Relative growth of the anulus fibrosus and nucleus pulposus in human and kitten fetuses.

Growth in volume of the anulus fibrosus (AF) and nucleus pulposus (NP) was quantified using serial histological sections of human and kitten fetuses. Fetal intervertebral discs were studied that had clearly outlined AF and NP. Regression equations were calculated and graphs plotted by microcomputer. An increase in surface areas of these intervertebral structures was also recorded; however, volume was a better indicator of relative growth than was surface area. The AF volume of the fetal human increased more in proportion to the intervertebral disc than it did for the fetal kittens. There was s…

Charge-engineered microparticles for targeted delivery of RhoA activator to nucleus pulposus.

Intervertebral disc degeneration (IVDD) involves dysregulated mechanobiology of nucleus pulposus (NP) cells, marked by disrupted F-actin organization and inflammation-driven extracellular matrix (ECM) catabolism. Restoring actomyosin contractility and cytoskeletal integrity can blunt ECM degradation. Employing a constitutive Rho activator, CN03, reinstates F-actin organization via the RhoA-ROCK pathway and rescues NP cells from cytokine-driven degeneration. However, its constitutive RhoA activity is short-lived, which demands sustained, spatially targeted NP delivery. In this study, an enginee…

The role of nucleus pulposus progenitor cells in intervertebral disc degeneration and regeneration.

Intervertebral disc degeneration (IDD) is a significant public health issue, commonly associated with symptoms such as low back pain (LBP), and poses a substantial burden on both society and affected families. The pathogenesis of IDD primarily involves the depletion and impaired function of nucleus pulposus (NP) cells, as well as alterations in the extracellular matrix (ECM) composition. Current clinical management strategies, including conservative and surgical treatments, aim primarily to alleviate symptoms rather than reverse the pathological process or promote structural regeneration of th…

Gait abnormalities and inflammatory cytokines in an autologous nucleus pulposus model of radiculopathy.

STUDY DESIGN: The authors investigated gait abnormalities and mechanical hypersensitivity associated with invertebral disc herniation in a rat model of radiculopathy. Further evaluation involved assessing how nucleus pulposus (NP) injury affected systemic cytokine expression and molecular changes at the dorsal root ganglion (DRG). OBJECTIVE: The objective of this work was to describe the gait and behavioral changes in an animal model of disc-herniation induced radiculopathy. A second objective included examining how these functional changes correlated with neuroinflammation and autoreactive l…

The response of the nucleus pulposus of the lumbar intervertebral discs to functionally loaded positions.

STUDY DESIGN: Asymptomatic volunteers underwent magnetic resonance imaging to investigate how different positions affect lumbar intervertebral discs. OBJECTIVE: To quantify sagittal migration of the lumbar nucleus pulposus in 6 functional positions. SUMMARY OF BACKGROUND DATA: Previous studies of the intervertebral disc response in the sagittal plane were limited to imaging of recumbent positions. Developments of upright magnetic resonance imaging permit investigation of functional weight-bearing positions. METHODS: T2-weighted sagittal scans of the L1-L2 to L5-S1 discs were taken of 11 vol…

HACE1 alleviates intervertebral disc degeneration by inhibiting ferroptosis in nucleus pulposus cells.

HACE1 (HECT domain and ankyrin repeat containing E3 ubiquitin protein ligase 1) is an antioxidant gene that plays a key role in activating a … summary, HACE1 alleviates the progression of IDD by inhibiting oxidative stress and ferroptosis in nucleus pulposus cells. Its protective effect may be related to the activation of the Nrf2/ARE signaling pathway. In conclusion, our results support that the antioxidant gene HACE1 could serve as a novel potential target for treating IDD.

Resveratrol alleviates intervertebral disc degeneration by regulating ferroptosis of nucleus pulposus cells.

Intervertebral disc degeneration (IVDD) is a major contributor to chronic low back pain, yet its molecular mechanisms remain incompletely understood. Emerging evidence suggests that ferroptosis, an iron-dependent form of regulated cell death, may play a key role in the degeneration of nucleus pulposus cells (NPCs). However, its involvement in IVDD has not been systematically explored. Resveratrol (Res), a natural polyphenol with antioxidant and cytoprotective effects, shows therapeutic potential, but its regulatory role in ferroptosis during IVDD is unclear. In this study, bulk transcriptome a…

GPR4 promotes ferroptosis in nucleus pulposus cells via MAPK-dependent mitophagy suppression.

Inflammatory diseases are known to lower the pH value of the extracellular microenvironment. However, the role of proton-activated G protein … GPR4 expression was significantly elevated in tert-butyl hydroperoxide (TBHP)-treated degenerative nucleus pulposus cells. Proteomic sequencing further demonstrated that GPR4 overexpression altered gene expression related to the mitochondrial energy metabolism and ferritin homeostasis. Notably, knockdown of GPR4 via shRNA (shGPR4) enhanced the MAPK-mediated mitop …

Nucleus pulposus cell-associated PLAU promotes intervertebral disc degeneration through HIPPO pathway.

Intervertebral disc degeneration (IDD) is the most common and critical pathological basis for various spinal disorders. In this study, we investigated how plasminogen activator urokinase (PLAU) promotes IDD progression through the HIPPO pathway. Bioinformatics analysis showed that PLAU is a hub gene in IDD. Both in vitro and in vivo experiments confirmed that PLAU can promote the apoptosis of nucleus pulposus chondrocytes. PLAU activates the HIPPO signaling pathway by increasing the phosphorylation levels of mammalian ste20-like kinases 1/2 (MST1/2), large tumor suppressor 1/2 (LATS1/2), and y…

Evaluation of alginate/poly(vinyl alcohol)/BaSO4 hydrogels for nucleus pulposus regeneration.

