiology and documented…

In clinical settings, understanding the markers and clinical signs of infection is critical for timely diagnosis and treatment. However, a point-of-care diagnostic test is lacking for noma, a debilitating orofacial infectious disease. This retrospective study reviewed electronic medical records (EMRs) and paper medical records (PMRs) of noma patients from Facing Africa (235 EMRs), Yekatit 12 Hospital (68 PMRs), and Project Harar (33 PMRs) in Ethiopia to identify essential infection markers and clinical presentations of acute noma. The review identified seven acute noma patients aged 4-9 years.…

Fifth-generation (5G) communication technologies, such as millimeter wave communication, massive multiple-input-multiple-output and non-orthogonal-multiple-access (NOMA) are playing a pivotal role in promoting the modern applications of the Internet-of-Things. Using non-orthogonal resource allocation, NOMA can increase spectrum efficiency and achieve wide connectivity with low transmission delay and signaling cost. Despite the high potential of NOMA in 5G communications, NOMA is susceptible to a pilot contamination attack (PCA), in which an attacker resents the same pilot signals as authorized…

Noma (Cancrum Oris), a recent addition to the WHO list of neglected tropical diseases, is a severe, rapidly progressing necrotizing disease of the oral cavity and facial complex with a case fatality rate of 90% if untreated. Active disease is common among children between two and six years in Sub-Saharan Africa while noma sequelae may be seen in individuals at any age. Though most cases have been reported in northern Nigeria, little research is available on the incidence of noma and its clinical presentation in this region using comprehensive data. Therefore, this study aims to estimate the in…

Noma horses are native Japanese horses. Health checkups revealed that many Noma horses developed dermatitis during summer, which subsided in winter. Seasonal development and signs of itching, suggestive of allergic dermatitis, were observed. In this study, allergen-specific IgE was measured using blood samples collected from 15 Noma horses in summer and winter to identify allergens highly associated with dermatitis. The presence of dermatitis in the subject individuals was recorded during blood sample collection. White blood cell and eosinophil counts, serum total IgE concentration, and serum …

Noma (cancrum oris) is an orofacial gangrene, which during its fulminating course causes progressive and mutilating destruction of the infected tissues. The disease occurs mainly in children with malnutrition, poor oral hygiene and debilitating concurrent illness. Noma is well documented in the literature, but because most patients do not report to a doctor until the disease is at an advanced stage, its onset and progression remain a mystery. This case report, with a survey of recent relevant literature, highlights the different stages in the development of tissue necrosis, including onset and…

RATIONALE: Cancrum oris, also known as noma, is a rare and rapidly progressing gangrenous infection affecting the oral cavity, commonly seen in malnourished children. We discuss the clinical presentation, diagnostic dilemma and management in a 54-year-old male human immunodeficiency virus-positive patient with oral ulceration clinically resembling squamous cell carcinoma (SCC).
PATIENT CONCERNS: The patient had severe oral ulceration with pain and difficulty eating food.
DIAGNOSIS: Histopathological examination was misleading as it revealed features akin to early invasion of SCC. Immunohisto…

BACKGROUND: Noma (cancrum oris) is a severe, rapidly progressing and necrotising neglected tropical disease affecting the mouth and face. The risk of noma incidence and geographical distribution at small administrative divisions remain poorly characterised. This study aimed to model the spatial distribution of noma incidence in Nigeria and identify high-incidence risk areas for targeted community actions.
METHODS: This study used data from the only noma specialist centre in northern Nigeria (Noma Children's Hospital; Sokoto, Nigeria) to estimate and map areal incidence risk. We used data from…

This case report describes a 4-year-old girl in Chad with noma, also called cancrum oris. This acute gangrenous stomatitis has a combined morbidity-mortality rate that can reach 70%. It occurs worldwide but is most common in sub-Saharan Africa in children aged 2 to 16 years. Its pathogenesis is uncertain, but several bacteria including Fusobacterium necrophorum, Prevotella intermedia and Pseudomonas aeruginosa may be responsible for the development of noma, which develops over the oral lesions these bacteria cause. Poverty is its most important risk factor. Due to its rapid course and high let…

Noma is a gangrenous disease of the orofacial region that leads to severe facial tissue destruction and is a significant cause of death among children. With the advent of modern antibiotics and improved nutrition, children with noma may survive into adulthood, but must face the challenge of undergoing repair of the sequelae of noma. This report describes a case of bony fusion of the maxilla and mandible in a 28-year-old female patient, which was a sequelae of a childhood case of noma.
