Reducing hazardous chemical exposures in the neonatal intensive care unit: a new role for nurses.

A wide range of toxic chemicals have been found in the umbilical cord blood of newborns, indicating the potential for health risks from chemical exposure that begin in utero. The neonatal intensive care unit (NICU) setting may also have potential chemical exposures that create health risks. Given the extreme vulnerability of this patient population, it is critical to minimize unnecessary hazardous chemicals. Neonatal intensive care unit nurses have an important role to play in making the NICU as safe as possible for their patients and themselves. This article will focus on the human health eff

Hypoplastic left heart syndrome: attitudes among pediatric residents and nurses towards fetal and neonatal management.

OBJECTIVE: To determine the attitudes of pediatric residents and nurses towards fetal/neonatal management of hypoplastic left heart syndrome (HLHS), and their basis. METHODS: Pediatric residents and nurses from three cardiac centers completed a survey with hypothetical scenarios in which their own fetuses or newborns had HLHS. While Institution A performs many HLHS surgeries, Institution C performs very few. RESULTS: A total of 43% of residents and 50% of nurses would terminate an affected pregnancy. More experience (4 to 7 years, p = 0.04; >7 years, p = 0.05) and employment at institution C

Neonatal intensive care nurses' assessment of preterm infants' pain and sedation: inter-rater reliability of the neonatal pain, agitation, and sedation scale.

BACKGROUND: Timely and accurate assessment of pain and sedation in newborns is essential for effective management. Therefore, neonatal pain and sedation assessment remains a key global issue in neonatal intensive care unit (NICU) nursing practice. This study examined the inter-rater reliability of Neonatal Pain Agitation, and Sedation Scale (N-PASS) scores among NICU patients. METHODS: This prospective observational study assessed agreement among 19 NICU nurses and two independent researchers who completed 190 observations from 82 preterm infants. Each evaluator rated N-PASS independently and

Alarm Fatigue and Missed Nursing Care Among Neonatal and Paediatric Intensive Care Nurses: A Cross-Sectional Study.

BACKGROUND: Intensive care units (ICUs) are high-technology environments where patient safety is monitored through various devices. However, the high frequency of alarms can lead to alarm fatigue, especially in neonatal and paediatric ICUs due to rapid physiological fluctuations. This condition may decrease nurses' attention and lead to missed nursing care. AIMS: The primary aim of this study was to determine the levels of alarm fatigue and missed nursing care, along with the underlying reasons for missed care, among neonatal and paediatric intensive care nurses. The secondary aim was to exam

Employing a Nutrition Nurse in Neonatal Intensive Care Unit Improved Nutrition and Growth Outcomes in Preterm Neonates.

BACKGROUND: Nutrition nurses are clinical nurse specialists with knowledge and experience in nutrition support. We aimed to investigate the nutrition and growth outcomes after employment of a nutrition nurse in our level III neonatal intensive care unit (NICU). METHODS: A retrospective cohort study including preterm neonates < 34 weeks and < 2000 g were conducted. Nutrition and growth outcomes of infants in pre-nutrition-nurse and post-nutrition-nurse periods were compared. Primary outcome was presence of postnatal growth retardation (PGR) at term-equivalent age (body weight < 10 percentile a

Neonatal Intensive Care Unit Nurses' End-of-Life Care Experience: Becoming a More Empathetic Professional Caregiver.

BACKGROUND: Few studies have examined how empathy develops in neonatal intensive care units (NICUs) as nurses experience negative outcomes in end-of-life care delivery (eg, burnout, moral conflict, and psychological distress). Various characteristics of neonatal care contribute to frequent infant deaths and nurse emotional strain, rendering knowledge of how empathy evolves essential for improving end-of-life care and NICU nurse well-being nationwide. PURPOSE: To explore the experiences of NICU nurses providing end-of-life care for infants in South Korea, focusing on the impact of recurrent ex

Neonatal Resuscitation Practices and Associated Factors Among Nurses in Tanzanian Delivery Suites: A Cross-Sectional Study.

BACKGROUND: Neonatal deaths contribute significantly to under-five mortality, with most deaths seen in sub-Saharan Africa (SSA). Birth asphyxia is the leading cause of these deaths, but it is preventable with effective resuscitation. In SSA, including Tanzania, inadequate neonatal resuscitation practice by health care providers has been identified as a significant contributor to neonatal mortality. AIM OF THE STUDY: The study aimed to assess the current practice of neonatal resuscitation and the associated factors among nurses located in the delivery suites. METHODOLOGY: A hospital-based cro

Capacity building of nurses providing neonatal care in Rio de Janeiro, Brazil: methods for the POINTS of care project to enhance nursing education and reduce adverse neonatal outcomes.

BACKGROUND: Increased survival of preterm infants in developing countries has often been accompanied by increased morbidity. A previous study found rates of severe retinopathy of prematurity varied widely between different neonatal units in Rio de Janeiro. Nurses have a key role in the care of high-risk infants but often do not have access to ongoing education programmes. We set out to design a quality improvement project that would provide nurses with the training and tools to decrease neonatal mortality and morbidity. The purpose of this report is to describe the methods and make the teachin

Postpartum and neonatal nursing care during the 2009 H1N1 influenza pandemic.

We describe select influenza infection control policies and practices related to postpartum and newborn care during the 2009 H1N1 pandemic. In an online survey of obstetric and neonatal nurses, significantly more nurses indicated a written hospital policy supporting each of the practices during versus before the pandemic. The two practices least often implemented were temporary separation of healthy newborns from ill mothers (37.7 percent) and testing newborns for influenza virus infection if signs of influenza were observed (31.4 percent). Presence of written hospital policies increased imple

Favism by proxy in nursing glucose-6-phosphate dehydrogenase-deficient neonates.

Two nursing neonates deficient in glucose-6-phosphate dehydrogenase developed severe hyperbilirubinemia despite phototherapy. Mothers of both the infants had recently eaten fava beans. The hemolytic triggers found in fava beans may have been absorbed by the mothers and excreted in their breast milk. Carboxyhemoglobin determination performed on one of the infants reflected ongoing hemolysis.

Impact of bundle nursing intervention on jaundice index and development in neonates with hyperbilirubinemia.

Neonatal hyperbilirubinemia (NHB) management often suffers from fragmented conventional nursing approaches, which may compromise treatment efficacy and delay intervention. This prospective randomized controlled trial evaluated the effects of a novel bundle nursing intervention on jaundice index and developmental outcomes in 100 NHB neonates. Participants were randomized to a research group (n=50, bundle nursing) or a control group (n=50, standard care). The research group exhibited significantly accelerated meconium passage and jaundice resolution compared to the control group. Additionally, m