Intervertebral disc degeneration is a major contributor to lower back pain worldwide, underscoring the urgent need for effective, minimally invasive regenerative therapies. One of the critical challenges in nucleus pulposus (NP) replacement lies in developing injectable hydrogels with optimal gelation behavior, radiopacity, mechanical properties, and biocompatibility. This study investigates the influence of varying concentrations of Na2HPO4 (0.3%, 0.4%, 0.5%) and BaSO4 (1%, 1.5%) in ALG/PVA hydrogels through a series of gelation time measurements, radiopacity analysis, mechanical testing, and…

RIPK1-targeted therapy alleviates intervertebral disc degeneration via inhibiting nucleus pulposus PANoptosis.

Intervertebral disc degeneration (IVDD) is a major contributor to lumbar diseases, including low back pain, herniation, and stenosis. Despite significant efforts, there have been limited improvements in treatments to alleviate IVDD. The nucleus pulposus (NP) is a crucial component of the intervertebral disc (IVD), responsible for secreting aggrecan, collagen II, and other extracellular matrix components. Programmed cell death (PCD) of NP cells is believed to play a central role in IVDD. RIPK1 is a key mediator of PCD and recently reported PANoptosis, playing essential role in kidney injury, ar…

MINK1 deficiency stimulates nucleus pulposus cell pyroptosis and exacerbates intervertebral disc degeneration.

Intervertebral disc (IVD) degeneration, induced by aging and irregular mechanical strain, is highly prevalent in the elderly population, serving as a leading cause of chronic low back pain and disability. Evolving evidence has revealed the involvement of nucleus pulposus (NP) pyroptosis in the pathogenesis of IVD degeneration, while the precise regulatory mechanisms of NP pyroptosis remain obscure. Misshapen/Nck-interacting kinase (NIK)-related kinase 1 (MINK1), a serine-threonine protein kinase, has the potential to modulate the activation of NLRP3 inflammasome, indicating its pivotal role in…

Viscoelastic hydrogels regulate adipose-derived mesenchymal stem cells for nucleus pulposus regeneration.

Low back pain is a leading cause of disability worldwide, often attributed to intervertebral disc (IVD) degeneration with loss of the functional nucleus pulposus (NP). Regenerative strategies utilizing biomaterials and stem cells are promising for NP repair. Human NP tissue is highly viscoelastic, relaxing stress rapidly under deformation. However, the impact of tissue-specific viscoelasticity on the activities of adipose-derived stem cells (ASC) remains largely unexplored. Here, we investigated the role of matrix viscoelasticity in regulating ASC differentiation for IVD regeneration. Viscoela…

Human iPS cell-derived cartilaginous tissue spatially and functionally replaces nucleus pulposus.

The loss of nucleus pulposus (NP) precedes the intervertebral disk (IVD) degeneration that causes back pain. Here, we demonstrate that the implantation of human iPS cell-derived cartilaginous tissue (hiPS-Cart) restores this loss by replacing lost NP spatially and functionally. NP cells consist of notochordal NP cells and chondrocyte-like NP cells. Single cell RNA sequencing (scRNA-seq) analysis revealed that cells in hiPS-Cart corresponded to chondrocyte-like NP cells but not to notochordal NP cells. The implantation of hiPS-Cart into a nuclectomized space of IVD in nude rats prevented the de…

Differentiation of human induced pluripotent stem cells into nucleus pulposus-like cells.

BACKGROUND: Intervertebral disc (IVD) degeneration is characterized by an early decrease in cellularity of the nucleus pulposus (NP) region, and associated extracellular matrix changes, reduced hydration, and progressive degeneration. Cell-based IVD therapy has emerged as an area of great interest, with studies reporting regenerative potential for many cell sources, including autologous or allogeneic chondrocytes, primary IVD cells, and stem cells. Few approaches, however, have clear strategies to promote the NP phenotype, in part due to a limited knowledge of the defined markers and different…

Spinal balloon nucleoplasty: a hypothetical minimally invasive treatment for herniated nucleus pulposus.

Low back pain is the most common cause of disability under the age of 45. The annual incidence of back pain is estimated to be 5% and the lifetime prevalence is 80%. Majority of the patients with persistent symptoms are suffering from radiculopathy that is mainly caused by a herniated nucleus pulposus (HNP). HNP can heal spontaneously due to spontaneous resorption. Besides pressure nucleus pulposus, without any compression, may induce similar changes when applied epidurally to the nerve roots. Nevertheless, combination of chronic mechanical compression and application of nucleus pulposus cause…

Cultured, autologous nucleus pulposus cells induce functional changes in spinal nerve roots.

STUDY DESIGN: Nerve conduction velocity in pig nerve roots was assessed after application of various preparations of nucleus pulposus and control. OBJECTIVE: To study whether cultured nucleus pulposus cells could reduce nerve conduction velocity after epidural application. SUMMARY OF BACKGROUND DATA: It is known that nucleus pulposus applied epidurally may reduce the nerve conduction velocity of the adjacent nerve roots and that this reduction seems to be related to the cells of the nucleus pulposus. METHODS: Nucleus pulposus cells and fibroblasts were cultured for 3 weeks, and various prep…
